Symptoms and otoscopic signs in bilateral and unilateral acute otitis media.

OBJECTIVE: Bilateral acute otitis media (AOM) is considered more severe than unilateral AOM, and several guidelines recommend more active treatment and/or follow-up of bilateral AOM. We studied whether bilateral AOM is a clinically more severe illness than unilateral AOM by comparing symptoms and otoscopic signs between bilateral and unilateral AOM. METHODS: Two hundred thirty-two children aged 6 to 35 months diagnosed with AOM were eligible. We surveyed the symptoms with a structured questionnaire and recorded the otoscopic signs systematically. RESULTS: Ninety-eight children had bilateral

Application of Patient-operated Otoscope in Diagnosing Patulous Eustachian Tube Dysfunction.

OBJECTIVE: A patulous Eustachian tube (PET) is characterized by persistent Eustachian tube patency, leading to symptoms, such as aural fullness, autophony, and audible breathing. The diagnostic criteria include both characteristic symptoms and objective findings; however, intermittent symptom presentation often prevents confirmation during clinic visits, leading to underdiagnosis. We aimed to evaluate the clinical utility of a patient-operated portable digital otoscope for confirming the diagnosis of PET. STUDY DESIGN: Prospective observational study. SETTING: The study was conducted between

Light field otoscope design for 3D in vivo imaging of the middle ear.

We present a light field digital otoscope designed to measure three-dimensional shape of the tympanic membrane. This paper describes the optical and anatomical considerations we used to develop the prototype, along with the simulation and experimental measurements of vignetting, field curvature, and lateral resolution. Using an experimental evaluation procedure, we have determined depth accuracy and depth precision of our system to be 0.05-0.07 mm and 0.21-0.44 mm, respectively. To demonstrate the application of our light field otoscope, we present the first three-dimensional reconstructions o

Is otoscopy reliable? A structured teaching method to improve otoscopic accuracy in trainees.

Otoscopy is an important skill for primary care physicians and otologists. Until now, training has been by repeated exposure to patients with ear disease. Structured instruction in how to assess an ear has not previously been reported. Not-diseased ears and those with varying types of chronic (suppurative) otitis media were chosen to be photographed as this is an important condition to be able to diagnose and in which pneumatic otoscopy has no role. Two sets of 30 slides of equal difficulty were shown to 10 trainees, one before and one after structured teaching. The overall error rate fell fro

3D Otoscope: toward an extra diagnostic dimension for middle-ear related issues.

We present an industrially manufacturable digital otoscope designed to measure the three-dimensional shape of the tympanic membrane while being compatible with standard examination procedures. The system employs optical profilometry and overcomes the limitations of previous work by integrating single-shot surface reconstruction strategies-from classical Fourier profilometry to modern deep learning methods-thereby eliminating the need for multi-pattern projection and, in the latter case, even phase unwrapping. A full-field three-dimensional surface map enables the assessment of the tympanic mem

Improved Methodology for Liquid Delivery to the Mouse Lung: Intubation using a Consumer Otoscope.

Consistent delivery of liquids, including potential therapies, to the lungs is critical for modeling pulmonary disease and pre-clinical therapeutic development. However, current methods used in mice -such as oropharyngeal aspiration by the tongue-pull method- can result in variable delivery. Here, we describe the use of an affordable, mobile phone-linked, lighted otoscope to visualize the glottis and permit direct inoculation into the airway. We detail an optimized staging technique with adequate lighting and positioning aids. How intubation can be reliably conducted using a 20 G flexible cath

Optical coherence tomography otoscope for imaging of tympanic membrane and middle ear pathology.

SIGNIFICANCE: Pathologies within the tympanic membrane (TM) and middle ear (ME) can lead to hearing loss. Imaging tools available in the hearing clinic for diagnosis and management are limited to visual inspection using the classic otoscope. The otoscopic view is limited to the surface of the TM, especially in diseased ears where the TM is opaque. An integrated optical coherence tomography (OCT) otoscope can provide images of the interior of the TM and ME space as well as an otoscope image. This enables the clinicians to correlate the standard otoscopic view with OCT and then use the new infor

Use of Otoscope as a Diagnostic and Therapeutic Aid in Umbilical Pilonidal Sinus: A Novel Technique.

INTRODUCTION: Umbilical disorders are frequently encountered in general surgical practice. Although the sacrococcygeal region is the most common site for pilonidal sinus disease, it can be seen occasionally in periumbilical area. Treatment is mostly conservative for umbilical pilonidal sinus in contrast to the sacrococcygeal sinus where it is always surgical. In the era of endoscopy and minimally invasive surgery, we describe the use of otoscope as a novel technique for the diagnosis and treatment of umbilical pilonidal sinus. SUBJECT AND METHOD: In this prospective study, patients with a cli

Multiwavelength fluorescence otoscope for video-rate chemical imaging of middle ear pathology.

A common motif in otolaryngology is the lack of certainty regarding diagnosis for middle ear conditions, resulting in many patients being overtreated under the worst-case assumption. Although pneumatic otoscopy and adjunctive tests offer additional information, white light otoscopy has been the main tool for diagnosis of external auditory canal and middle ear pathologies for over a century. In middle ear pathologies, the inability to avail high-resolution structural and/or molecular imaging is particularly glaring, leading to a complicated and erratic decision analysis. Here, we propose a nove

An unusual otoscopic finding associated with a type II first branchial cleft anomaly.

OBJECTIVE: We report an interesting case involving a child with a branchial cleft anomaly with two fistulous tracts, one of which was associated with an unusual otoscopic finding. CASE REPORT: A seven-year-old girl presented with an apparent type II first branchial cleft cyst after an acute infection. Parotidectomy and excision of the tract were performed, with subsequent development of pre-auricular swelling three months later. Further surgery was performed to remove a second duplication anomaly of the external auditory canal. Otomicroscopy showed a fibrous band arising from the wall of the

A comparison of in-person examination and video otoscope imaging for tympanostomy tube follow-up.

The objective of this study was to determine if video otoscope still images (640 x 480 pixel resolution) of the tympanic membrane following surgical placement of tympanostomy tubes are comparable to an in-person microscopic examination. Forty patients having undergone tympanostomy tube placement in both ears were independently examined in-person by two otolaryngologists and imaged using a video otoscope and telemedicine software package. The two physicians later reviewed images at 6 and 12 weeks. Physical examination findings and diagnosis were documented and compared for their concordance usi

Parametric Physically Grounded Rendering of Otoscopic Morphology for Synthetic Medical Image Generation.

The tympanic membrane (TM) is a thin, semi-transparent structure whose morphology and optical appearance provide important diagnostic cues. In the early stages of middle-ear pathology, subtle shape and compliance alterations may precede overt clinical signs, making them valuable early indicators of disease. Such structural changes are difficult to assess reliably using conventional (micro-)otoscopy, which lacks quantitative depth information and is operator-dependent. Data-driven monocular image analysis could enable quantitative assessment of TM geometry and compliance, but the limited availa
