Recurrent patellar dislocations with patellar cartilage defects: A pain in the knee?

BACKGROUND: Recurrent patellar dislocation in combination with cartilage injures are difficult injuries to treat with confounding pathways of treatment. The aim of this study was to compare the clinical and functional outcomes of patients operated for patellofemoral instability with and without cartilage defects. METHODS: Eighty-two patients (mean age 28.8 years) with recurrent patellar dislocations, who underwent soft-tissue or bony procedures, were divided into two matched groups (age, sex, follow up and type of procedure) of 41 each, based on the presence or absence of cartilage defects in

MRI of traumatic patellar dislocation in children.

BACKGROUND: Traumatic patellar dislocations (TPD) are common injuries in children, and MRI is useful in evaluation of pediatric musculoskeletal injuries. However, no pediatric studies on the MR features of TPD have been reported. OBJECTIVE: To review the injuries after TPD in children. MATERIALS AND METHODS: Patients with clinical or radiological recognition of TPD and those with suggestive MR findings were selected. Bone, cartilage and soft-tissue injuries and patellofemoral relationships were assessed. RESULTS: A total of 26 patients (age range 10-18 years) were identified. The following

Biomechanical evaluation of lateral patellar dislocations.

This investigation was undertaken to identify the structures torn within the medial retinaculum and localize the injury site anatomically following acute lateral dislocation of the patella in a cadaver model. The patellae of 10 fresh-frozen cadavers were translated laterally 135% of the patella width on a universal testing instrument. Magnetic resonance imaging (MRI) was performed on all specimens prior to testing and immediately following testing. Anatomical dissection also was performed on the medial retinaculum following testing. Dissection revealed avulsion fractures from the inferomedial

Femoral derotation osteotomy for recurrent patellar dislocation.

PURPOSE: Excessive femoral internal torsion is an important risk factor for patellar dislocation. The aim of the present study was to estimate the effect of derotational osteotomy of the femur on the tibial tubercle trochlear groove (TTTG) distance or patellar tilt angle (PTA) and to report our clinical outcomes of recurrent patellar dislocation after femoral derotation osteotomy. METHODS: A retrospective analysis of 16 patients (17 knees) with recurrent patellar dislocation treated by femoral derotation osteotomy in our department from January 2016 to February 2019 was carried out. The proce

Tibial tuberosity transfer for episodic patellar dislocation.

Numerous surgical techniques have been described to address episodic patellar dislocations. Some of them involve the soft tissues whereas others primarily address a bony correction. Four principal anatomic factors have been identified that increase the risk for episodic patellar dislocations: trochlear dysplasia, patella alta, patellar tilt, and an excessive tibial tubercle-trochlear groove distance. A treatment algorithm has been proposed to correct each of these factors. It includes the tibial tuberosity transfer, which is able to correct both a patella alta and an excessive tibial tubercle-

Acute Irreducible Rotational Patellar Dislocation with Patellar Tendon Rupture: A Case Report.

CASE: In this report, we present a case of acute patellar dislocation with both vertical and longitudinal rotation of the patella. As a result of rotation in the longitudinal axis, the articular surface of the patella faced outward, and the dorsal surface faced the femoral condyle. The patient underwent open reduction. One year after open reduction and soft-tissue repair, the patient had acceptable knee function and no knee pain. CONCLUSION: This case shows how similar cases may be treated with an open reduction to properly address the patellar dislocation.

Predictive Scoring for Recurrent Patellar Instability After a First-time Patellar Dislocation.

PURPOSE: Patellofemoral instability is a common acute knee injury seen in the pediatric population. First-time patellar dislocations usually undergo conservative management, but ~15% to 80% of patients experience recurrent instability. This study aims to develop a prediction model using radiographic parameters of the patellofemoral joint seen on computed tomography scans in different degrees of knee flexion, to determine the risk of recurrence after the first episode of patellofemoral instability. METHODS: A 12-year retrospective case-control study was performed. All patients in a single inst

Patellar lateral closing-wedge osteotomy in habitual patellar dislocation with severe dysplasia.

