[Chronic pancreatitis and diabetes].

Chronic pancreatitis appears to be increasing likely because increase in alcohol consumption and improved diagnostic tools. So the diabetes due to chronic pancreatitis is gaining importance. In order to obtain a better understanding of that particular secondary diabetes, 179 patients with chronic pancreatitis were studied from 1970 to 1981. 154 patients were male and 25 female. Average admission age was 45, ranging from 25 to 86. All the patients underwent medical therapy, based on diet, pancreatic enzymes, pain control. 99 of them underwent even to surgery. The follow-up is ranging from 6 mon

Management of chronic pancreatitis.

Chronic pancreatitis results from repeated episodes of pancreatic inflammation and associated fibrosis leading to the loss of functional exocrine and endocrine pancreatic function. The disease is manifested by abdominal pain, deterioration in quality of life, food maldigestion and malabsorption, diabetes, and an increased risk for pancreatic adenocarcinoma. This review summarizes the latest evidence on the diagnosis and management of chronic pancreatitis and its manifestations. In particular, this review discusses advances in understanding of the role of genetic disorders in the mechanisms of

Nutrition in chronic pancreatitis.

The pancreas is a major player in nutrient digestion. In chronic pancreatitis both exocrine and endocrine insufficiency may develop leading to malnutrition over time. Maldigestion is often a late complication of chronic pancreatic and depends on the severity of the underlying disease. The severity of malnutrition is correlated with two major factors: (1) malabsorption and depletion of nutrients (e.g., alcoholism and pain) causes impaired nutritional status; and (2) increased metabolic activity due to the severity of the disease. Nutritional deficiencies negatively affect outcome if they are no

[Chronic pancreatitis: beyond alcohol].

For many years, the classification of chronic pancreatitis has been oversimplified according to whether the cause is alcoholic (60-70% of cases according to published series) or non-alcoholic (20-40% of the remaining cases). Recognition of smoking as an important risk factor and increasing identification of factors of genetic susceptibility have placed these percentages in doubt and have led to a reconceptualization of the disease as a multifactorial process. Mutations in the PRSS1, SPINK1 and CFTR genes have been confirmed as major risk factors, while mutations in the CTRC and CASR genes are

Surgery for chronic pancreatitis.

Gynaecomastia is a common clinical condition. Persistent pubertal or late onset idiopathic gynaecomastia is the leading cause of gynaecomastia in different series. The aim of this study was the assessment of the prevalence and characteristics of different causes of gynaecomastia in young adult males, and evaluation of the factors associated with idiopathic gynaecomastia. Fifty-three male patients (mean age 22.04 +/- 2.22, range 19-29), who had been admitted to our outpatient clinics with gynaecomastia as the main presenting symptom were enrolled in the study. Patients were evaluated with breas

Management of chronic pancreatitis.

Chronic pancreatitis (CP) is an inflammatory disorder that results in permanent impairment of the glandular anatomy of the pancreas with or without functional abnormalities. The pathogenesis of CP is usually unclear, except in the case of alcohol-induced disease. The most common symptoms of CP are abdominal pain, diarrhea, and weight loss often requiring recurring hospitalization. Over time, pancreatic endocrine and exocrine dysfunction may develop as the disease progresses, and a variety of complications can occur. Among the possible complications are nutrient malabsorption and diabetes melli

[Therapy of chronic pancreatitis].

Treatment of chronic pancreatitis is dependend on the stage of the disease and its complications. Pain therapy should be based on the knowledge of various causes of pain. In therapy of complications such as bile duct or pancreatic duct strictures interventional endoscopy is usually the first choice followed by surgery. Exocrine insufficiency is treated by porcine pancreatic extracts, endocrine insufficiency by insulin. One has to apply various imaging procedures such as sonography, MRCP, ERCP, endosonography, CT for exact diagnosis of complications and planning various therapeutic strategies.

Acute and chronic pancreatitis.

