BACKGROUND: The medial patello-femoral ligament (MPFL) is a major patellar stabiliser whose reconstruction in adults involves graft fixation within a femoral tunnel. In skeletally immature patients, in contrast, the graft is fixed to the soft tissues to allow normal growth. The primary objective of this prospective study was to perform computed tomography (CT) and magnetic resonance imaging (MRI) assessments of medium-term correction of patellar tilt and of the axial patellar engagement index (AEI) after a paediatric variant of MPFL reconstruction in skeletally immature patients.

Patella fracture is either an infrequent or underreported complication after anterior cruciate ligament (ACL) reconstruction using a portion of the patellar ligament. Patella fracture pattern has been related to the mechanism of injury. Direct fractures resulting from impaction forces delivered to the patella are typically stellate or comminuted. From among 312 patients treated with arthroscopic anterior cruciate ligament reconstruction (one-incision, transtibial with the use of ipsilateral bone-patellar tendon-bone autograft) between 1992 and 1994 256 (82%) were reviewed with a mean follow-up of 26.4 months. An aggressive rehabilitative protocol advocated by Shelbourne has been employed. In a consecutive series of 91 patients with 92 injured knees in which the anterior cruciate ligament was irrevocably destroyed, 79 were males; 12 were females. After a 7-year interval one male patient had surgery on his other knee. The insertion tendinitis of the proximal part of the patellar tendon frequently occurs in weight-lifters. In a prospective non-randomized study soft-laserlight (helium/neon-laser, wave length: 632.8 nm, power: 5 mW) was applicated on standardized acupuncture points on 21 knee joints of 15 performance athletes. Pain reduction during sports activity was obtained in 85.7% after the end of therapy and in 81 % in a three months follow-up. Local tenderness improved in 57.1 %. INTRODUCTION: The most frequent procedure in treatment of acute or chronic anterior cruciate ligament (ACL) rupture is the so called bone-tendon-bone reconstruction. A transverse dislocated patella fracture is a rare complication of this procedure with an incidence of 0.23%-2.3%. We studied patients who underwent primary anterior cruciate ligament reconstruction using either the contralateral (N = 434) or ipsilateral (N = 228) autogenous patellar tendon graft to determine the difference between groups for the return of range of motion, quadriceps muscle strength, and return to sports.
