Accuracy of working diagnosis by paramedics for patients presenting with dyspnoea.

OBJECTIVE: The present study aims to determine the agreement between paramedic and ED or hospital working diagnosis in dyspnoeic patients. METHODS: Non-consecutive written patient report forms were retrospectively audited for patients suffering from dyspnoea, who were transported to a tertiary hospital ED by ambulance paramedics. Accuracy of the paramedic working diagnosis was assessed by comparing agreement with either the primary or secondary ED diagnoses or hospital discharge diagnosis. RESULTS: The study cohort was 293 patients. Exact agreement between paramedic versus ED or hospital dia

Paramedic perspectives on workplace peer support: a thematic analysis.

PURPOSE: Trauma exposure and workplace psychosocial hazards can negatively impact paramedics' mental health. Peer support programs are increasingly deployed, but evidence regarding their use in this context is limited. This qualitative study explored paramedics' experiences with peer support. METHODS: Semi-structured interviews were conducted with 13 paramedics, from a single Australian ambulance service, who had recently (<12 months) engaged with peer support. Interviews explored support needs, helpful and unhelpful program elements, and program outcomes. Transcripts were analysed using refl

Avoid the Goose! Paramedic Identification of Esophageal Intubation by Ultrasound.

OBJECTIVES: Rapid identification of esophageal intubations is critical to avoid patient morbidity and mortality. Continuous waveform capnography remains the gold standard for endotracheal tube (ETT) confirmation, but it has limitations. Point-of-care ultrasound (POCUS) may be a useful alternative for confirming ETT placement. The objective of this study was to determine the accuracy of paramedic-performed POCUS identification of esophageal intubations with and without ETT manipulation. METHODS: A prospective, observational study using a cadaver model was conducted. Local paramedics were recru

Paramedic Differentiation of Asthma and COPD in the Prehospital Setting Is Difficult.

INTRODUCTION: Separate clinical practice guidelines (CPG) for asthma and chronic obstructive pulmonary disease (COPD) often guide prehospital care. However, having distinct CPGs implies that paramedics can accurately differentiate these conditions. We compared the accuracy of paramedic identification of these two conditions against the emergency department (ED) discharge diagnosis. METHODS: A retrospective cohort of all patients transported to ED by ambulance in Perth, Western Australia between July 2012 and June 2013; and identified as "asthma" or "COPD" by paramedics. We linked ambulance da

Flight paramedic scope of practice: current level and breadth.

This study's objective was to determine the current level and breadth of flight paramedic scope of practice. A six-item survey of lead flight paramedics in 158 air medical programs addressed five issues: 1) Certifications required above state certification; 2) Procedures included in scope of practice; 3) Medications flight paramedics are allowed to administer; 4) Requirements needed to expand scope of practice; and 5) Views on establishing a National Flight Paramedic Certification to alter their scope of practice. Eighty programs out of the 90 respondents (89%) stated that they utilize flight

The Prevalence of Neck and Back Pain among Paramedics in Poland.

Neck Pain (NP) and low back pain (LBP) are ubiquitous musculoskeletal conditions and some of the major causes of disability worldwide. The aim of the study was to assess the incidence and risk factors of back pain among paramedics and their correlation with the nature of work, anthropometric features and the level of recreational physical activity. A group of 201 individuals (39 females (19.40%); 162 males (80.60%)), licensed to practice as paramedics in Poland completed a questionnaire containing the author's interview as well as the Neck Disability Index (NDI) questionnaire and the Revised O

Retention of cricothyrotomy skills by paramedics using a wire guided technique.

INTRODUCTION: Cricothyrotomy may be necessary for airway management when a patient's airway cannot be maintained through standard techniques such as oral airway placement, blind insertion airway device, or endotracheal intubation. Wire-guided cricothyrotomy is one of many techniques used to perform a cricothyrotomy. Although there is some controversy over which cricothyrotomy technique is superior, there is no published data regarding long term retention rates. The purpose of this study is to determine whether ground based paramedics can be taught and are able to retain the skills necessary to

Delayed Sequence Intubation by Intensive Care Flight Paramedics in Victoria, Australia.

OBJECTIVE: Delayed sequence intubation (DSI) involves the administration of ketamine to facilitate adequate preoxygenation in the agitated patient. DSI was introduced into the Clinical Practice Guideline for Intensive Care Flight Paramedics in Victoria in late 2013. We aimed to describe the clinical characteristics of patients receiving DSI. METHODS: A retrospective analysis was undertaken of patients who received DSI between January 1, 2014, and December 31, 2016, during both primary response and retrieval missions. Patients' clinical characteristics, DSI success rates, and complications wer

Prevalence and risk factors of musculoskeletal disorders among paramedics in Quebec, Canada.