Pancreatitis is under appreciated during childhood although its diagnosis is simple and management straightforward in most cases. There is a range of possible causes, which is quite different to the situation in adults. The commonest underlying problems are probably structural abnormalities of the pancreatic and biliary ducts such as choledochal malformation, common pancreatobiliary channel and pancreas divisum. Other causes, which can be important in certain groups and geographical areas, are those due to drug reactions, viral infection and parasitic infestation, and blunt abdominal trauma. T

Imaging of chronic pancreatitis.

Chronic pancreatitis is characterized by progressive, irreversible morphologic changes whose most common cause is excessive alcohol intake. Radiologic imaging plays a major role in the diagnosis, staging of disease severity, detection of complications and selection of treatment options. The sensitivity of US ranges from 60% to 70% while its specificity is higher reaching 80%-90% in the detection of abnormalities of main pancreatic duct. As for CT, its sensitivity and specificity in the diagnosis of chronic pancreatitis, in recent studies is 74% and 85% respectively. The performance of Magnetic

Acute and chronic pancreatitis.

Acute pancreatitis has multiple causes, an unpredictable course, and myriad complications. The diagnosis relies on a combination of history, physical examination, serologic markers, and radiologic findings. The mainstay of therapy includes aggressive hydration, maintenance of NPO, and adequate analgesia with narcotics. Antibiotic and nutritional support with total parenteral nutrition should be used when appropriate.

Chronic pancreatitis in Jamaica.

Thirty-five patients with chronic pancreatitis (CP) treated over a 15-year-period were studied. There were 29 men and 6 women with a mean age of 47 years (range 21-67). Twenty-seven (77%) were chronic alcoholics, two (6%) had gallstones, one had stenosis of the Ampulla of Vater and in five (14%) no obvious cause was found. Thirty patients (86%) presented with abdominal pain. Chronic diarrhoea was present in 8 (23%), and steatorrhoea was documented in 6 of these. Fifteen (43%) had pancreatic calcifications. Five developed pseudocysts and 16 (46%) developed diabetes mellitus. Twelve patients req

Chronic pancreatitis and diabetes.

Chronic pancreatitis is defined by a persistent destruction of the pancreatic parenchyma replaced by fibrosis. The lesions generally start in the exocrine gland, islets being attacked later in the fibrosis. The two most frequent forms are: 1. Chronic calcifying pancreatitis which is a pancreatic lithiasis responsible for more than 95% of chronic pancreatitis. In its most frequent form, calculi are built up of more than 98% calcium salts together with fibres of a degraded residue of lithostathine, a secretory protein. This disease is related (i) in most countries to alcohol, protein, fat and to

[Surgery of chronic pancreatitis].

Between 1982 and 1988 47 patients underwent surgery for chronic pancreatitis at our institution. The main etiological factor was alcohol (41 cases). The mean age of the patients with alcohol-induced chronic pancreatitis was 15 years below that of the other patients. Calcifications were found in 27 patients at the time of surgery. Calcifications and exocrine insufficiency increased from 40% and 27% to 80% and 79% respectively if the history was shorter than 5 years or more than 10 years. The main indications for surgery were cholestatic jaundice (23 cases) and pseudocysts (22 cases), while only

Complications of chronic pancreatitis.

The most common complication of chronic pancreatitis is pain, which in many cases seems related to pancreatic ductal obstruction with ductal hypertension. Longitudinal pancreaticojejunostomy is indicated in patients with a dilated (larger than 7 mm) duct and pain that requires narcotic analgesics for relief. Chronic pseudocysts may be corrected surgically without the usual 6-week wait, and asymptomatic pseudocysts less than 4 cm in diameter may not require surgery at all. The relative efficacy and risks of percutaneous drainage of pseudocysts versus the standard surgical approaches need to be

Sonography of chronic pancreatitis.

Morphologic alterations detectable by US in CP may be schematically classified as: (1) size increase or decrease; (2) variations in shape with particular regard to contour irregularities; (3) changes in the parenchymal echo-texture; (4) presence of calcifications; (5) dilatation of the MPD; (6) presence of fluid collections; (7) dilatation of the biliary tree; and (8) obstruction of the portal venous system. These findings are associated in various combinations and occur with differing frequencies. Changes in pancreatic size may be found in many physiologic and pathologic conditions other than
