Role of obturators and other feeding interventions in patients with cleft lip and palate: a review.

BACKGROUND: Newborns with cleft palate have a distorted maxillary arch at birth. Depending upon the type of cleft, infants suffer from a variety of problems, many of which are related to feeding difficulties. Feeding these babies is an immediate concern because there is evidence of delayed growth of children with cleft lip and palate (CLCP) compared to normal infants. Many methods have been devised to overcome these problems, including the use of special bottles, nipples, and initial obturator therapy. REVIEW: A Pub Med search was conducted using the following search terms: feeding interventi

Computer-Aided Design of a Palatal Obturator in Treating Feline Oronasal Fistula: A Case Report.

Oronasal fistulas in felines present significant clinical challenges and complicate treatment planning and intervention. This report investigates the application of the virtual twin concept and advanced digital technologies, specifically 3-dimensional (3D) scanning, computer-aided design (CAD), and additive manufacturing techniques, toward the development of a custom palatal obturator for managing a chronic oronasal fistula in an 18-month-old male cat. Despite more than 3 prior surgical attempts, the fistula remained unresolved. The approach involved the CAD of the palatal obturator. This invo

Simplified design and precautionary measures in fabrication of a feeding obturator for a newborn with cleft lip and palate.

The immediate problem to be addressed in a new born with cleft lip and palate is the interference of feeding. Here we present a case of cleft lip and palate for which a feeding obturator was made to facilitate feeding till the defect could be surgically repaired. The design was modified by placing orthodontic wire on the acrylic plate for attaching the ties of mouth mask. Different precautionary measures undertaken while making impression for infant have also been described. Feeding appliance improved the sucking ability of the infant. The attachment of mouth-mask ties help in easy retrieval o

Improvement in Quality of Life Using an Ethylene-Vinyl Acetate Palatal Obturator in a Patient with Self-Inflicted Nasal Defect and Palatal Perforation.

A 67-year-old female patient with intellectual disability presented with a self-inflicted nasal defect and palatal perforation. In 2006, she was referred to our department by a nurse from her care facility with the chief complaint of food leakage into the nasal cavity due to the palatal perforation. Her medical history revealed a long-standing habit of scratching the nasal cavity, which had persisted for more than 30 years since approximately 27 years of age. Clinical and radiographic examinations, including three-dimensional computed tomography (3D-CT), demonstrated a nasal defect, crust form

Customized protective palatal obturator for intubation in newborns in cleft lip surgery: a randomized controlled trial.

BACKGROUND: Orofacial clefts are common congenital malformations, affecting both facial aesthetics and function. Intubation in newborns with cleft lip and palate is challenging and carries a high risk of oral tissue damage. This study investigates the use of a customized protective palatal obturator (CPPO) to improve intubation safety and reduce tissue injury during cleft lip surgery. METHODS: A single-center, randomized neonatal sub-study was conducted, including 55 newborns who underwent cleft lip surgery. Patients were randomized into an intervention group (CPPO use) and a control group (s

A simple method of enhancing retention in interim hollow bulb obturator in a case of an acquired palatal defect.

Maxillary defects occur either as a result of surgical resection of malignant tumors of the nasal cavity and paranasal sinuses or of the congenital causes. Rehabilitation of the patients with maxillectomy defects presents a challenge in restoring the lost form, function and speech. Maxillary interim obturators in prosthetic reconstruction of the defects are often complicated with lack of adequate retention, stability, and support. This case report presents the simplified approach, to rehabilitate a case of sub-total maxillectomy due to squamous cell carcinoma of maxillary sinus, using a closed

Surgical reconstruction versus prosthetic obturation of extensive soft palate defects: a comparison of speech outcomes.

PURPOSE: The restoration of speech after an extensive resection of the soft palate has been a challenge faced by both prosthodontists and surgeons. Few comparisons between prosthetic rehabilitations and surgical reconstructions of large soft palate defects exist in equally matched groups of patients. The purpose of this study was to evaluate speech outcomes in patients with soft palate defects that were rehabilitated with either a pharyngeal obturator or surgical reconstruction. MATERIALS AND METHODS: Nine patients who were treated via prosthetic obturation were compared to nine patients who

Changes in Quality of Life After Secondary Closure of Palatal Defects: Prosthetic Obturation Versus Surgical Reconstruction.

BACKGROUND: The closure of palatal defects after tumor resection or irradiation is performed with either a prosthesis or autogenous tissue; however, there are no clear criteria regarding selection of the method. Thus, this study aimed to investigate the real-world situation and problems of palatal closure using prostheses, and examined patient opinion on how palatal closure using autogenous tissue improved their postoperative quality of life (QOL). METHODS: In 5 patients whose palatal defects resulted from treatment for head and neck cancer and were closed with a prosthesis, the palate was cl

A Novel Self-Retentive Obturator to Facilitate Feeding in an Infant with Cleft Palate: A Case Report and Literature Review.

Cleft lip and palate are common congenital defects with multifactorial complex etiology characterized by complete or incomplete cleft of lip, alveolus, and hard and soft palate. In infants, the main problem lies in the difficulty of suckling with frequent nasal regurgitation leading to respiratory and middle ear infections. This article presents a case report of an 8-day-old female with complete cleft of hard and soft palate who was given a novel self-retentive type feeding obturator. This obturator eliminated the need for supplementary aids to help in retention and has a simple design, is cos

Single-visit Feeding Obturator Fabrication in Infants with Cleft Lip and Palate: A Case Series and Narrative Review of Literature.

UNLABELLED: Cleft lip and palate (CLP) is one of the most common craniofacial anomaly affecting newborns. In the early years of life to survive baby requires nutrition from the mother. Lip seal of infant is affected because of cleft palate and thereby feeding is greatly compromised. As there is communication between nasal cavity and oral cavity there are more chances of aspiration of milk into the lungs. The main role of pedodontist is to fabricate a palatal obturator which facilitates feeding. In this article we have discussed fabrication of feeding obturator in single visit in infants with c

An integrated hollow bulb obturator prosthesis with a metal framework for a soft palate defect fabricated by multiple digital techniques.

The digital workflow to fabricate an integrated hollow bulb obturator prosthesis with a metal framework for a patient with soft palate defect is described. The framework was digitally designed with an open lattice denture base connector to facilitate the assembly of the hollow bulb obturator and printed with titanium. A functional impression of the palatopharyngeal area was made, and an integrated 3-dimensional (3D) cast was obtained by aligning the data of the functional impression to the preliminary intraoral scan data. The hollow bulb obturator and a palatal cover were designed based on the

Obturator Manufacturing for Oronasal Fistula after Cleft Palate Repair: A Review from Handicraft to the Application of Digital Techniques.

An oronasal fistula (ONF) is an abnormal structure between the oral and nasal cavities, which is a common complication of cleft palate repair due to the failure of wound healing. When some patients with ONF are unsuitable for secondary surgical repair, the obturator treatment becomes a potential method. The objectives of the obturator treatment should be summarized as filling the ONF comfortably and cosmetically restoring the dentition with partial function. The anatomy of patients with cleft palate is complex, which may lead to a more complex structure of the ONF. Thus, the manufacturing proc
