Paraspinal muscle volume in patients with Scheuermann's Kyphosis.

To measure the cross-sectional area (CSA) of para-spinal muscles in Scheuermann's kyphosis patients. Preoperative MRI images of 16 Scheuermann's kyphosis patients were analysed and compared to 16 patients with normal MRI images (control group). The CSAs were measured at L3-4 and L4-5. Both groups showed similar demographics and patient characteristics. The multifidus muscles CSA were found to be significantly smaller at L3/4 level in Scheuermann's kyphosis patients (p = 0.022 on the left and p = 0.016 on the right side compared to control group). There was no significant change in multifidus C

Does lumbar paraspinal muscles improve after corrective fusion surgery in degenerative flat black?

BACKGROUND: Degenerative flat back (DFB) is characterized by sagittal imbalance resulting from the loss of lumbar lordosis (LL). Extensive degeneration and weakness of lumbar paraspinal extensor muscle (PSE) are thought to be the main cause of DFB. This study is to evaluate correlation between preoperative PSE conditions and angular severity of DFB and to evaluate correlation between preoperative PSE conditions and degree of improvement of DFB obtained by corrective surgery. MATERIALS AND METHODS: Forty five patients with DFB who took magnetic resonance image (MRI) preoperatively and conducte

Epimuscular fat and its clinical relevance: a narrative review focused on paraspinal muscles.

BACKGROUND AND OBJECTIVE: Epimuscular fat (EF) is a distinct adipose tissue located outside the epimysial border of muscles, with growing interest in its biomechanical and metabolic functions. Although initial studies have investigated EF in the rotator cuff, its role in the lumbar spine and its potential relationship with low back pain (LBP) remain poorly understood. This narrative review examines the structural, functional, and molecular characteristics of EF, focusing on its presence in the paraspinal muscles and its potential impact on spinal stability and pathology. METHODS: A narrative

Electrical Impedance Myography for Assessing Paraspinal Muscles of Patients with Low Back Pain.

The objective of this study was to determine the potential value of electrical impedance myography (EIM) for assessing lumbosacral paraspinal muscle (LPM) condition in lower back pain (LBP) patients. Standard methods for assessing the condition of LPMs, such as magnetic resonance imaging, are inconvenient and expensive. One tool that could be useful for this purpose is electrical impedance myography (EIM) a technique that can be performed rapidly at the bedside. After undergoing a screening history and examination, subjects were studied with the mView EIM device (Myolex, Inc, Boston). Bilatera

Oxidative stress and heat shock protein response in human paraspinal muscles during retraction.

OBJECT: The need for wide dissection and forceful retraction of paraspinal muscles often required for posterolateral lumbar fusion and fixation may severely jeopardize the muscles, structurally and functionally. The underlying pathophysiology of muscle damage may involve both mechanical and ischemic mechanisms. On the other hand, the surgery-related stress may trigger certain protective responses within the insulted paraspinal muscles. This study was conducted to assess the relationship between the oxidative stress and the stress response mediated by heat shock protein 70 (HSP70) induction wit

Relation of bone mineral density with fat infiltration of paraspinal muscles: The Goutallier classification.

OBJECTIVES: Muscle and bone tissue are interrelated throughout their developmental processes via paracrine and endocrine pathways. Osteosarcopenia has emerged with the growing data proving the high rate of simultaneous occurrence of sarcopenia and osteoporosis. We aimed to evaluate the relationship between osteoporosis, and muscle quality by grading the fatty infiltration in paraspinal muscles according to the Goutallier classification in magnetic resonance imaging (MRI). METHODS: Data of postmenopausal patients who underwent MRI for low back pain were analyzed retrospectively. Lumbar spine a

Associations of Lumber Disc Degeneration With Paraspinal Muscles Myosteatosis in Discogenic Low Back Pain.

