Sinus node dysfunction masquerading as complete atrioventricular block in the setting of atrial parasystole after heart transplantation.

Heart transplantation is the gold standard for treatment of end stage heart failure. Atrial parasystole (AP), defined as the presence of an additional dissociated atrial pacemaker on a surface electrocardiogram, has been described to be seen after heart transplantation using biatrial surgical technique. AP may pose a challenge in diagnosing underlying rhythm disturbances in these patients. Here, we report a rare phenomenon of junctional escape rhythm with sinus node dysfunction, mimicking as complete atrioventricular block due to the presence of an AP, in a heart transplant recipient.

Reappraisal of the coupling interval of ventricular extrasystoles as an index of ectopic mechanisms.

OBJECTIVE: A mathematical model of modulated ventricular parasystole based on the relation between the coupling interval and the preceding RR interval was developed in an attempt to distinguish between parasystolic automaticity and other mechanisms. MATHEMATICAL MODEL: The relation between the coupling interval and the preceding RR interval was examined by plotting the coupling interval of each extrasystole against the preceding RR interval (coupling interval/RR diagram). The coupling interval/RR diagrams obtained from simulations with various modulation modes suggested that the parasystolic m

Doubling of the ventricular rate by interpolated junctional extrasystoles resembling supraventricular tachycardia.

In a study of seven cases of paroxysmal supraventricular tachycardia, it was noted that the fast rate was not caused by the mechanism of rapid firing, reentry, or dual atrioventricular nodal conduction but by an abrupt doubling of the rate by interpolation of junctional extrasystoles between adjacent sinus beats while the sinus mechanism remained undisturbed. Dual ventricular response to a single atrial depolarization was seriously considered in each case. The intervals separating the junctional extrasystoles tended to be quite fixed, thus conforming to the pattern of junctional parasystole wi

Pseudo-2:1 bundle branch block. "Fusion causes confusion".

The fusion of narrow-QRS sinus-generated beats with end-diastolic ventricular extrasystoles occurring in bigeminy can produce an electrocardiographic pattern difficult to differentiate from parasystole. Such an ECG should not be interpreted as 2:1 RBBB because of the variability of the PR intervals.

Ventricular arrhythmias in children with attention deficit disorder--a symptom of autonomic imbalance?

OBJECTIVES: Potential side effects of stimulants for attention deficit disorder are in the focus of scientific discussions, intensified by the higher number of prescriptions. Children with known arrhythmias or other severe cardiac problems should not receive stimulants because of their sympathomimetic effects. METHODS: This is a retrospective analysis of 24-hour Holter electrocardiograms from 100 consecutive children with attention deficit disorder from January, 2006 to April, 2012. RESULTS: In all, nine children had significant ventricular arrhythmia (mean age 11.4 ± 3.1 years, 77% male, 77

Model of rate-dependent property in myocardial tissues as a useful algorithm for the PC-based arrhythmia generator.

Two mathematical models were presented as an arrhythmia generator. One was the so-called modulated parasystole model. The other was a model of the rate-dependent property of atrioventricular nodal conduction represented by the atrioventricular recovery curve (AVRC). Both models were mathematically formulated as a nonlinear first-order difference equation, and implemented on a personal computer (PC) as an arrhythmia simulator. It was utilized for evaluating the diagnostic performance of computerized ECG analyzers.

Transient P' waves in a hyperkalemic cat: artifact or dissociation?

A 15-year-old Sphynx cat was referred for cervical ventroflexion, ataxia, and lethargy associated with hypokalemia. After administration of supplemental potassium, the cat became severely hyperkalemic. Transient P' (vs. pseudo P' waves) were detected on the electrocardiogram. Over the course of hospitalization, the cat's potassium normalized, and the abnormal P' waves did not recur. These images are presented to highlight the differential diagnoses for this type of electrocardiogram. Diagnostic considerations included complete or transient atrial dissociation (as a rare consequence of hyperkal

[Ventricular arrhythmias and mitral valve prolapse. A study of 35 cases].

35 patients with mitral valve prolapse (MVP) underwent investigation by Holter monitoring with computerised analysis. Ventricular arrhythmias were observed in 70% patients with MVP. There does not seem to be a close correlation between the severity of the arrhythmia and the degree of prolapse. The mechanisms of the ventricular arrhythmia are variable: two patients had the necessary criteria for ventricular reentry, at least six had parasystolic foci, protected or not from the sinus rhythm. The most severe ventricular arrhythmias are characterised by their variable morphology (but usually showi

Ventricular arrhythmias induced by snake bite in a horse: A case report.

