Cognitive and emotional outcomes were assessed after unilateral posteroventral pallidotomy (PVP) and ventral intermediate nucleus thalamotomy (Vim-Th) in patients suffering from idiopathic Parkinson's disease (PD). PVP was performed on 12 PD patients (8 men and 4 women, mean age 56.4 years, 6 left lesions and 6 right lesions) and Vim-Th was performed on 13 PD patients (5 men and 8 women, mean age 63.2 years, 6 left lesions and 7 right lesions). In both the PVP group and the Vim-Th group, the Hoehn and Yahr staging scores (p < 0.01) and the Unified Parkinson's Disease Rating Scale activities of

OBJECTIVE: To examine the efficacy of intracerebral recording of visual evoked potentials (VEPs) during posteroventral pallidotomy comparing macro- and microelectrode stimulation.
MATERIALS AND METHODS: The optic tract was identified by intracerebral recording of VEPs in 16 patients. Electrical stimulation through a lesioning electrode (macroelectrode stimulation: biphasic wave, 50 Hz, 0.2 ms, 0-6 V) and a microelectrode (microelectrode stimulation: impedance of 1 M(omega) at 300 Hz, biphasic pulse, 300 Hz, 0.2 ms, 10-50 microA) was delivered to assess visual responses.
RESULTS: VEPs were re

OBJECTIVE: To study the functional accuracy of stereotactic targeting for the posteroventral pallidotomy (PVP), comparing targets chosen on magnetic resonance images (MRI), and fused MRI to computed tomographic (CT) images, with electrophysiologically refined anatomical targets. METHDOS AND MATERIALS: For each of the 10 pallidotomies three sets of targets were collected, beginning with the MRI targets. The second target set was measured on images generated by nonlinear volumetric fusion of MRI images with CT using Image Fusion (Radionics, Inc.). The anatomical target site was then determined e

OBJECTIVE: Dystonias are relatively rare disorders characterized by sustained or intermittent muscle contractions causing abnormal movements or postures. Generalized dystonia is a therapeutic challenge because medications are unable to control dystonia adequately in most patients. These patients may be candidates for surgical therapy. The commonly used surgical procedures in these patients are pallidotomy and deep brain stimulation. Limited studies are available on the role of pallidotomy in children with acquired/heredodegenerative generalized dystonia. The objective of this study was to desc

OBJECTIVE: Ablation of the globus pallidus internus (pallidotomy) is an effective surgical intervention for dystonia. However, the current literature on the efficacy and safety of pallidotomy for dystonia is derived only from single-case reports and small cohort studies.
METHODS: We retrospectively analyzed patients with primary dystonia who underwent pallidotomy at our institution between 2014 and 2019. Neurological conditions were evaluated using the Burke-Fahn-Marsden Dystonia Rating Scale (BFMDRS, range: 0-120). We evaluated the total BFMDRS score and each subitem score (nine body regions

Postoperative lesion volume and clinical outcome were assessed in 19 Parkinson's disease (PD) patients who received posteroventral pallidotomy, and in 14 essential tremor (ET) patients who received ventrolateral thalamotomy. Before and after surgery, PD patients were evaluated using the Unified PD Rating Scale (UPDRS), and ET patients were evaluated using the Fahn-Tolosa-Marin (FTM) tremor rating scale. Inner and total lesion volumes were determined with postoperative MR imaging and three-dimensional data segmentation. Lesion volumes were compared to percent improvement in UPDRS and FTM scores

A 47-year-old female diagnosed as having juvenile parkinsonism at age 38 years developed progressive motor fluctuation while receiving levodopa medication. She underwent right posteroventral pallidotomy which achieved only a transient effect on the wearing-off. An additional procedure on the left pallidum resulted in long-lasting relief of the parkinsonian symptoms.

The major motor disturbances in Parkinson's disease are thought to be caused by overactivity of the internal segment of the globus pallidus (GPi), in large part due to excessive drive from the subthalamic nucleus. The excessive inhibitory activity of GPi is thought to "brake' the motor thalamus and the cortical motor system to produce the slowness, rigidity, and poverty of movement characteristic of parkinsonian states. To test the hypothesis that direct reduction of Gpi activity can improve motor function, we studied the effect of GPi pallidotomy in 14 patients. The location of the GPi nucleu

