Biphasic Analysis of Cartilage Stresses in the Patellofemoral Joint.

The objective of this study was to examine the state of stress within the solid matrix of articular cartilage in the patellofemoral joint, using anatomically faithful biphasic models of the articular layers, with the joint subjected to physiologic muscle force magnitudes. Finite element models of five joints were created from human cadaver knees. Biphasic sliding contact analyses were performed using FEBio software to analyze the response of the joint from 30 to 60 degrees of knee flexion. Results demonstrated that the collagen matrix always sustains tensile stresses, despite the fact that the

Biomechanics of the patellofemoral joint and its clinical relevance.

An analysis of the patellofemoral joint based on experimental determinations of pressure distributions on the patellar cartilage and vectorial calculations is presented. The extension torque of all quadriceps muscles is similar. The force transmitted to the patella by the retinacula is small in all knee positions. Because the rigid patella has its own lever arms and tendon insertions at different heights, the forces on patellar and quadriceps tendons are unequal. The small patellar contact areas change according to flexion and are dependent on quadriceps force. Retropatellar pressure is thereb

Patellofemoral Joint Replacement for Isolated Patellofemoral Osteoarthritis: Mid- to Long-Term Survivorship and Functional Outcomes.

Background/Objectives: Patellofemoral joint (PFJ) replacement is a bone-preserving option for isolated patellofemoral osteoarthritis; however, reported survivorship and failure patterns remain variable. This study evaluated implant survivorship, functional outcomes, reoperations, and failure mechanisms following PFJ replacement using standard second-generation implant systems, with or without patellar resurfacing. Methods: We retrospectively reviewed a consecutive cohort of 39 patients (48 knees) who underwent PFJ replacement for isolated patellofemoral osteoarthritis between 2011 and 2021. Me

Pressure pain thresholds in adults with patellofemoral pain and patellofemoral joint osteoarthritis: a case-control study.

BACKGROUND AND AIMS: Patellofemoral pain (PFP) and patellofemoral joint osteoarthritis (PFJOA) are common non-self-limiting conditions causing significant pain and disability. The underlying pain pathologies lack consensus with evidence suggesting reduced pressure pain thresholds (PPTs) in adolescent females with PFP and individuals with knee osteoarthritis. A paucity of evidence exists for mixed-sex adults with PFP and PFJOA in isolation. Exploring if pain sensitisation is a dominant feature of PFP and PFJOA may have important implications for the delivery of a patient centred management appr

Joint motion quality in vibroacoustic signal analysis for patients with patellofemoral joint disorders.

BACKGROUND: Chondromalacia, lateral patellar compression syndrome and osteoarthritis are common patellofemoral joint disorders leading to functional and/or structural disturbances in articular surfaces. The objective of the study was to evaluate their impact on joint motion quality via the vibroacoustic signal generated during joint movement analysis. METHODS: Seventy-three patients (30 with chondromalacia, 21 with lateral patellar compression syndrome, and 22 with osteoarthritis) and 32 healthy controls were tested during flexion/extension knee motion for vibroacoustic signals using an accel

Patellofemoral Joint Preservation: Cartilage Restoration, Unloading, and Avoiding Complications.

Patellofemoral (PF) chondral lesions are a common source of knee pain and disability, typically arising from traumatic (instability/fracture) or degenerative (overload) etiologies. This review synthesizes contemporary evidence on PF joint preservation with an emphasis on avoiding complications. We highlight the importance of defining lesion characteristics and identifying the primary mechanical driver(s) of chondral injury. We then summarize current and emerging cartilage restoration options. Finally, we emphasize that durable outcomes often require concomitant biomechanical optimization throu

Cartilage Disease of the Patellofemoral Joint: Realignment, Restoration, Replacement.

Chondral lesions of the patellofemoral joint are common, and when symptomatic they can be difficult to manage. Not only are there various degrees of injury with multiple etiologies, but patellofemoral anatomy is complex and the patient's lower extremity alignment may contribute to the pathology. Treatment depends on the location, size, and depth of the lesion and may require realignment or concomitant stabilizing procedures. Tibial tubercle osteotomy can be performed in isolation or combined with various cartilage-based treatments, including marrow stimulation techniques, autologous chondrocyt

Trochlear groove osteochondritis dissecans of the knee patellofemoral joint.