Paramedics face a challenging work environment with a high risk of musculoskeletal disorders (MSD). This study examined the prevalence of musculoskeletal pain and its associations with self-reported individual, physical, and psychosocial risk factors among 334 paramedics in Quebec. Data were collected through self-administered questionnaires and analysed using multiple logistic regressions. Results showed that 69% of participants reported musculoskeletal pain in the past 12 months, most commonly in the back, shoulders, and neck. Tasks involving hand movements above shoulder level and working w

False Electrical Capture in Prehospital Transcutaneous Pacing by Paramedics: A Case Series.

INTRODUCTION: The use of transcutaneous pacing (TCP) for unstable bradycardia has a class 2B recommendation from the American Heart Association. Prior studies have not adequately described the frequency or possible causes of treatment failure. EMS clinicians and leaders have reported false electrical capture as a potential cause. In this study, we aimed to describe the frequency of true electrical capture, documented verification of mechanical capture, and its association with systolic blood pressure (SBP) and survival. METHODS: This was a retrospective study of patients treated by an urban,

Prevalence and Characteristics of Violence against Paramedics in a Single Canadian Site.

Violence against paramedics has been described as a 'serious public health problem' but one that remains 'vastly underreported', owing to an organizational culture that stigmatizes reporting-hindering efforts at risk mitigation in addition to creating a gap in research. Leveraging a novel reporting process developed after extensive stakeholder consultation and embedded within the electronic patient care record, our objective was to provide a descriptive profile of violence against paramedics in a single paramedic service in Ontario, Canada. Between 1 February 2021 and 31 January 2023, a total

Qualitative Research of Violent Incidents Toward Young Paramedics in the Czech Republic.

INTRODUCTION: Prehospital and emergency medical services (EMS) providers are usually the first to respond to an individual's urgent health needs, sometimes in emotionally charged circumstances. Because violence toward EMS providers in the Czech Republic is often overlooked and under-reported, we do not have a complete understanding of the extent of such violence, nor do we have recommendations from EMS professional organizations on how to resolve this problem in prehospital emergency medicine. METHODS: We conducted this study to explore the process of violence against EMS providers, using the

[General and specific individual post-traumatic stress disorder-related mechanisms among paramedics].

BACKGROUND: Due to the nature of their work, paramedics face an increased risk of developing a post-traumatic stress disorder. The main goal of this research project was to point out specific correlates of post-traumatic stress. The authors decided to approach that issue from the perspective of emotional-cognitive deficits and resources, such as alexithymia, rumination and coping with stress. MATERIAL AND METHODS: The authors conducted 3 independent research projects in the first half of the year 2017 in the Wielkopolskie voivodship, that included paramedics. In each one of them, they estimat

Use of morphine sulphate by South African paramedics for prehospital pain management.

BACKGROUND: Evidence in the literature highlights the fact that acute pain in the prehospital setting remains poorly managed. Morphine remains the most commonly used analgesic agent in the South African prehospital emergency care setting. Although guidelines and protocols relating to the dosage and administration of morphine exist, little data are available describing its use by South African paramedics. OBJECTIVES: To document and describe the way in which morphine is administered by a sample of South African paramedics for the management of acute pain in the prehospital setting. METHODS: A

[A paramedical team "on the front line" of hospital care for legionnaires].

The psychiatric department at the Laveran Army Training Hospital in Marseille is a referral center for legionnaires, a population at high risk of psychological trauma. As part of a multi-disciplinary team, nurses and orderlies play a major role in their care. The dual status of caregiver and soldier is an asset in providing comprehensive care for psychologically injured legionnaires. The service's civilian staff also have extensive knowledge of the military environment.

OBJECTIVE: Making an accurate clinical diagnosis in the field of prehospital is of great challenge in medical services. This study aimed to determine agreement between prehospital and in-hospital diagnoses. METHODS: The diagnostic agreement was determined by a comparison of the discharge diagnosis with the prehospital emergency technicians in a period of 6 months at the emergency medical services in northwest Azerbaijan. The diagnostic agreement of discharge diagnoses was compared with the fist diagnosis by the paramedics. The results were analyzed using the kappa agreement coefficient and th