A Mangalarga Marchador mare was referred to treatment with a history of poisoning with evident tachyarrhythmia detected on cardiac auscultation. The electrocardiogram was performed and detected occurrences of paroxysmal monomorphic ventricular bigeminy (pMVB), pMVB with pairs of premature ventricular contractions (PVC) (pMVBPC), and paroxysmal monomorphic ventricular tachycardia (pMVT). In all cases, incomplete atrioventricular dissociation and coupling variation are observed. These characteristics, associated with non-reduction of successive subsequent coupling intervals and the absence of ty

[Abnormal communications in acute bacterial endocarditis of the aortic valve].

Abnormal communications acquired during acute aortic valve bacterial endocarditis are rare but serious complications. Seven cases are reported; between the left ventricle and right atrium (3 cases), the left and right ventricles (2 cases), the aorta and right atrium (I case) and the aorta and left atrium (I case). The usual causal organisms is a staphylococcus (4 out of 7). The diagnosis is suspected on the development of atrio-ventricular block, a parasystolic murmur and sudden severe cardiac failure, but can only be confirmed by catheterisation and angiocardiography (impractical in our patie

The Characteristics and Clinical Analysis of Lorenz Plot of Neonatal Atrial Tachycardia.

BACKGROUND: Lorenz plot (LP) analysis enhances the accuracy of arrhythmia diagnosis and serves as an attractive diagnostic tool for patients with arrhythmias. We explored the utility of the LP in the classification of various types of neonatal atrial tachycardia and evaluated the treatment effects and prognosis. METHODS: Ninety-six newborns with atrial tachycardia had their dynamic electrocardiograms and clinical data collected between January 2018 and June 2023. They were divided into different atrial tachycardia groups based on the morphological characteristics of the LP. The clinical featu

High power radiofrequency ablation of incessant atrioventricular re-entrant tachycardia in a patient with a Para-Hisian accessory pathway.

Radiofrequency ablation is the interventional therapy of choice for the definitive cure of patients with tachycardias facilitated by accessory pathways. Ablation of an accessory pathway at sites close to the His bundle is inevitably associated with an increased risk of causing complete atrioventricular block. Here we describe the case of a patient with pre-excitation syndrome and episodes of supraventricular tachycardia, in whom an electrophysiological study identified an accessory pathway so close to the His bundle that discrete sites between the pathway and the His bundle were extremely diff

Evolution of rhythms during periodic stimulation of embryonic chick heart cell aggregates.

During periodic stimulation of spontaneously beating chick heart cell aggregates, there is often an evolution of coupling patterns between the stimulator and the aggregate action potential. For example, at rapid stimulation frequencies, a rhythm that is initially 1:1 (stimulus frequency:aggregate frequency) can evolve to other rhythms such as 5:4 and 4:3. Time-dependent effects generated during periodic stimulation are characterized by three types of experiments to determine 1) the effect of periodic stimulation on the intrinsic cardiac beat rate (overdrive suppression), 2) the effect of perio

[Electrophysiological mechanisms of atrial tachyarrhythmia].

Both supraventricular and ventricular arrhythmias are generated by one of the following 2 mechanisms: increased automaticity and reentry. In this paper we will refer about the mechanisms of the commonest atrial tachyarrhythmias. Atrial parasystole: it is determined by an automatic focus protected by entrance block. In some cases the focus is only partially protected owing to a partial block or to the electrotonic influence of the sinus rhythm. Atrial tachycardia: it can be due both to increased automaticity and to reentry. Atrial flutter: it is generated by a counterclockwise macroreentry loca

Mechanical Ablation of Concealed Left Lateral Bypass Tract.

A 50-yaer-old man with hypertension had been treated for supraventricular tachycardia with several medications for nine years. In 1990, he was started on amiodarone but a year later he developed side effects causing discontinuation of amiodarone. Because of his recurrent episodes of palpitations associated with near syncope, chest pain and shortness of breath, he underwent an electrophysiology study in 1992 that showed orthodromic AVRT with the presence of a concealed left-sided accessory bypass tract. Scheduled for radiofrequency ablation the following day, after catheters were placed and dur

Comparison of the effect of inhaled selective and non-selective adrenergic agonists on cardiorespiratory parameters in chronic stable asthma.

Increased hypoxia has been found after beta 2 adrenoceptor agonists (but not adrenaline) in asthmatics. Combined with hypokalaemia and sympathomimetic stimulation, this may predispose to cardiac arrhythmias. We have compared the effects of nebulized adrenaline and a selective beta 2 agonist (salbutamol) on the arterial oxygen saturation (SaO2), minute ventilation (VE), forced expiratory volume in 1 s (FEV1), plasma potassium and the electrocardiogram (ECG) in patients with chronic stable asthma. Six patients were studied according to a randomized, placebo-controlled, double-blind cross-over pr
