A Multidisciplinary Standardized Patient Simulation for Using Trauma-Informed Care for Pregnant Patients.

INTRODUCTION: Pregnant patients with prior traumatic experiences and their providers face challenges during prenatal care, peripartum, and postpartum. To date, no structured simulations have been published focused on improving care for patients in subsequent pregnancies. METHODS: This multidisciplinary simulation included trainees and providers. Standardized patients were used. The patient was multiparous with a remote history of substance use and was initiating prenatal care late due to concerns related to the prior traumatic experience of losing custody of a newborn after a Department of Ch

Pediatric Simulation of Intrinsic PEEP and Patient-Ventilator Trigger Asynchrony During Mechanical Ventilation.

BACKGROUND: Intrinsic PEEP during mechanical ventilation occurs when there is insufficient time for expiration to functional residual capacity before the next inspiration, resulting in air trapping. Increased expiratory resistance (RE), too rapid of a patient or ventilator breathing rate, or a longer inspiratory to expiratory time ratio (TI/TE) can all be causes of intrinsic PEEP. Intrinsic PEEP can result in increased work of breathing and patient-ventilator asynchrony (PVA) during patient-triggered breaths. We hypothesized that the difference between intrinsic PEEP and ventilator PEEP acts a

Numerical Simulation of Blood Flows in Patient-specific Abdominal Aorta with Primary Organs.

The abdominal aorta is the largest artery in the abdominal cavity that supplies blood flows to vital organs through the complex visceral arterial branches, including the celiac trunk (the liver, stomach, spleen, etc.), the renal arteries (the kidneys) and the superior and inferior mesenteric arteries (the small and large intestine, pancreas, etc.). An accurate simulation of blood flows in this network of arteries is important for the understanding of the hemodynamics in various organs of healthy and diseased patients, but the computational cost is very high. As a result, most researchers choos

Patient-Specific Patch for an Intra-Atrial Rerouting Procedure Developed Through Surgical Simulation.

BACKGROUND: In pediatric cardiac surgery, an application of three-dimensional (3D) modeling to develop custom-made prostheses is limited, and currently surgeons use their intraoperative visual estimation to develop 3D complex structures from 2D patch materials. Contemporary 3D designers are developing complex surfaces using surface modeling in other industries, which can be applied to pediatric cardiac surgery. However, its free-form nature may lead to intradesigner variability. METHODS: A patient with a body weight of 4 kg with partial anomalous pulmonary venous connection and preoperative c

Simulation-Based Mastery Learning for Thoracentesis Skills Improves Patient Outcomes: A Randomized Trial.

PURPOSE: Physicians-in-training often perform bedside thoracenteses in academic medical centers, and complications are more common among less experienced clinicians. Simulation-based mastery learning (SBML) is one potential solution to this problem. This study evaluated the effects of a randomized trial of thoracentesis SBML on patient complications: iatrogenic pneumothorax (IP), hemothorax, and reexpansion pulmonary edema (REPE). METHOD: The authors randomized internal medicine residents to undergo thoracentesis SBML at a tertiary care academic center from December 2012 to May 2016. They sub

Design and implementation of a simulation exercise for teaching confidentiality of patient information.

Allied health students, making the transition from a purely academic to a professional school setting, are suddenly faced with judgment decisions about disclosure of medical information. Obscure guidelines and new interpersonal relationships with other members of the health care team complicate this transition and pose a threat to confidentiality of patient information. This article describes the design and implementation of a simulation exercise to reinforce lecture guidelines specifying disclosure of medical information without risk to the patient or student. The simulation is comprised of 1

Simulation of oculomotility in Craniosynostosis patients.

Craniosynostosis syndromes can be associated with missing extraocular muscles, or muscles with abnormal insertions, and so provide useful test cases for assessing our understanding of the mechanics of the extraocular muscles. Patient with craniosynostosis syndromes often show eye movements in which a horizontal movement by one eye is accompanied by upshoot or downshoot in the other eye. An hypothesis which has been put forward to explain these movements is that the muscles in the patients are excyclorotated and that the upshoots and downshoots follow directly from the application of Hering's l

A new cardiology patient simulator.

We report a new cardiology patient simulator developed through the application of new digital computer technology, capable of playing back selected physical findings (jugular-venous waves, arterial pulses of carotid, brachial, radial and femoral arteries, and cardiac impulses), as well as cardiac sounds, murmurs and respiratory sounds that have been prerecorded from actual patients. The examining physician uses an ordinary type of stethoscope. Four built-in speakers are located in the human-sized mannequin at the classic auscultatory sites (aortic, pulmonic, tricuspid and mitral). This compact

Left Ventricular Outflow Tract Diameter Variability and Prosthesis-Patient Mismatch: A Simulation Study.

A recent discussion that highlighted the differences between the reported effective orifice area and corresponding incidence of prosthesis-patient mismatch within the 1- and 7-year results of the PERIcardial SurGical AOrtic Valve ReplacemeNt (PERIGON) Trial of the Avalus valve has once again emphasized the instability of such measurements. The effective orifice area, calculated using the continuity equation through measurements of the left ventricular outflow tract diameter, is highly susceptible to measurement variability, as previously demonstrated in literature through both intra- and inter

Simulation training improves diagnostic performance on a real patient with similar clinical findings.

BACKGROUND: Training on a cardiopulmonary simulator improves subsequent diagnostic performance on the same simulator. But data are lacking on transfer of learning. The objective of this study was to determine whether training on a cardiorespiratory simulator improves diagnostic performance on a real patient. METHODS: We randomly allocated first-year medical students at the University of Calgary to simulator training in one of three clinical scenarios of acute-onset chest pain: pulmonary embolism with right ventricular strain but no murmur, symptomatic aortic stenosis, or myocardial ischemia c

An efficient procedure for the blood flow computer simulation of patient-specific aortic dissections.

In this work we present a novel methodology for the numerical simulation of patient-specific aortic dissections. Our proposal, which targets the seamless virtual prototyping of customized scenarios, combines an innovative two-step segmentation procedure with a CutFEM technique capable of dealing with thin-walled bodies such as the intimal flap. First, we generate the fluid mesh from the outer aortic wall disregarding the intimal flap, similarly to what would be done in a healthy aorta. Second, we create a surface mesh from the approximate midline of the intimal flap. This approach allows us to