The "à la carte" surgical strategy for treating patellar instability developed in Lyon, France, is well known. The corrective procedures are planned based on a preoperative analysis of the morphological abnormalities. Among factors responsible for patellofemoral incongruity, patellar dysplasia is among the most challenging to correct. We report a case of habitual patellar dislocation with severe patellar dysplasia that required a complex surgical strategy including patellar lateral closing-wedge osteotomy to improve patellofemoral congruity. This treatment was effective in ensuring stability a

Traumatic patellar dislocation: nonoperative treatment compared with MPFL reconstruction using patellar tendon.

BACKGROUND: Over the long term, acute patellar dislocations can result in patellar instability, with high recurrence rates after nonoperative treatment. PURPOSE: To compare the results of operative (reconstruction of the medial patellofemoral ligament [MPFL]) versus nonoperative treatment of primary patellar dislocation. STUDY DESIGN: Randomized controlled trial; Level of evidence, 1. METHODS: Thirty-nine patients (41 knees) (mean age, 24.2 years; range, 12-38 years) with acute patellar dislocation were randomized into 2 groups. One group was treated nonoperatively with immobilization and p

A Rotated and Laterally Displaced Patellar Dislocation.

BACKGROUND: Patellar dislocations are a common orthopedic emergency with several variants. The rarer variants include rotational dislocations. These often require open reduction in the operating room. CASE REPORT: We report on a case of a combined rotational and lateral patellar dislocation in a young female. We suspected and made the diagnosis of a rotational dislocation after initial unsuccessful attempts at reduction under sedation. With the assistance of our orthopedic colleagues, we were able to perform a reduction of this patient's patella under sedation in the emergency department. WHY

Arthroscopic patellar "bankart" repair after acute dislocation.

We present a case of acute patellar dislocation in a skeletally immature patient treated with arthroscopic medial patellofemoral ligamentous complex repair using suture anchors with a horizontal mattress suture technique. Patellar dislocation is a common problem in the skeletally immature. Treatment is controversial for first-time dislocators because of the high rate of recurrent instability and functional disability in these patients. Surgical repair of the medial restraints may decrease recurrent instability, although anterior knee pain and crepitus is a common finding after open surgical te

MR imaging of patellar dislocation and relocation.

Patellar dislocation and relocation (PDR) typically occurs suddenly after trauma or torsional stress on the extensor mechanism. Clinical evaluation after patellar dislocation/relocation usually reveals a swollen knee that is difficult to examine. Radiographs may show hemathroses and a minority of patients will have a chip fracture of the patella. Magnetic resonance (MR) imaging features seen with PDR include disruption or sprain of the medial retinaculum, lateral patellar tilt or subluxation, lateral femoral condylar and medical patellar osseous contusions, osteochondral injury, damage to Hoff

Prior lateral patellar dislocation: MR imaging findings.

PURPOSE: To illustrate the constellation of magnetic resonance (MR) findings that suggest prior patellar dislocation. MATERIALS AND METHODS: A computer search of all MR examinations of the knee with a coded diagnosis of patellar dislocation performed between February 1988 and October 1990 disclosed 22 cases (11 male and 11 female patients, aged 1-70 years). RESULTS: Eighteen cases (82%) demonstrated hemarthrosis, medical retinacular disruption, and contusion of the lateral femoral condyle. In 11 cases, the patella was displaced from the trochlear sulcus, but in no patient was there complete

[Pathology, diagnosis and therapy of patellar dislocation].

Patella dislocation in sports trauma is a common injury and is influenced by minor alterations or derangements of dynamic or static factors. The sulcus angle should not exceed 138 degrees and the Q angle should not be more than 15 degrees. Patella alta has some influence on the dislocation tendency, and so has weakness and atrophy of the musculus vastus medialis obliquus, causing muscular imbalance. Diagnosis is made via x-ray in tangential as well as a.p. and lateral view. Clinical examination of the patella reveals in lateral dislocation or redislocation can be provoked after positioning. Te

Traumatic Patellar Dislocations in Childhood and Adolescents.

Patellar instability and associated patellar dislocation can result in significant pain, disability, and associated injuries in young athletes. The patellofemoral joint is a complex articulation with stabilizing restraints, both medially and laterally, that help guide the patella into the corresponding trochlear groove as the knee cycles through an extension-to-flexion arc. In addition to soft tissue injuries, many osseous aberrancies can contribute to patellar instability in young athletes, including trochlear dysplasia, patella alta, and axial and coronal plane abnormalities. There is a role
