[Meralgia paraesthetica as complication of patient positioning : A not fully controllable risk].

Meralgia paraesthetica (MP) is the consequence of an entrapment or stretch injury of the lateral femoral cutaneous nerve at the crossing region with the inguinal ligament where the nerve exits the pelvis. It results in temporary or permanent sensory loss, paraesthesia and pain in the anterolateral region of the thigh. Idiopathic forms are known for example as seat belt syndrome or jeans syndrome. An MP can also occur as a complication of surgical or intensive care patient positioning. In focus are the lithotomy position, prone position and beach chair position. We analyzed 21 complaints about M

[Important Points for Patient Positioning and Head Fixation in Cerebral Aneurysm Surgery].

Patient positioning and head fixation are two of the most important aspects of cerebral aneurysm surgery. These procedures require an accurate understanding of the anatomy of the scalp and skull and of the process for approaching aneurysms. We describe the basics of this procedure in frontotemporal craniotomy, which is most frequently used in cerebral aneurysm surgery, with a focus on head fixation using the MAYFIELD three-pin skull clamp(Integra LifeSciences). The insertion sites of the head pins should be avoided in areas with thin bone, such as the frontal sinus and temporal squama, and jus

Brachialis syndrome: a rare consequence of patient positioning causing postoperative median neuropathy.

BACKGROUND: Poor positioning of patients can result in devastating permanent neurologic deficits. We describe a previously unreported cause of median nerve compression that we have termed the brachialis syndrome, associated with patient positioning that results in permanent median nerve damage. METHODS: We identified this condition affecting 6 median nerves. All patients underwent surgical decompression of the proximal median nerve at the level of the antecubital fossa. RESULTS: Five patients presented with symptoms of median nerve compression; 6 affected median nerves manifested brachialis

Patient positioning for the operative approach to midline intracerebral lesions: technical note.

Patients undergoing craniotomies for intracerebral lesions are generally positioned in such a way that the lesion is highest in the field. For midline lesions alongside the falx, patients are commonly positioned on their backs for an anatomically symmetrical approach. We propose positioning the patient in the lateral decubitus position ipsilateral to the lesion to take advantage of gravity by allowing the brain to fall away from the midline, thus obviating the need for retraction. We have used this position in 15 cases of falcine and parafalcine tumors to great advantage, without encountering

Effect of patient positioning on urethral mobility: implications for radical pelvic surgery.

Patient positioning is of critical importance in the successful performance of several radical pelvic operations. Adequate exposure of the prostatic apex is vital in ligating the dorsal vein complex and performing vesicourethral anastomosis. To test the effect of different patient positions on urethral mobility, we conducted a prospective controlled study measuring the relative cephalad displacement of the prostatic apex in 3 positions, that is the supine, frog-leg and dorsal lithotomy positions. A combination of radiographic and cystoscopic techniques was used to mark the position of the pros

[Determining responsibility for surgical positioning of the patient and malpractice for damage caused by patient position].

The cooperation of surgeon and anaesthetist in positioning of the patient is subject to the principles of horizontal division of labour recognized in the interdisciplinary agreement and confirmed by the legislature: anaesthetist and surgeon carry out their respective tasks independently of each other, each bearing full responsibility for their own work (principle of strict separation of functions), they tailor their procedures to fit in with each other (duty of coordination), and each is entitled to expect and rely on due care in the other (principle of trust). In the case of conflict--when th

Patient Positioning and Treatment Techniques in Total Skin Irradiation: A Scoping Review.

Introduction: Total skin irradiation (TSI) is a radiotherapy technique used to treat cutaneous lymphomas, such as mycosis fungoides. A key aspect of the success of the treatment is the correct positioning of the patient, which ensures a homogeneous distribution of radiation over the skin surface, minimizing exposure to the surrounding healthy tissues. Materials and Methods: Following the preferred reporting items for systematic review and meta-analysis (PRISMA) extension for scoping reviews and the Arksey and O'Malley framework, electronic searches of EMBASE, PubMed, SCOPUS, and the Web of Sci

Cone beam CT doses in radiotherapy patient positioning in Finland-prostate treatments.

