Influence of Weight Bearing on Postoperative Complications after Surgical Treatment of the Lower Extremity.

PURPOSE: In order to prevent implant failure and secondary fracture dislocation, it is often recommended that patients perform partial weight-bearing after surgery of the lower extremity. Previous examinations showed that patients are often not able to follow these instructions. In this study, patients who had undergone surgery of the lower extremity were studied in order to analyze whether incorrect loading influenced the number and severity of complications. METHODS: Fifty-one patients were equipped with electronic shoe insoles, which measure loading and other parameters. The measurement pe

An insole sensor for recording weight bearing behavior during tibial fracture rehabilitation.

Partial weight bearing prescriptions for tibial fracture care are made with little data to support their efficacy. To provide long term tibial load data that can be used to study, guide, and monitor partial weight bearing, we developed a novel load sensor that can record the load placed on an injured limb over a two-week period of time. The prototype load sensor demonstrated high linearity, low hysteresis and low static and dynamic drift. Preliminary human testing on the load sensor demonstrated the ability of the load sensor to capture load profile during underfoot recording. Presented are th

Comparing Outcomes for Fasciocutaneous versus Muscle Flaps in Foot and Ankle Free Flap Reconstruction.

BACKGROUND: The distal lower extremity poses unique reconstructive challenges due to its requirements for durability of the load-bearing plantar surface and for thin, pliable contour in the dorsal foot and ankle region. This study compares outcomes between muscle and fasciocutaneous flaps in patients with foot and ankle defects. METHODS: A retrospective review of soft tissue free flaps used for traumatic foot and ankle defects was performed. Outcomes included takebacks, partial flap failure, total flap failure, and wound complications. RESULTS: A total of 165 cases met inclusion criteria, wi

Early Full Weight-Bearing After Calcaneal Fracture Using a Heel-Unloader Orthosis: Equivalent Radiographic Outcomes Compared to Traditional Cast Treatment.

BACKGROUND: Standard treatment of calcaneal fractures involves prolonged immobilization with partial weight-bearing, risking ankle stiffness and lower-leg deconditioning. The heel-unloader enables early full weight-bearing by redirecting axial load from the hindfoot to the midfoot, forefoot, and lower leg. This study compares radiologic and clinical outcomes of heel-unloader vs conventional cast immobilisation. METHODS: We retrospectively analyzed 101 calcaneal fractures treated with cast immobilization and partial weight-bearing or a heel-unloader with early full weight-bearing. Böhler angle

Weight-Bearing Approaches After Neck of Femur Fractures: A Narrative Review of Evidence and Outcomes.

Neck of femur fractures, a prevalent injury among the elderly, significantly impair mobility, independence, and quality of life. The timing and extent of weight bearing post-surgery are critical to recovery, yet clinical practices vary widely. This narrative review synthesizes evidence on weight-bearing strategies following neck of femur fractures, focusing on their impact on functional outcomes, complications, and hospital stay duration. A comprehensive literature search was conducted using PubMed, Science Direct, and Google Scholar for studies published between 2012 and 2024. The keywords, a

Permissive weight bearing in trauma patients with fracture of the lower extremities: prospective multicenter comparative cohort study.

BACKGROUND: The standard aftercare treatment in surgically treated trauma patients with fractures around or in a joint, known as (peri)- or intra-articular fractures of the lower extremities, is either non-weight bearing or partial weight bearing. We have developed an early permissive weight bearing post-surgery rehabilitation protocol in surgically treated patients with fractures of the lower extremities. In this proposal we want to compare our early permissive weight bearing protocol to the existing current non-weight bearing guidelines in a prospective comparative cohort study. METHODS/DES

In vivo hip joint loading during post-operative physiotherapeutic exercises.

INTRODUCTION: After hip surgery, it is the orthopedist's decision to allow full weight bearing to prevent complications or to prescribe partial weight bearing for bone ingrowth or fracture consolidation. While most loading conditions in the hip joint during activities of daily living are known, it remains unclear how demanding physiotherapeutic exercises are. Recommendations for clinical rehabilitation have been established, but these guidelines vary and have not been scientifically confirmed. The aim of this study was to provide a basis for practical recommendations by determining the hip joi

Distal finger reconstruction technique combining a distally-based finger flap and a partial toe flap.

BACKGROUND: Although aesthetic reconstruction of an amputated distal finger can be achieved through partial toe transfer, this approach often damages the weight-bearing region of the toe from which the flap is harvested. The purpose of this report is to introduce the minimum invasive surgery technique to reconstruct the distal finger aesthetically without damaging the weight-bearing region of the toe. PATIENTS AND METHODS: Thirty-one amputated fingertips in 30 patients aged 18 to 68 years were treated using this operative technique. Operations were performed between January 2010 and December

Weight Bearing as Tolerated After Intramedullary Nailing of the Femur: A Retrospective Analysis of Clinical and Radiographic Outcomes.

Intramedullary nailing of femur fractures has become the standard of care, with high union rates. Few high-level studies have discussed the effect that early weight bearing has on the healing of these fractures, regardless of nail size or fracture pattern. The goal of this study was to determine the clinical and radiographic outcomes of femoral shaft fractures for patients allowed immediate weight bearing after intramedullary nailing. We performed a retrospective review of 341 femoral shaft fractures, with 131 allowed immediate weight bearing, 99 allowed partial weight bearing, and 111 kept no

Neonatal Spinal Cord Transection Decreases Hindlimb Weight-Bearing and Affects Formation of Achilles and Tail Tendons.

Mechanical loading may be required for proper tendon formation. However, it is not well understood how tendon formation is impacted by the development of weight-bearing locomotor activity in the neonate. This study assessed tendon mechanical properties, and concomitant changes in weight-bearing locomotion, in neonatal rats subjected to a low thoracic spinal cord transection or a sham surgery at postnatal day (P)1. On P10, spontaneous locomotion was evaluated in spinal cord transected and sham controls to determine impacts on weight-bearing hindlimb movement. The mechanical properties of P10 Ac

Weight-bearing in Trauma Surgery (WiTS) Study: A national survey of UK Trauma & Orthopaedic multidisciplinary health professionals.

INTRODUCTION: Weight-bearing (WB) status following a fracture or surgical fixation is an important determinant of the mechanical environment for healing. In order for healthcare professionals to communicate and understand the extent of bearing weight through a limb, clear terminology must be used. There is widespread variation in the usage and definitions of WB terminology in the literature and clinical practice. This study sought to define the understanding and extent of variation across the United Kingdom. METHODS: A nationwide online survey of UK-based Trauma & Orthopaedic (T&O) multidisci

Long-term results of displaced talar neck fractures.

Of 36 fractures of the talar neck without comminution of the body, eight were undisplaced, treated closed, and 28 were displaced and treated by open reduction. Twenty of the operations were less than 12 hours after injury. Nineteen of 20 were performed through a medial approach, six with a medial malleolar osteotomy. The long-term results were evaluated by a standard rating system based on classification by the fracture. A protective brace was developed for non-weight-bearing in two patients with complete avascular necrosis, and ankle protection with weight-bearing in ten with partial necrosis
