Warm Handoff Versus Fax Referral for Linking Hospitalized Smokers to Quitlines.

INTRODUCTION: Few hospitals treat patients' tobacco dependence. To be effective, hospital-initiated cessation interventions must provide at least 1 month of supportive contact post-discharge. STUDY DESIGN: Individually randomized clinical trial. Recruitment commenced July 2011; analyses were conducted October 2014-June 2015. SETTING/PARTICIPANTS: The study was conducted in two large Midwestern hospitals. Participants included smokers who were aged ≥18 years, planned to stay quit after discharge, and spoke English or Spanish. INTERVENTION: Hospital-based cessation counselors delivered the in

Structured Handover in General Surgery: An Audit of Current Practice.

INTRODUCTION: Verbal handover alone compromises patient safety, and supporting written documents significantly increases retention of information, with printed handover sheets being the best at avoiding data loss. The Royal College of Surgeons (RCS) has produced guidelines on safe handover practice, in which a minimum dataset is recommended for inclusion when handing over patients to incoming surgical teams, and studies have indicated better adherence to these guidelines when preprinted handover proformas are used. METHODS: All surgical handover sessions were attended for a one-week period, a

Handoffs and transitions in critical care-understanding scalability: study protocol for a multicenter stepped wedge type 2 hybrid effectiveness-implementation trial.

BACKGROUND: The implementation of evidence-based practices in critical care faces specific challenges, including intense time pressure and patient acuity. These challenges result in evidence-to-practice gaps that diminish the impact of proven-effective interventions for patients requiring intensive care unit support. Research is needed to understand and address implementation determinants in critical care settings. METHODS: The Handoffs and Transitions in Critical Care-Understanding Scalability (HATRICC-US) study is a Type 2 hybrid effectiveness-implementation trial of standardized operating

The potential contribution of deficiencies in handoffs between anaesthetists to the fatal outcome following ventral slot surgery in a dog.

Increasingly complex cases in veterinary anaesthesia challenge anaesthetists, especially when patient care is transferred (handed off) between team members. Standardized and structured patient handoffs are used in human medicine to improve patient safety. A male neutered Dachshund, aged 9 years, with a history of cervical pain presented for magnetic resonance imaging. The medical history included mitral valve insufficiency (grade B1) and intra-anaesthetic bradycardia and apnoea at a different clinic. Induction of anaesthesia was performed by a fourth year anaesthesia resident and the dog hande

Surgical specimen handover from the operating theatre to laboratory-Can we improve patient safety by learning from aviation and other high-risk organisations?

Essential communication between healthcare staff is considered one of the key requirements for both safety and quality care when patients are handed over from one clinical area to other. This is particularly important in environments such as the operating theatre and intensive care where mistakes can be devastating. Health care has learned from other high-risk organisations (HRO) such as aviation where the use of checklists and human factors awareness has virtually eliminated human error and mistakes. To our knowledge, little has been published around ways to improve pathology specimen handove

Behavioral health referrals and patient engagement in integrated primary care: a brief report.

Objective: The current study explored how and why primary care providers make referrals to embedded behavioral health clinicians (BHCs) and related outcomes. Method: Primary care patients referred to BHCs were followed for 15 weeks, across five student health centers. Results: Of the 7395 patients who attended medical appointments in the first 4 weeks of the study, 170 (2%) were referred to BHCs. Of these students, 126 (74%) attended at least one BHC appointment by week 15. The two most common referral methods were scheduling (58%) and warm handoff (35%). Patients referred by warm handoff were

One Notable Complication of Nasopharyngeal Airway: A Case Report.

INTRODUCTION: The nasopharyngeal airway (NPA) is used by emergency providers and first responders to assist with oxygenation in obtunded, critically ill patients. There are few recorded NPA complications. CASE REPORT: We describe a unique case in which a patient went multiple days with recurrent symptoms of upper airway obstruction secondary to retained NPA. DISCUSSION: Nasopharyngeal airways may be uniquely prone to being displaced and retained due to their use in emergent situations, their small size, and time of insertion in the field prior to emergency department (ED) contact where hando

Efficiency Meets Safety: Transforming Operating Theatre-To-Paediatric Intensive Care Unit Transfers in a Paediatric Hospital.

BACKGROUND: Transfers from the Operating Theatre (OT) to the Paediatric Intensive Care Unit (PICU) represent a high-risk transition for critically ill children. At a Tertiary Paediatric Hospital, the absence of a standardised handover protocol resulted in prolonged recovery waits of up to 120 min, medication errors, haemodynamic instability, and reduced family and staff satisfaction. AIM: To enhance patient safety, reduce handover time, and improve family and staff satisfaction through a structured Operating Theatre-to-PICU handover process. STUDY DESIGN: Single-centre, pre-post quality impr

Predictors of engagement in post-discharge quitline counseling among hospitalized smokers.

Quitlines provide evidence-based tobacco treatment and multiple calls yield higher quit rates. This study aimed to identify subgroups of smokers with greater quitline engagement following referral during hospitalization. Data were from a randomized clinical trial assessing the effectiveness of fax referral (referral faxed to proactive quitline) versus warm handoff (patient connected to quitline at bedside) (n = 1054). Classification and regression trees analyses evaluated individual and treatment/health system-related variables and their interactions. Among all participants, warm handoff, high

From Emails to EMR: Implementing I-PASS Among Inpatient Palliative Care Clinicians at a Comprehensive Cancer Center-A Quality Improvement Initiative.

BACKGROUND: Inpatient palliative care consultation services operate with an interdisciplinary team, where effective handoffs are crucial for coordinated patient care. We aimed to replace encrypted email handoffs with a more concise and uniform handoff using I-PASS (illness severity, patient summary, action list, situational awareness, contingency planning, and synthesis by receiver) integrated within the electronic medical record (EMR). Aim and Measures: Within six months of launch, our goal was to achieve 90% I-PASS utilization for hospitalized acutely ill patients with cancer receiving palli

A qualitative study of handovers at shift changeovers in five care homes for older people in England.

BACKGROUND: It is widely acknowledged that inadequate handovers are associated with putting patients at risk in clinical settings; however, handover practices have received little attention in other 24-hr settings such as long-stay residential care facilities. AIM: This study aimed to explore the perceived purpose and organisational processes involved in the handover of information between shifts of staff caring for older residents in five care homes in England. METHODS: The study took an ethnographic approach to fieldwork, undertaken between February and June 2016. It consisted of observati

Exploring Practices of Bedside Shift Report and Hourly Rounding. Is There an Impact on Patient Falls?

OBJECTIVE: To describe the relationship of inpatient falls to bedside shift report (BSR) and hourly rounding (HR). BACKGROUND: Falls are a major healthcare concern. Although measures such as BSR and HR are reported to reduce falls, studies are often based on self-reported data related to nurse compliance with protocols for HR and bedside report. METHODS: Observational data were collected on nursing tasks, including BSR and HR. RESULTS: Nine thousand six hundred ninety-three observations were recorded on 11 units at 4 hospitals over 281 shifts. Falls were associated with shift and day of the

Cross-cultural adaptation and psychometric validation of the ISBAR structured handover observation tool for ICU-to-ward patient transfer in China.

The Identity, Situation, Background, Assessment, Recommendation (ISBAR) framework is used to standardize clinical handovers. Effective ISBAR implementation requires both training and objective assessment of handover. In Chinese clinical settings, handover quality is commonly evaluated through self-reports or locally developed checklists, which provide limited information about the completeness and accuracy of information transfer. The absence of validated Chinese observational instruments restricts the identification of communication deficiencies, benchmarking of handover performance, and deve
