Prognostic and clinicopathological value of prognostic nutritional index in patients with multiple myeloma: a meta-analysis.

BACKGROUND: Prognostic nutritional index (PNI) has been extensively investigated for its effect on forecasting multiple myeloma (MM) survival; however, the conclusions are conflicting. This meta-analysis identified an accurate MM prognosis forecasting role for the PNI. METHODS: We systematically searched PubMed, Web of Science, Embase, Cochrane Library, and CNKI databases until July 2, 2025, and evaluated the overall survival (OS) and progression-free survival (PFS) forecasting ability of the PNI by determining pooled hazard ratios (HRs) with 95% confidence intervals (CIs). RESULTS: This stu

[Oto-vestibular symptoms in patients with temporomandibular joint dysfunction. Electromyographic study].

The authors analyse the otovestibular symptoms (acoupheny, muffled hearing, vertigo, hypoacusia, otalgia) present among other symptoms of craniomandibular dysfunctions. A number of hypotheses have been advanced regarding the etiopathogenesis of these symptoms, but all stem from three theories. The first theory is based on the possibility of a mechanical transmission of force from the temporomandibular joint to the middle ear by the discomalleolar ligament. Another theory hypothesises the possible direct irritation by the condyl on the auriculotemporal nerve. The most widely credited etiopathog

When can I go home after my knee replacement? Factors affecting the duration of in-hospital stay after knee replacement.

BACKGROUND: Despite a sevenfold decline in the number of postoperative nights (21-3) after a total knee arthroplasty (TKA) over the last four decades, predictors of length of stay (LOS) are not fully understood. We attempted to ascertain these factors by analyzing a large cohort of patients. METHODS: Prospectively collected data between January 2016 and March 2017 were retrospectively analyzed at our institute. Charts of 1663 consecutive, simple primary unilateral and bilateral TKAs were reviewed for the LOS excluding staggered bilateral, complex primary and revision knees. STATISTICAL ANALY

The Case for Advocacy Curricula and Opportunities in Medical Education: Past Examples to Inform Future Instruction.

With recent advances in understanding racial, socioeconomic, and mental health issues in medicine and their relation to policy and legislation, medical professionals are increasingly involved in local and national advocacy efforts. At the frontlines of these initiatives are medical students who, in addition to completing required coursework and clinical training, devote themselves to serving patients through civic participation. The burgeoning evidence concerning health care disparities and inequity, along with greater awareness of racial and socioeconomic discrimination, have made advocacy an

How will the 'cancer moonshot' impact health disparities?

In 1971, President Nixon signed into law the National Cancer Act (NCA), colloquially known as the "War on Cancer", which pushed cancer onto the national agenda and is credited for many subsequent increases in the knowledge of the molecular, cellular, and genetic causes and effects of cancer. But even though cancer mortality has declined overall in intervening years after the NCA, cancer health disparities persist in the form of higher cancer incidence and mortality rates among certain cancer types and certain populations. Breast and cervical cancers disproportionately affect African American,

Psychometric evaluation of the Disabilities of the Arm, Shoulder and Hand (DASH) with Dupuytren's contracture: validity evidence using Rasch modeling.

BACKGROUND: Dupuytren's contracture is a progressive, fibroproliferative disorder that causes fixed finger contractures and can lead to disability. With the advances of new therapeutic interventions, the necessity to assess the functional repercussions of this condition using valid, reliable and sensitive outcome measures is of growing interest. The Disabilities of the Arm, Shoulder and Hand (DASH) is one frequently used patient-reported outcome measure but its reliability and validity have never been demonstrated specifically for a population affected with Dupuytren's contracture. Does a pre-hospital emergency pathway improve early diagnosis and referral in suspected stroke patients?--Study protocol of a cluster randomised trial [ISRCTN41456865].

BACKGROUND: Early interventions proved to be able to improve prognosis in acute stroke patients. Prompt identification of symptoms, organised timely and efficient transportation towards appropriate facilities, become essential part of effective treatment. The implementation of an evidence based pre-hospital stroke care pathway may be a method for achieving the organizational standards required to grant appropriate care. We performed a systematic search for studies evaluating the effect of pre-hospital and emergency interventions for suspected stroke patients and we found that there seems to be

Financial Toxicity in Plastic Surgery: A Scoping Review.

INTRODUCTION: Financial toxicity (FT) describes the physical, psychosocial, and financial burden of health care-related costs on patients' well-being. Plastic surgery patients may be susceptible to FT due to high procedure and adjunct therapy costs. Despite these risks, FT resulting from plastic and reconstructive surgery remains unclear. This scoping review assessed the prevalence and impact of FT on plastic surgery patients. METHODS: MEDLINE, Embase, Web of Science Core Collection, Scopus, and the Cumulative Index to Nursing and Allied Health Literature were searched from inception through

De-escalation of aggressive behaviour in healthcare settings: Concept analysis.

BACKGROUND: De-escalation is the recommended first-line response to potential violence and aggression in healthcare settings. Related scholarly activity has increased exponentially since the 1980s, but there is scant research about its efficacy and no guidance on what constitutes the gold standard for practice. OBJECTIVES: To clarify the concept of de-escalation of violence and aggression as described within the healthcare literature. DESIGN: Concept analysis guided by Rodgers' evolutionary approach. DATA SOURCES: Multiple nursing and healthcare databases were searched using relevant terms.

RSV in adult ED patients: Do emergency providers consider RSV as an admission diagnosis?

BACKGROUND: Respiratory Syncytial Virus (RSV) has been recognized for over half a century as a cause of morbidity in infants and children. Over the past 20years, data has emerged linking RSV as a cause of illness in adults resulting in 177,000 annual hospitalizations and up to 14,000 deaths among older adults. OBJECTIVE: Characterize clinical variables in a cohort of adult RSV patients. We hypothesize that emergency physicians do not routinely consider RSV in the differential diagnosis (DDx) of influenza like illness. METHODS: Observational study of all adult inpatients, age≥19, with a posit

COVID 19-associated chilblain-like acral lesions among children and adolescents: an Italian retrospective, multicenter study.

BACKGROUND: Since the COVID-19 pandemic started, great interest has been given to this disease, especially to its possible clinical presentations. Besides classical respiratory symptoms, dermatological manifestations occur quite often among infected and non-infected patients, particularly in children. A prominent IFN-I response, that is generally higher in children compared to adults, may not only cause chilblain lesions, but it could also prevent infection and viral replication, thus justifying the negative swab results, as well as the absence of relevant systemic symptoms in positive cases.

Meta-analysis and systematic review of factors biasing the observed prevalence of congenitally missing teeth in permanent dentition excluding third molars.

No meta-analyses or systematic reviews have been conducted to evaluate numerous potential biasing factors contributing to the controversial results on congenitally missing teeth (CMT). We aimed to perform a rather comprehensive meta-analysis and systematic review on this subject. A thorough search was performed during September 2012 until April 2013 to find the available literature regarding CMT prevalence. Besides qualitatively discussing the literature, the meta-sample homogeneity, publication bias, and the effects of sample type, sample size, minimum and maximum ages of included subjects, g
