Standardization of the Cirrhosis Admission Process and Its Impact on Inpatient Management and Patient Outcomes.

INTRODUCTION: An increase in cirrhosis-related hospitalizations has highlighted opportunities for improved patient care and standardization of practice. We created a cirrhosis admission order set with the goal of promoting guideline-directed care and improving patient outcomes. METHODS: We developed and implemented a cirrhosis order set that included dietary recommendations, diagnostic paracentesis orders and studies, common complications of portal hypertension and educational materials at time of discharge. We used a retrospective cohort design to compare post-intervention process measures a

Krukenberg's Spindle-Like Corneal Changes in a Patient With Prolonged ICU Admission: A Case Report.

Krukenberg's spindles are vertical, spindle-shaped pigment deposits on the posterior cornea and are primarily associated with pigment dispersion syndrome (PDS). We present a case of Krukenberg's spindle-like corneal changes in a 39-year-old male patient following prolonged ICU management for thyroid storm complicated by infective endocarditis. The patient's complex ICU course included mechanical ventilation, veno-arterial extracorporeal membrane oxygenation (VA-ECMO), and multiple medications. Ophthalmological examination revealed bilateral, brownish, linear changes in the central corneal stro

Patient perspectives and the risk of compulsory admission: the Amsterdam Study of Acute Psychiatry V.

OBJECTIVE: Compulsory admission to a psychiatric hospital is associated with a three- to fourfold increase in the risk of another compulsory admission. Given the rising numbers of civil detentions in The Netherlands and other European countries, it is important to understand the mechanism behind this association. Our aim is to study the links between opinions about prior psychiatric treatment, insight, service engagement and the risk of (new) civil detentions. METHODS: We took a random sample of 252 from the 2,682 patients consecutively coming into contact with two psychiatric emergency teams

[A case of emergency admission for CO2 narcosis in a patient with amyotrophic lateral sclerosis].

A 68-year-old man with severe dyspnea was admitted as an emergency case. He had no past history of any respiratory or neuromuscular diseases. Immediately after insufflation of oxygen, respiratory arrest occurred. The blood gas analysis showed hypoxemia and severe hypercapnia (PaO2; 32 mmHg, PaCO2; 127 mmHg). We diagnosed as CO2 narcosis, and he was treated with a respirator in the ICU. He showed nonflaccid bilateral diaphragmatic paralysis and muscle atrophy of the upper extremities. As the EMG showed giant spikes of neurogenic pattern, he was diagnosed as ALS. Weaning from the respirator fail

Plasma metabolomics signatures predict COVID-19 patient outcome at ICU admission comparable to clinical scores.

SARS-CoV-2 significantly impacts the human metabolome. This study aims to evaluate the predictive capability of a comprehensive module clustering approach in plasma metabolomics for identifying the risk of critical complications in COVID-19 patients admitted to intensive care units (ICUs). We conducted a prospective monocenter study, gathering blood samples within 24 h of ICU admission, alongside clinical, biological, and demographic patient characteristics. Subsequently, we quantified patients' plasma metabolome using a comprehensive untargeted metabolomics approach. First, we stratified pati

Stability of admission diagnoses; data from a specialized in-patient treatment facility for dual diagnosis.

AIM: We investigated the stability of diagnoses during admission over an 11-year period in patients admitted to a highly specialized integrated dual diagnosis treatment facility in Denmark using diagnosis coded in patient charts. MATERIALS AND METHODS: Admission and discharge diagnoses from patient files were examined for stability of primary diagnosis and association with year of admission, age, sex, and duration of admission, in 1570 patients from 2007 to 2017. RESULTS: A vast proportion (69.6%) of the patients retained their diagnosis during a 3-month admission. Stability was highest for

Recent Patient Characteristics and Medications at Admission and Discharge in Hospitalized Patients With Heart Failure.

BACKGROUND: To improve the clinical outcome of heart failure (HF), it is important to evaluate the etiology and comorbidities of HF. We previously reported the baseline clinical characteristics and medications in hospitalized patients with HF in years 2000 - 2002 (group 2000) and 2007 - 2009 (group 2008). METHODS: We conducted a retrospective study of 158 patients who were hospitalized due to HF between 2012 and 2014 (group 2013) in the Department of Cardiology, Fukuoka University Hospital. We analyzed the clinical characteristics and medications at admission and discharge, and compared the f

Discrepancies between patient report, clinical assessment, and urine analysis in psychiatric patients during inpatient admission.

Self-report and clinical assessment of substance use were compared with urine analysis results in 56 male patients consecutively admitted for inpatient psychiatric treatment. All subjects received DSM-III-R Axis I diagnosis and were classified into diagnostic groups. Urine samples were tested for cocaine, marijuana, opiates, phencyclidine (PCP), amphetamines, and barbiturates. Thirty-five of the 56 patients (62%) produced urine samples that were positive for at least 1 substance of abuse. Of this group, 15 patients (27% of total sample) denied substance use during the week prior to admission.

Associations between day of admission, admission hyponatremia and hospital outcomes in medical patients: A retrospective multicenter cohort study.

INTRODUCTION: We investigated the relationship between admission day, serum sodium concentration, and outcomes in medical inpatients. Hyponatremia, the most common electrolyte abnormality in hospitalized adults, is associated with prolonged hospital length of stay (LOS) and higher mortality. Weekend admissions are also linked to worse outcomes, but the magnitude of this "weekend effect" may vary with diagnoses and particular day of admission. DESIGN: Retrospective multicenter cohort study. SETTING: Four Ministry of National Guard - Health Affairs hospitals in Saudi Arabia (January 1, 2016 -

[On the Admission of Patients with Borderline Personality Disorder to Outpatient Psychotherapy].

BACKGROUND: Patients with borderline personality disorder (BPD) present difficulties in self-regulation and interaction, which is a challenge for psychotherapists that is also addressed in BPD-specific interventions. Against this background, outpatient psychotherapists were surveyed about the factors playing a decisive role in their treatment offer for patients with BPD. METHODS: Psychotherapists for adults were contacted via their email address published on the website of the Kassenärztliche Vereinigung (Association of Statutory Health Insurance Physicians), 231 of whom answered an online qu

Admission diagnoses of patients later diagnosed with autoimmune encephalitis.

BACKGROUND: Since the detection of autoantibodies against neuronal surface antigens, autoimmune encephalitis (AE) has been more frequently diagnosed, especially in patients with symptoms typical of limbic encephalitis, such as seizures, short-term memory deficits, or psychosis. However, the clinical spectrum of AE may be much wider, making correct clinical diagnosis difficult. METHODS: We retrospectively analysed symptoms and admission diagnoses at first clinical presentation in 50 AE patients. We included patients with a clinical diagnosis of AE for whom a FDG-PET imaging was available. Fina

Impact of Hospital Admission for Patients with Transient Ischemic Attack.

OBJECTIVES: To determine the impact of admission among transient ischemic attack (TIA) patients in the emergency department (ED). STUDY DESIGN: Retrospective cohort study using national Veterans Health Administration data (2008). METHODS: We first analyzed whether admitted patients were discharged from the hospital with a diagnosis of TIA. We then analyzed whether admission was associated with a composite outcome (new stroke, new myocardial infarction, or death in the year after TIA) using multivariate logistic regression modeling with propensity score matching. RESULTS: Among 3623 patients

Hospital admission decision for patients with community-acquired pneumonia.

Where to treat patients is probably the single most important decision in the management of community-acquired pneumonia (CAP), with a substantial impact on both patients' outcomes and health-care costs. Several factors can contribute to the decision of the site of care for CAP patients, including physicians' experience and clinical judgment and severity scores developed to predict mortality, as well as social and health-care-related issues. The recognition, both in the community and in the emergency department, of the presence of severe sepsis and acute respiratory failure and the coexistence

Hospital admission due to infections in multiple sclerosis patients.

BACKGROUND AND PURPOSE: Multiple sclerosis (MS) patients are at increased infection risk. Here the influences of susceptibility, severity and surveillance bias on infection-related hospital admission are assessed. METHODS: Swedish registers identified 20,276 patients with MS, matched with 203,951 people from the general population without MS. Risk of first hospital admission for infection and mortality over 36 years was estimated by Poisson regression. RESULTS: Multiple sclerosis was associated with an increased hospital admission risk for all infections, with an adjusted relative risk (and

[Reconsideration of the admission and discharge criteria of tuberculosis patients in Japan].

Japanese Ministry of Health, Labour and Welfare issued new criteria for admission and discharge of tuberculosis patients in 2007. The criteria for admission are extended for the patients of the risk of Mycobacterium tuberculosis transmission and of the possible risk of treatment failure. The criteria for discharge are consisted of the 3 factors, (1) symptoms (cough, fever, etc.) are free after the standard chemotherapy of more than 2 weeks, (2) three different sputum smears are negative for acid-fast bacilli after the standard chemotherapy of more than 2 weeks, (3) patients are estimated to ad

Inappropriate psychiatric admission of elderly patients with unrecognized delirium.

OBJECTIVES: To explore factors that might contribute to misattribution of mental status changes to psychiatric illness when an elderly patient actually has a delirium (mental status changes due to a medical condition). METHODS: Records of 900 elderly patients referred to a Veterans Affairs psychiatric inpatient unit and 413 to an inpatient psychiatric team at a public hospital from 2001 to 2007 were reviewed. Cases referred because of symptoms secondary to an unrecognized delirium underwent further analysis of preadmission assessments. Comparisons were made to elderly patients with delirium a
