Medicinal cannabis in palliative medicine: lessons learnt from randomised controlled trials.

OBJECTIVES: To detail important lessons learnt while conducting several large, medicinal cannabis (MC) randomised clinical trials in a palliative cancer population. METHODS: Investigators involved in these trials had several meetings to agree on the major lessons learnt and how the various challenges could be mitigated in the future. RESULTS: The lessons were sorted into separate categories: patient confidentiality, family dynamics, driving, cost, unfounded beliefs, accessing specific MC products, trial funding, telehealth and COVID-19, and miscellaneous issues. CONCLUSION: Using MC as the

Palliative Medicine Point of Care UltraSound in Cancer Patients in a Specialist Palliative Medicine Outpatient Department: A Retrospective Analysis of Diagnostic and Procedural Impact.

OBJECTIVES: This retrospective study aimed to evaluate the utilisation and clinical impact of palliative medicine POCUS (PM-POCUS) in a specialist palliative care OPD, in a tertiary cancer centre in India. Patients with advanced cancer frequently present to palliative care outpatient departments (OPDs) with symptoms such as dyspnoea and abdominal distension, often due to pleural effusion, ascites or deep vein thrombosis. Timely diagnosis of these conditions is critical for symptom relief, yet access to imaging is frequently delayed, particularly in resource-limited settings. Point-of-care ultr

Prescribing for nausea in palliative care: a cross-sectional national survey of Australian palliative medicine doctors.

BACKGROUND: Nausea can be a debilitating symptom for patients with a life-limiting illness. While addressing reversible components, nonpharmacological strategies and antiemetics are the main therapeutic option. The choice of medication, dose, and route of administration remain highly variable. OBJECTIVE: The aim of this study was to codify the current clinical approaches and quantify any variation found nationally. METHODS: A cross-sectional study utilizing a survey of palliative medicine clinicians examined prescribing preferences for nausea using a clinical vignette. Respondent characteris

Palliative Care in Emergency Medicine: A Perspective from a Trainee.

Emergency medicine is practiced under conditions in which decisions and conversations often precede diagnostic or prognostic certainty. In this context, palliative care is frequently misconceived as a late intervention rather than a clinical stance applicable during uncertainty. This perspective posits that palliative care in the emergency department should be understood as an approach grounded in early recognition of unmet needs, communication under uncertainty, and deliberate clinical presence. Drawing on the experience of a physician training in palliative care, the article explores how "st

Management of Wound Myiasis in the Hospice and Palliative Medicine Setting.

Complex wounds are common complications in hospice and palliative medicine (HPM), especially in patients with aggressive malignancies. Myiasis, or an infestation of maggots, is a rare but significant complication of such wounds. While uncommon in the United States, many HPM patients have multiple risk factors and comorbidities that increase their vulnerability to this condition. Currently, there are no standard diagnostic or treatment guidelines for wound myiasis. In addition, common management strategies may not be easily accessible in HPM settings. We present this case of a patient with mali

[PALLIATIVE CARE OF PATIENT WITH GASTROINTESTINAL CANCER IN FAMILY MEDICINE].

Palliative care is defined as the care for patients whose disease is not responsive to curative treatment. The goals of palliative care are symptom control, life prolongation and enabling the best possible quality of life for patients and their families. For most patients with an advanced progressive incurable disease, 90% of care in their last year of life is provided at home by family physician and his team and patient family. Patients suffering from cancer have a mean of of 11.9 symptoms in the last three months of life. The most common symptoms are digestive tract symptoms and pain. The gr

The therapeutic application of melatonin in supportive care and palliative medicine.

Melatonin is a hormone produced mainly in the pineal gland. Plasma levels exhibit a circadian variation with the highest concentration occurring at night. The human biologic effects of melatonin depend upon the time of day it is made available. One of these effects is the setting and resetting of circadian clocks (chronobiotic effect). Additionally, it may be a potent antioxidant and immunomodulator and has been shown to have antitumor, anticytokine, anti-insomnia, and anticachexia effects. Melatonin has also been shown to improve survival and performance status in patients with advanced cance

Pharmacotherapy in palliative medicine-a decade of evidence: bibliometric review.