Imaging parameters, frequencies and resulting patient organ doses in treatments of prostate cancer were assessed in Finnish radiotherapy centres. Based on a questionnaire to the clinics, Monte Carlo method was used to estimate organ doses in International Commission on Radiological Protection standard phantom for prostate, bladder, rectum and femoral head. The results show that doses from cone beam computed tomography imaging have reduced compared to earlier studies and are between 3.6 and 34.5 mGy per image for the above-mentioned organs and for normal sized patients. There still is room for

Postoperative Glaucoma Decompensation Following Spine Surgery: The Importance of Proper Patient Positioning.

Transforaminal lumbar interbody fusion (TLIF) is a widely utilized surgical procedure for the treatment of degenerative lumbar spinal conditions, including lumbar disc herniations, spinal stenosis, and spondylolisthesis. One such rare and underreported complication is vision loss following spinal surgery. Postoperative vision loss (POVL) is an extremely uncommon complication, occurring in approximately 0.002% to 0.2% of all non-ocular surgeries, including spinal procedures. We presented a 70-year-old male with complaints of left-sided cruralgia, lumbar radicular pain (sciatica-type L5), and ri

Effect of patient positioning on toe pressure measurement using noninvasive vascular testing.

OBJECTIVE: Interpretation of digital pressure in the assessment of healing potential for diabetic foot disease has become common because of the potential for false elevation with the ankle-brachial index. However, the specific testing protocol for segmental Doppler examinations and photoplethysmography require patients to be in the supine position, with the lower limbs at heart level, in order to minimise the effect of hydrostatic pressure. This may be difficult in many patients with lower extremity pathology, particularly those who are nonambulatory, with painful wounds, or with orthopnea. In

Evaluation of the patient positioning during DXA measurements in daily clinical practice.

For prompt diagnosis of osteoporosis, properly instructed dual-energy X-ray absorptiometry (DXA) measurements are required. To get correct DXA results in addition to quality control procedures, proper positioning of specific body regions such as hip and spine should also be achieved. The aim of this study was to assess the accurateness of the patient positioning for hip and spine regions in daily clinical practice. Hip and spine scan images of 113 DXA reports were evaluated for whether or not the patient was positioned correctly. Out of 113 hip and spine DXA reports, 61 hips and 94 spines were

Optimal positioning for an adult athetoid cerebral palsy patient in a wheelchair.

The purpose of this case study was specifically to reinforce the necessity for proper positioning in a wheelchair for an adult athetoid cerebral palsy patient. A 36-year old female with cerebral palsy sat in the wheelchair with pelvic sliding, uneven weight distribution on the hips, hip adduction, and right lateral trunk tilt and rotation. The abnormal posture observed was first corrected by seating the patient well back in the wheelchair with the patient's weight evenly distributed between both hips. A seat belt, abduction pommel, trunk belt, seat cushion, and foot pad were inserted to help t

Resuscitation of the pregnant patient: What is the effect of patient positioning on inferior vena cava diameter?

STUDY OBJECTIVE: Patients in the third trimester of pregnancy presenting to the emergency department (ED) with hypotension are routinely placed in the left lateral tilt (LLT) position to relieve inferior vena cava (IVC) compression from the gravid uterus thereby increasing venous return. However, the relationship between patient position and proximal intrahepatic IVC filling has never assessed directly. This study set out to determine the effect of LLT position on intrahepatic IVC diameter in third trimester patients under real-time visualization with ultrasound. METHODS: This prospective obs

The impact of patient positioning on pressure ulcers in patients with severe ARDS: results from a multicentre randomised controlled trial on prone positioning.

PURPOSE: Placing patients with severe acute respiratory distress syndrome (ARDS) in the prone position has been shown to improve survival as compared to the supine position. However, a higher frequency of pressure ulcers has been reported in patients in the prone position. The objective of this study was to verify the impact of prone positioning on pressure ulcers in patients with severe ARDS. METHODS: This was an ancillary study of a prospective multicentre randomised controlled trial in patients with severe ARDS in which the early application of long prone-positioning sessions was compared
