Temporal and cross-site validation of an AI system for self-harm detection.

Adequate self-harm surveillance is a key part of suicide prevention. Our previous research demonstrated that an artificial intelligence (AI)-based system could effectively detect self-harm in emergency department triage notes. However, the system was developed using data from a single hospital, raising concerns about its generalisability. Here, we aim to validate the system prospectively and externally to better understand its portability across hospitals. We leveraged emergency department data from two Australian hospitals, with free-text triage notes manually annotated for self-harm. The AI

Characterization of the histone methyltransferase PRDM9 using biochemical, biophysical and chemical biology techniques.

PRDM proteins have emerged as important regulators of disease and developmental processes. To gain insight into the mechanistic actions of the PRDM family, we have performed comprehensive characterization of a prototype member protein, the histone methyltransferase PRDM9, using biochemical, biophysical and chemical biology techniques. In the present paper we report the first known molecular characterization of a PRDM9-methylated recombinant histone octamer and the identification of new histone substrates for the enzyme. A single C321P mutant of the PR/SET domain was demonstrated to significant

A machine learning approach to conglomerate multi-domain features of cardiac aging.

AIMS: Owing to the breadth of complex and highly dimensional clinical data associated with ageing, integration of multiple health domains is needed towards determining cardiac outcomes of older adults. We designed a machine learning (ML) approach to conglomerate multi-domain data and identify determinants of cardiac function in older adults. METHODS AND RESULTS: We applied a structured ML pipeline including data pre-processing, feature selection, and model development using Random Forest, Gradient Boosting, XGBoost, LightGBM, and support vector machine. Model performance was evaluated using s

Cross-Protocol Domain Gap in Internet of Things Intrusion and Anomaly Detection: An Empirical Internet Protocol-to-Bluetooth Low Energy Study of Domain-Adversarial Training.

Intrusion and anomaly detectors trained on Internet Protocol (IP) traffic are increasingly deployed in heterogeneous IoT environments where Bluetooth Low Energy (BLE) links coexist with IP networks. We quantify the cross-protocol domain gap in an IP → BLE transfer setting under unsupervised domain adaptation (UDA), where target labels are unavailable for training and model selection. Using 14 lightweight window-level statistics and leakage-aware splits, we benchmark classical baselines and alignment methods (CORAL and MMD) against domain-adversarial neural networks (DANNs). Under random window

M3FusionNet: Cross-cohort multimodal prediction of breast cancer biomarkers.

BACKGROUND: Accurate prediction of breast cancer biomarkers - including oestrogen receptor (ER), progesterone receptor (PR), human epidermal growth factor receptor 2 (HER2), molecular subtypes (PAM50), and the proliferation marker Ki-67 (MKI67) - is essential for treatment stratification and prognosis. Existing computational approaches predominantly rely on single-cohort data, limiting their clinical generalisability. METHODS: We propose M3FusionNet (Multi-modal, Multi-scale, Multi-source Fusion Network), a comprehensive multimodal deep learning framework integrating haematoxylin-and-eosin (H

Transfer Learning-Based Quantification of Mitotic Activity and Its Prognostic and Molecular Associations in Lung Squamous Cell Carcinoma.

Background: Automated assessment of mitotic activity may provide a reproducible marker of tumor proliferation, but computational mitosis recognition remains challenging because of cross-domain variability. We developed a transfer-learning framework to quantify mitotic activity in lung squamous cell carcinoma (LUSC) and investigated its clinical and molecular associations. Methods: A ResNet-34 classifier was trained using MIDOG source-domain data and adapted to TCGA-LUSC diagnostic whole-slide images. The target cohort included 478 patients and 512 whole-slide images. Tumor regions and mitotic

Auditory oddball hypoactivation in schizophrenia.

Individuals with schizophrenia (SZ) show aberrant activations, assessed via functional magnetic resonance imaging (fMRI), during auditory oddball tasks. However, associations with cognitive performance and genetic contributions remain unknown. This study compares individuals with SZ to healthy volunteers (HVs) using two cross-sectional data sets from multi-center brain imaging studies. It examines brain activation to auditory oddball targets, and their associations with cognitive domain performance, schizophrenia polygenic risk scores (PRS), and genetic variation (loci). Both sample 1 (137 SZ

Phenome-wide Association Analysis of Substance Use Disorders in a Deeply Phenotyped Sample.