Accompanied with intervertebral disc (IVD) degeneration, increasing fat infiltration of paraspinal muscles may be related to discogenic low back pain (DLBP), but their relationship is still unclear and the classical animal models are not completely applicable. The purpose of this study was to assess the paraspinal muscle fat infiltration in patients with DLBP by quantitative MRI, and to develop a novel DLBP rat model to explore the potential relationship between DLBP paraspinal muscle fat infiltration and TNF-α levels. We measured the proton density fat fraction (PDFF) of the multifidus and er

Cellular and molecular mechanisms of fatty infiltration in paraspinal muscles in spine-specific sarcopenia.

Spine-specific sarcopenia has recently been recognized as a localized degenerative disorder distinct from systemic sarcopenia. Its main symptom is selective degeneration of paraspinal muscles and is closely associated with various spinal disorders and postoperative prognosis. One of the core features of this condition is fatty infiltration (FI), which more accurately reflects muscle quality and functional impairment than muscle cross-sectional area (CSA). FI is closely associated with paraspinal muscle atrophy and is regulated by multiple interrelated mechanisms, including pathological activat

Electromyographic responses of paraspinal muscles to postural disturbance with special reference to scoliotic children.

BACKGROUND: Electromyographic (EMG) responses of erector spinae to a postural perturbation have been described and interpreted as an unloading reflex. Moreover, these responses have been found clearly and constantly abnormal in subjects presenting a progressive idiopathic scoliosis when compared with responses observed in subjects presenting a nonprogressive scoliosis or in normal subjects. OBJECTIVE: To investigate responses to obtain more precise information on their components, on their origin, and on their variations in scoliotic children. SUBJECTS: Thirteen scoliotic children, with 3 ca

Molecular evidence for local denervation of paraspinal muscles in failed-back surgery/postdiscotomy syndrome.

A study was performed to analyze whether local denervation of the medial branch of the dorsal ramus of the lumbar spinal nerve occurs in a patient with postoperative failed-back surgery syndrome/postdiscotomy syndrome (FBSS/PDS). We investigated the effect of the loss of innervation of the multifidus muscle on neuronal nitrite oxide synthetase (n-NOS) and endothelial nitrite oxide synthetase (e-NOS) applying realtime RT-PCR and immunohistochemistry. Our study demonstrates a substantial reduction of n-NOS expression, supporting the view that local denervation of the multifidus is involved in th

Paraspinous Muscle Flaps for the Treatment of Complex Spinal Wounds.

UNLABELLED: MINI: This retrospective case series investigated paraspinous flaps for coverage of complex spinal wounds. 6.90% of patients developed postoperative wound infections and 0.00% of patients required instrumentation removal for infection. This suggests that these flaps may offer a long-term solution in wound management for patients with repeated spinal operations. STUDY DESIGN: Retrospective case series. OBJECTIVE: To investigate the efficacy and complication profile of the use of paraspinous muscle flaps for closure of complex spinal wounds. SUMMARY OF BACKGROUND DATA: Paraspinous

Paraspinal Muscle Segmentation Based on Deep Neural Network.

The accurate segmentation of the paraspinal muscle in Magnetic Resonance (MR) images is a critical step in the automated analysis of lumbar diseases such as chronic low back pain, disc herniation and lumbar spinal stenosis. However, the automatic segmentation of multifidus and erector spinae has not yet been achieved due to three unusual challenges: (1) the muscle boundary is unclear; (2) the gray histogram distribution of the target overlaps with the background; (3) the intra- and inter-patient shape is variable. We propose to tackle the problem of the automatic segmentation of paravertebral

Paraspinal muscle vasculature contributes to posterolateral spinal fusion.

STUDY DESIGN: Study of posterolateral fusions in a rabbit model. OBJECTIVES: To characterize the contribution of paraspinal musculature to the healing of posterolateral spinal fusions in a rabbit model. SUMMARY OF BACKGROUND DATA: Previous studies have demonstrated that successful spinal arthrodesis requires vascular ingrowth from adjacent decorticated bone. In other areas of the body, such as the tibia, vascular ingrowth from the surrounding musculature has also been shown to be important. The role of the surrounding paraspinal musculature in spinal fusions has yet to be assessed. METHODS:
