Impact of a Thermal Care Bundle (Short Prewarming) on Shivering and Thermal Comfort in Osteosynthesis Patients: A Randomized Controlled Trial.

PURPOSE: Shivering during a surgical procedure has been shown to lead to patient thermal discomfort that can result in an unsatisfactory experience of the whole surgical process besides the physiological consequences that may arise due to the adrenergic activation response. The aim was to determine if implementation of a thermal care bundle with a short prewarming period decreased the incidence of shivering and improved thermal comfort in patients undergoing osteosynthesis. DESIGN: Open-label, single-center randomized controlled trial. METHODS: In total, 148 patients who underwent osteosynth

Cost-effectiveness analysis of a postoperative 48-hour care bundle for high-risk patients undergoing abdominal surgery.

BACKGROUND: Enhancing surgical care pathways, including scaling-up care after surgery, seeks to deliver better patient-centered care and improve outcomes. However, the value associated with these pathways need to be investigated. METHODS: We analyzed the potential increase in efficiency of a new care pathway, a postoperative 48-hour care bundle designed to enhance care for high-risk patients undergoing major abdominal surgery, based on micro-costing and cost-effectiveness analyses. Data from a prospective cohort of patients who underwent a high-risk surgical bundle were compared with those wh

Utilizing a Standardized Bundle of Care to Improve Outcomes for Perinatal Patients with Substance Use Disorder: A Quality Improvement Project.

BACKGROUND: Our interdisciplinary hospital system team used the Alliance for Innovation on Maternal Health (AIM) bundle, Care for Pregnancy and Postpartum People with substance use disorder (SUD), as a framework for program development. Our care teams noted underreporting of perinatal patients with SUD, and once identified, nurses and obstetric providers expressed a knowledge deficit related to the care of this patient population. Using the AIM bundle for guidance, the objectives of this project were to identify pregnant patients with SUD to increase referral rates, provide naloxone at dischar

Effect of a Nurse Practitioner-Led Bundled Care on Postoperative Recovery in Cardiac Surgery Patients: A Randomized Controlled Trial.

Postoperative delirium, pain, and sleep disturbances prolong recovery after cardiac surgery. Evidence for multicomponent, nurse practitioner-led interventions is limited, especially in resource-constrained settings. This study evaluated their effect on delirium, pain, sleep quality, and hospital stay. In a single-blinded, post-test only RCT at a tertiary cardiac hospital in India, 100 adults undergoing elective coronary artery bypass graft, mitral valve replacement, or aortic valve replacement were randomized to nurse practitioner-led bundled care (n = 50) or routine care (n = 50). The interve

A Quality Improvement-based Approach to Implementing a Remote Monitoring-Based Bundle in Transitional Care Patients for Heart Failure.

BACKGROUND: Congestive heart failure (HF) is a leading cause of hospitalization and readmission, leading to increased health care utilization and cost. This is complicated by high incidence, prevalence, and hospitalization rates among racial and ethnic minorities, with a widening in the mortality disparity gap. Remote patient monitoring (RPM) has the potential to proactively engage patients after discharge to optimize medication management and intervene to avoid rehospitalization. However, it also may widen the equity gap due to technological barriers and bias. METHODS: A prospective, observa

The application of patient-centered care bundle significantly reduces incidence of perioperative respiratory complications in hip fracture patients aged 80 and over.

This study was designed as a pilot test to analyze the effect of patient-centered care (PCC) bundle intervention on perioperative respiratory complications and other outcomes in hip fracture patients aged ≥80. Between Jan 2018 and Dec 2019, 198 patients comprised the routine care group and 187 comprised the PCC bundle group. After propensity score matching, 151 remained in each group. Incidence of perioperative respiratory complications in the PCC bundle group was significantly lower than in the routine care group (all P < 0.05). Furthermore, significant reductions were observed in surgery del

The Impact of Outpatient Physical Therapy on Patient-Reported Outcomes After Lumbar Fusion: Perspective from A Bundled Care Dataset.

OBJECTIVE/BACKGROUND: As value-based care grows in popularity across the United States, more payers have turned toward bundled payment models for surgical procedures. Though episode costs in spine are highly variable, physical therapy (PT) has been identified as a driver of 90-day cost. The goal of this study is to assess the impact of postoperative PT on patient-reported outcomes and cost after lumbar fusion surgery using bundled insurance data. METHODS: Bundled payment information of lumbar fusion episodes-of-care (EOC) from 2019 to 2021 was reviewed at a single, urban, tertiary care center

Improving Health Care for Critically Ill Patients Using an Evidence-Based Collaborative Approach to ABCDEF Bundle Dissemination and Implementation.

BACKGROUND: Patients admitted to intensive care units (ICUs) often experience pain, oversedation, prolonged mechanical ventilation, delirium, and weakness. These conditions are important in that they often lead to protracted physical, neurocognitive, and mental health sequelae now termed postintensive care syndrome. Changing current ICU practice will not only require the adoption of evidence-based interventions but the development of effective and reliable teams to support these new practices. OBJECTIVES: To build on the success of bundled care and bridge an ongoing evidence-practice gap, the
