Current practices regarding middle turbinate resection among otolaryngologists.

OBJECTIVES: Resection of the middle turbinate (MT) during endoscopic sinus surgery (ESS) has been a controversial topic among otolaryngologists for many years. Some studies advocate resection and have shown improved outcomes postoperatively, while studies favoring preservation show a decreased incidence of postoperative complications. The current practice pattern regarding this subject is unknown. The goal of this study was to learn the current practice of MT resection during ESS among otolaryngologists. METHOD: We performed an electronic anonymous survey of practicing otolaryngologists. RES

Otolaryngologists' radiological assessment of nasal septum deviation symptomatology.

OBJECTIVES: Nasal Septal Deviation (NSD) is one of the most common causes of nasal obstruction. This study aims to further examine the clinical utility of imaging assessment in the workup and management of symptomatic nasal septal deviation, across all levels of medical training. STUDY DESIGN: Cross-sectional survey. METHODS: CT scans of 10 confirmed NSD patients and 36 healthy controls (HC) were mixed and emailed through anonymous REDCap surveys to otolaryngologists in the US. The HC had no reported sinonasal obstruction symptoms-NOSE (NSD: 62.2 ± 12.5; HC: 5.69 ± 5.99, p < 0.05); SNOT-22 (

Epiphora: A Structured Diagnostic Approach for the Otolaryngologist.

INTRODUCTION: Epiphora, or excessive tearing, has historically been predominantly managed by ophthalmologists. However, as there is substantial overlap between ophthalmic and otolaryngologic anatomy and pathology that may contribute to epiphora, patients with this symptom often present to otolaryngologists as well. Similarly, the differential of underlying etiologies for epiphora extends well beyond nasolacrimal duct obstruction, encompassing a wide array of conditions, each of which may be managed primarily by one or the other field. Judicious clinical evaluation of epiphora in each case is t

Idiopathic Intracranial Hypertension: Implications for the Otolaryngologist.

Idiopathic intracranial hypertension (IIH) is a triad of headaches, visual changes, and papilledema in the absence of a secondary cause for elevated intracranial pressure. There is an association with obesity, and the incidence is rising in parallel with the obesity epidemic. Sometimes these patients present to an otolaryngologist with complaints like tinnitus, dizziness, hearing loss, and otorrhea or rhinorrhea from cerebrospinal fluid leak. IIH diagnosis in conjunction with neurology and ophthalmology, including neuroimaging and lumbar puncture with opening pressure, is key to managing of th

The role of the otolaryngologist in the evaluation and management of headaches.

BACKGROUND: Headaches are commonly evaluated in otolaryngology and often represent a diagnostic dilemma. This review addresses rhinogenic headache as well as trigeminal neuralgia and migraine, both of which can masquerade as sinus headache and whose management increasingly involves otolaryngology intervention. Discussion considers diagnostic criteria and novel therapies and derives an algorithm for clinical decision-making. DATA SOURCES: OVID MEDLINE, Cochrane Library, and Google Scholar databases. METHODS: A literature search was performed to identify relevant articles published in the past

Red ear syndrome: Perspectives for the otolaryngologist.

Red ear syndrome (RES) is an unusual, often unrecognized condition that has rarely been reported in the otolaryngology literature despite the presence of symptoms and associated disorders of relevance to the otolaryngologist. When present, RES may have diagnostic value when uncertainties arise in cases of sinus headache and referred otalgia. Despite the appearance of a distinctive red ear, as well as symptoms and associated disorders relating to otolaryngology, discussion of RES is generally limited to neurology-based publications. A red ear is typically the one constant finding in RES while c

Tumid lupus: An unexpected diagnosis for the otolaryngologist.

Tumid lupus is an uncommon variant of lupus erythematosus. Patients present with subcutaneous lesions. Ophthalmic literature reports disease manifestation as orbital inflammation. Autoimmune serology is often negative. Without a high index of suspicion, the diagnosis is easily overlooked delaying treatment. Tumid lupus is not significantly discussed in the Otolaryngologic literature. Here we present a Case Report of a male who initially presented to Ophthalmology with unilateral orbital complaint of eyelid puffiness. Orbital biopsy and subsequent biopsy of his submental skin lesions ultimately

Hereditary angioedema: a clinical review for the otolaryngologist.

Hereditary angioedema (HAE) is a relatively rare genetic disorder that is usually characterized by either low levels of C1 esterase inhibitor (C1-INH) or the presence of dysfunctional C1-INH. It can present with relatively mild and self-limiting symptoms, but it is also potentially fatal; the most common cause of death is asphyxiation secondary to edema of the upper airway. The diagnosis of HAE, especially in the emergency situation, is not straightforward. HAE must be distinguished from several other types of angioedema that require different management approaches. Management approaches inclu

Paediatric eosinophilic oesophagitis presenting to the otolaryngologist.

INTRODUCTION: The prevalence of eosinophilic oesophagitis is increasing. A Pubmed search for 'eosinophilic oesophagitis' and 'eosinophilic esophagitis' yielded 345 publications since 1976. Only seven were in otolaryngology journals.1-7 Patients typically present with dysphagia, vomiting, dyspepsia or food impaction and are therefore usually referred to a paediatric gastroenterologist; otolaryngologists are not usually involved in management. A missed diagnosis may result in oesophageal stricture. METHODS: Two patients, aged two and four years, were referred to the paediatric otolaryngology de

[The role of the otolaryngologist in the treatment of children with clefts].

Cheilognathopalatoschises are a complex therapeutic problem not yet completely solved and demanding collaboration of a number of medical and paramedical branches. In a therapeutic team, an otorhinolaryngologist has an important role since he has to deal with impaired hearing, impaired nasal breathing, as well as with disturbed speech development. On the basis of an analysis of the state of hearing in children who were treated at the University Clinic for Otorhinolaryngology and Cervicofacial Surgery in Ljubljana, it was found that more than 76% of them suffered from hearing loss of different l

The otolaryngologist's role in the craniofacial anomalies team.

The otolaryngologist has a multifaceted role on the craniofacial anomalies team. The diagnosis and treatment of hearing disorders, both congenital and acquired; maintenance of communication through adequate speech and hearing; management of the airway and deglutition; and reconstructive surgery of the head and neck are the primary responsibilities to children and adults with craniofacial anomalies. The complete evaluation and treatment of these disorders can be both very challenging and very rewarding.

The otolaryngologist and flexible fiberoptics: photographic considerations.

The ability to document normal and pathological anatomy is of paramount concern to all medical and surgical practitioners. Until recently the otolaryngologist was forced to resort to the use of clumsy and expensive procedures, frequently requiring the use of general anesthesia in order to obtain endoscopic photodocumentation. The author describes a simple technique, using standard flexible fiberoptic nasopharyngolaryngoscopes, standard light sources, and standard photographic processing which enable the otolaryngologist to obtain high quality transparencies and prints of the tympanic membrane,

[The otolaryngologist's role in evaluating diving fitness].

Scuba diving and other modes of device-supported diving are popular activities that can be especially demanding and hazardous for people with preexisting physical conditions. Due to the high ambient pressure, the temperature differences, and potential unpredictable events, which have manifold effects on the organism, diving carries a high risk of life-threatening disease. A special risk is present if the body does not readily equalize air pressure changes. Therefore, prior to diving, all divers should undergo detailed education regarding the physical principles of the sport as well as specific