BACKGROUND: Pharmacotherapy has a strategic role in palliative care (PC). Medications are prescribed in this context for various reasons, leading to polypharmacy, associated with several negative outcomes. Despite its clinical relevance, the scientific literature about pharmacological interventions in PC remains scarce and fragmented. This review aimed to explore and map the pharmacological interventions in PC. METHODS: A bibliometric review was conducted using combined data from Scopus and Web of Science. Inclusion criteria covered original articles and reviews, published between 2014 and 20

Growing the Next Generation of Hospice and Palliative Medicine Researchers.

Hospice and palliative medicine (HPM) research is needed to improve care for the rapidly growing population of patients and caregivers affected by serious illness across the lifespan. Presently, multiple barriers hamper the advancement of HPM research, including limited scientific training opportunities, a relatively small pool of senior mentors, inconsistent access to institutional support, and scarce funding mechanisms to support increased numbers of early career investigators. Given this landscape, strategic investment in and guidance for the next generation of HPM researchers is critical t

The approach of internal medicine healthcare personnel to palliative care.

BACKGROUND: Healthcare workers in internal medicine wards often encounter challenges while providing palliative care due to a lack of training, emotional support, and organizational resources. Existing studies have explored attitudes toward palliative care in specialized teams, but less is known about internal medicine workers' perspectives. This study aims to address this gap by investigating the attitudes of internal medicine staff, analyzing demographic and professional factors, and assessing specific features, including knowledge of the legal aspects of palliative care. METHODS: A novel,

State of the science: cannabis and cannabinoids in palliative medicine-the potential.

Cannabinoids are chemicals derived naturally from the cannabis plant or are synthetically manufactured. They interact directly with cannabinoid receptors or share chemical similarity with endocannabinoids (or both). Within palliative medicine, cannabinoid receptors (CB1 and CB2) may modulate some cancer symptoms: appetite, chemotherapy-induced nausea and vomiting, and mood, pain and sleep disorders. Opioid and cannabinoid receptors have overlapping neuroanatomical receptor distribution, particularly at the dorsal horn, dorsal striatum and locus coeruleus. They have a favourable safety profile

Palliative Medicine Fellows' Discussions, Perceptions, and Training Regarding Medical Cannabis.

CONTEXT: Medical cannabis is increasingly considered for palliation of pain, nausea/vomiting, anorexia, and other symptoms. OBJECTIVES: We aimed to determine whether training in hospice and palliative medicine (HPM) adequately prepares fellows to counsel patients about medical cannabis. METHODS: A previously validated questionnaire was adapted for HPM fellows. Domains included fellows' practices recommending cannabis and their knowledge of its effectiveness and risks compared with standard treatments. U.S. HPM fellowships were sent surveys in 2022 and 2023. RESULTS: Forty six programs parti

Educational Perspectives: Palliative Care Education in Neonatal-Perinatal Medicine Fellowship.

The neonatal period from birth to less than or equal to 28 days is one of increased risk of death. Congenital anomalies and prematurity are 2 of the most common risk factors for death at this early age. Many of these neonates will die in an intensive care unit, some with full resuscitative efforts being undertaken despite the understanding that these actions are highly unlikely to yield an outcome different from death. Palliative care allows curative therapies to be provided alongside supportive techniques such as enhanced family communication, attention to spirituality and the psychosocial he

Does simulation have a role in palliative medicine specialty training?

Simulation training has been adopted by other industries, particularly aviation, for many years. With patient safety on every agenda, more recently there has been considerable interest and investment in its use for the acute medical specialities. Evidence in palliative medicine, for the use of simulation is mainly limited to advanced communication skills but little is described about its use in developing acute clinical skills. This article describes how in the Yorkshire and Humber Deanery a simulation training day was set up for Palliative Medicine specialty trainees, to assess their knowledg