OBJECTIVE: The objective of this study was to assess the efficacy of unilateral pallidotomy in patients with asymmetrical cervical dystonia.
METHODS: This study retrospectively included 25 consecutive patients with asymmetrical cervical dystonia refractory to botulinum toxin injections, who underwent unilateral pallidotomy between January 2015 and April 2017. Toronto Western Spasmodic Torticollis Rating Scale (TWSTRS) scores were evaluated preoperatively and 1 week, 3 months, and 6 months postoperatively. The clinical responses were defined as good responders, exhibiting > 50% improvement in

BACKGROUND: Stereotactic pallidotomy in the treatment of primary dystonia results in permanent damage to the posteroventral region of the internal globus pallidus. Lesions within the basal ganglia may change cognitive functioning. Subcortical structures interact with the frontal cortex, which plays an important role in cognition.
OBJECTIVES: The aim of this study is to evaluate the effect of stereotactic pallidotomy on cognitive function in patients with primary dystonia.
METHODS: Thirty patients with primary dystonia who qualified for pallidotomy were tested 1-2 days before surgery, 2 days

The authors present a case of Hallervorden-Spatz disease (HSD) in a 10-year-old boy treated with stereotactic pallidotomy for control of severe dystonia. Hallervorden-Spatz disease is a rare type of neuraxonal dystrophy that can be familial or sporadic. This is the first case of HSD reported in the literature in which a pallidotomy was performed. The patient had progressively worsening dystonias and spasms that prevented useful function of his entire right side and eventually threatened his respiratory ability. Pre- and postoperative magnetic resonance images are presented along with electroph

Pallidotomy is a surgical procedure done widely for Parkinson's disease and various dystonias refractory to medical treatment. The technique involves radiofrequency (RF) thermal coagulation of globus pallidus internus, either unilaterally or bilaterally. The technique has been shown to produce good success. However, the involvement of nearby vital structures can result in new post-operative complications. We encountered a case of delayed emergence from anaesthesia associated with bilateral mydriasis and visual field defects in a patient after bilateral RF thermal lesioning.

BACKGROUND: We investigated the utility of pallidotomy for cervical dystonia after failed selective peripheral denervation.
CASE DESCRIPTION: A 36-year-old man presented with cervical dystonia with limited range of motion of the left shoulder, particularly in abduction. His Tsui score was 8. Owing to the ineffectiveness of botulinum toxin injection, he underwent selective peripheral denervation with an unsatisfactory outcome, with a postoperative Tsui score of 6. Simultaneous bilateral pallidotomy was performed with local anesthesia 1 year after the initial surgery. The day after the pallidot

BACKGROUND: Thalamotomies and pallidotomies were commonly performed before the deep brain stimulation (DBS) era. Although ablative procedures can lead to significant dystonia improvement, longer periods of analysis reveal disease progression and functional deterioration. Today, the same patients seek additional treatment possibilities.
METHODS: Four patients with generalized dystonia who previously had undergone bilateral pallidotomy came to our service seeking additional treatment because of dystonic symptom progression. Bilateral subthalamic nucleus DBS (B-STN-DBS) was the treatment of choi

Chorea that occurs as a result of a stroke is rare. We report a patient who developed hemiplegia following an acute hemorrhagic stroke in the left cerebral peduncle and subthalamus; although he had recovered gradually from the severe limb weakness, monochorea occurred 6 months later. Pallidotomy eliminated this symptom and we found it is a safe treatment for medically intractable post-stroke chorea after hemiplegia has resolved.

Thirty-six patients with Parkinson's disease (PD) were randomized to either medical therapy (N = 18) or unilateral GPi pallidotomy (N = 18). The primary outcome variable was the change in total Unified Parkinson's Disease Rating Scale (UPDRS) score at 6 months. Secondary outcome variables included subscores and individual parkinsonian symptoms as determined from the UPDRS. At the six month follow-up, patients receiving pallidotomy had a statistically significant reduction (32% decrease) in the total UPDRS score compared to those randomized to medical therapy (5% increase). Following surgery, p

BACKGROUND: The results of several cohort studies suggest that patients with advanced Parkinson's disease would benefit from unilateral pallidotomy. We have assessed the efficacy of unilateral pallidotomy in a randomised, single-blind, multicentre trial.
METHODS: We enrolled 37 patients with advanced Parkinson's disease who had, despite optimum pharmacological treatment, at least one of the following symptoms: severe response fluctuations, dyskinesias, painful dystonias, or bradykinesia. Patients were randomly assigned to unilateral pallidotomy within 1 month or to pallidotomy after the prima