BACKGROUND: Substance use disorders (SUDs) are associated with a variety of co-occurring psychiatric disorders and other SUDs, which partly reflects genetic pleiotropy. Polygenic risk scores (PRSs) and phenome-wide association studies are useful in evaluating pleiotropic effects. However, the comparatively low prevalence of SUDs in population samples and the lack of detailed information available in electronic health records limit these data sets' informativeness for such analyses. METHODS: We used the deeply phenotyped Yale-Penn sample (n = 10,610 with genetic data; 46.3% African ancestry, 5

Association of Genetic Risk Variants With Attention-Deficit/Hyperactivity Disorder Trajectories in the General Population.

IMPORTANCE: Attention-deficit/hyperactivity disorder (ADHD) is a heritable neurodevelopmental disorder that shows clinical and genetic overlap with other childhood neurodevelopmental disorders. Levels of ADHD symptoms typically decline across childhood and adolescence, although they remain elevated for some individuals. The determinants of symptom persistence and decline are not yet fully understood. OBJECTIVES: To test the hypothesis that genetic risk variant load for ADHD (indexed by polygenic risk scores [PRS]), but not for other psychiatric disorders, is associated with population-based A

Effects of a long-term home-based exercise training programme using minimal equipment vs. usual care in COPD patients: a study protocol for two multicentre randomised controlled trials (HOMEX-1 and HOMEX-2 trials).

BACKGROUND: Exercise training is an important component of pulmonary rehabilitation (PR) programmes in chronic obstructive pulmonary disease (COPD), but the great majority of COPD patients who would benefit from PR never follow such programmes or fail to maintain exercise training after PR completion. Against this background, we developed an exercise training programme that requires minimal equipment and can be implemented long-term in the patient's home-setting. The aims of the HOMEX-1 and HOMEX-2 trials are to assess the effectiveness of this home-based exercise training programme in two gro

Development of an Interpretable QSAR Model for Predicting Coagulation Factor XIIa Inhibitors Using Ensemble Machine Learning.

Background/Objective: Activated coagulation factor XII (FXIIa) is a component of the contact activation pathway and a pharmacologically relevant target in contact-system-associated processes. In this study, scaffold-aware and interpretable machine-learning QSAR models were developed for human FXIIa activity. Methods: Bioactivity records for the human single protein target CHEMBL2821 were retrieved from ChEMBL release 37. Modeling was restricted to exact IC50 measurements from assays explicitly referring to FXIIa, Factor XIIa, or activated Factor XII. Median-consolidated pIC50 values and two-di

Nuclear localization of PRDM9 and its role in meiotic chromatin modifications and homologous synapsis.

Developmental progress of germ cells through meiotic phases is closely tied to ongoing meiotic recombination. In mammals, recombination preferentially occurs in genomic regions known as hotspots; the protein that activates these hotspots is PRDM9, containing a genetically variable zinc finger (ZNF) domain and a PR-SET domain with histone H3K4 trimethyltransferase activity. PRDM9 is required for fertility in mice, but little is known about its localization and developmental dynamics. Application of spermatogenic stage-specific markers demonstrates that PRDM9 accumulates in male germ cell nuclei

An auditable pipeline for fuzzy full-text screening in systematic reviews: integrating contrastive semantic highlighting and LLM judgment.

Full-text screening is the major bottleneck of systematic reviews (SRs), particularly in domains such as population health modelling of noncommunicable diseases (NCDs), where decisive eligibility information is scattered across long and heterogeneous full texts. In this methodological study, we introduce a scalable and auditable pipeline that reframes inclusion and exclusion as a fuzzy decision process. We evaluate the approach within the Population Health Modelling Consensus Reporting Network (POPCORN) and benchmark it against statistical and crisp baselines. Articles are parsed into overlapp

Associations between quality of health care and clinical outcomes in patients with rheumatic and musculoskeletal diseases: a rehabilitation cohort study.

BACKGROUND: The quality of provided health care may be an important source of variation in rehabilitation outcomes, increasing the interest in associations between quality indicators (QIs) and improved patient outcomes. Therefore, we examined the associations between the quality of rehabilitation processes and subsequent clinical outcomes among patients with rheumatic and musculoskeletal diseases (RMDs). METHODS: In this multicentre prospective cohort study, adults with RMDs undergoing multidisciplinary rehabilitation at eight participating centres reported the quality of rehabilitation after

Association of Polygenic Risk Score With Cognitive Decline and Motor Progression in Parkinson Disease.

IMPORTANCE: Genetic factors have a well-known influence on Parkinson disease (PD) susceptibility. The largest genome-wide association study (GWAS) identified 26 independent single-nucleotide polymorphisms (SNPs) associated with PD risk. Among patients, the course and severity of symptom progression is variable, and little is known about the potential association of genetic factors with phenotypic variance. OBJECTIVE: To assess whether GWAS-identified PD risk SNPs also have a cumulative association with the progression of cognitive and motor symptoms in patients with PD. DESIGN, SETTING, AND