BACKGROUND: The trochlear groove is the rarest location for osteochondritis dissecans (OCD) of the knee, with only about 50 previously reported cases, most of which were treated before the advent of magnetic resonance imaging (MRI) and modern techniques of cartilage fixation or osteochondral transplantation. The purpose of this multicenter study was to assess the patient presentation and clinical, radiographic, and functional results of treatment for trochlear groove OCD lesions. METHODS: Hospital records from 5 institutions of the Research in Osteochondritis of the Knee (ROCK) study group we

Atraumatic patellofemoral joint disorders. Long-term results of surgery.

Clinical and radiographic follow-up was done on a group of patients with atraumatic patellofemoral joint disorders who had been treated surgically by realignment of the extensor apparatus at the authors' institution from 1985 to 1989. The preoperative radiographic examination in the 65 patients treated (for a total of 74 realignments) evaluated the following parameters: sulcus angle, Insall-Salvati index, and congruence angle. The 46 patients who returned for follow-up (after an average of 2 years and 3 months) were evaluated clinically according to a classification system proposed by the auth

Chondromalacia patellae. The relation to abnormal patellofemoral joint mechanics.

Twenty-two patients treated non-operatively for chondromalacia patellae were followed up after an average of 5.7 years in order to evaluate the effect of abnormal patellofemoral biomechanics on the natural history of the symptoms. Clinical and radiological investigation revealed 15 patients with signs of malalignment, femoral trochlear dysplasia or excessive lateral pressure syndrome, 3 patients with marginal signs of malalignment and 4 patients with no demonstrable abnormalities. A statistically significant difference in the evolution of symptoms between the normal and the abnormal group is d

Clinical outcomes after medial patellofemoral ligament reconstruction: an analysis of changes in the patellofemoral joint alignment.

PURPOSE: Medial patellofemoral ligament (MPFL) reconstruction using the hamstring tendon is widely performed to treat recurrent patellar dislocation. MPFL reconstruction includes a post-operative process of necrosis and reperfusion of the hamstring tendon graft. We hypothesise that the patella gradually shifts laterally because of this process, ultimately affecting the patellofemoral joint alignment. This study aimed to analyse the chronological changes in the patellofemoral joint alignment and the outcomes of MPFL reconstruction. METHODS: In this retrospective case-series study, the Knee Soc

Distribution of patellofemoral joint pressures after femoral trochlear osteotomy.

Anterior osteotomy of the lateral femoral condyle was designed for the treatment of recurrent patellar dislocations. This study examined whether anterior femoral trochlear osteotomy significantly elevates contact pressures in the patellofemoral joint. Static intrajoint loads and contact area determination were recorded using prescale Fuji pressure-sensitive film under static loading. Peak pressures, average pressures and contact areas of the patellofemoral joint were calculated on intact specimens and after anterior osteotomy of the lateral condyle. Our results indicate that a 6-mm or 10-mm an

Cross-sectional imaging of the patellofemoral joint and surrounding structures.

Computed tomography (CT) and magnetic resonance (MR) imaging are extremely useful in the accurate diagnosis of anterior knee pain, a common complaint arising from numerous causes (including fracture, chondromalacia patellae, and alignment and tracking abnormalities). Plain CT is effective for evaluating intraosseous lesions of the knee. Although CT arthrography provides excellent visualization of the patellar articular cartilage, the technique is expensive and invasive. Cine CT is an excellent method for assessing patellofemoral tracking and alignment. Kinematic MR imaging can also perform thi

Osteochondral Allograft Transplantation in the Patellofemoral Joint: A Systematic Review.

BACKGROUND: The initial focus of cartilage restoration algorithms has been on the femur; however, the patellofemoral compartment accounts for 20% to 30% of significant symptomatic chondral pathologies. While patellofemoral compartment treatment involves a completely unique subset of comorbidities, with a comprehensive and thoughtful approach many patients may benefit from osteochondral allograft treatment. PURPOSE: To perform a systematic review of clinical outcomes and failure rates after osteochondral allograft transplantation (OCA) of the patellofemoral joint at a minimum 18-month follow-u

Internal Fixation of Osteochondritis Dissecans Lesions in the Patellofemoral Joint.

Osteochondritis dissecans (OCD) lesions of the patellofemoral joint can be difficult to identify and treat. Asymptomatic or stable lesions in skeletally immature patients may be treated nonoperatively, but symptomatic lesions often require surgical intervention. Evidence of instability should be carefully evaluated with preoperative magnetic resonance imaging or computed tomography arthrogram. Careful preoperative planning is necessary to ensure the appropriate surgical approach and implants are selected for surgical management. Multiple techniques have been described, but internal fixation of
