Supportive Care: Integration of Patient-Centered Kidney Care to Manage Symptoms and Geriatric Syndromes.

Dialysis care is often associated with poor outcomes including low quality of life (QOL). To improve patient-reported outcomes, incorporation of the patient's needs and perspective into the medical care they receive is essential. This article provides a framework to help clinicians integrate symptom assessment and other measures such as QOL and frailty scores into a clinical approach to the contemporary supportive care of patients with advanced CKD. This approach involves (1) defining our understanding of kidney supportive care, patient-centered dialysis, and palliative dialysis; (2) understan

Shared decision-making across the urinary incontinence care continuum: implications for patient-centered care.

PURPOSE OF REVIEW: Despite the high prevalence of urinary incontinence, many women do not receive care. Urinary incontinence requires multiple personalized decisions across the care continuum. This review examines how shared decision-making and decision science may inform efforts to improve the delivery of patient-centered urinary incontinence care. RECENT FINDINGS: Recent literature describes the complexity of patients' decision-making across multiple stages of the urinary incontinence care continuum, including deciding whether to mention urinary symptoms, to seek an appropriate clinician or

Moving Person-Centered Care Planning From Theory Into Practice: Patient Perspectives.

INTRODUCTION: Persons with multiple chronic conditions often receive care from a complicated network of multiple providers across multiple health systems unique to their conditions, needs, available resources, and insurance. Patients must navigate this healthcare system, which is a difficult and complicated process with just one provider or system, let alone multiple. Developing new solutions to overcome the systemic challenges within healthcare systems to provide high-quality, comprehensive, person-centered care for persons living with, or at risk for, multiple chronic conditions is critical.

Patient-centered care in von Willebrand disease: are we there yet?

INTRODUCTION: Von Willebrand Disease is the most common inherited bleeding disorder. Paradoxically, affected individuals are often misdiagnosed and experience substantial diagnostic delay. There are sex-specific health disparities in VWD rooted in the stigmatization of vaginal bleeding, which leads to symptom dismissal, lack of timely access to care and lower health-related quality of life. AREAS COVERED: Following the core elements of patient-centered care - respect for patient preferences, values, and needs, we describe the current state of VWD care. Challenges of diagnostic delay, serial m

[Treatment of psoriasis vulgaris : Therapy strategies for optimal patient-centered care].

Psoriasis is a chronic inflammatory systemic disease that requires optimal long-term management due to its high prevalence in the population and the numerous comorbidities that severely impair quality of life. A variety of treatment options are now available. In addition to objective skin findings and a specific location such as nails or genital area, the presence of psoriatic arthritis and other comorbidities as well as the disease burden of the affected person play a decisive role in individualized treatment decision-making. Good communication with the patient is fundamental to understand th

Patient-Centered Care Transitions After Injury Hospitalization: A Comparative Effectiveness Trial.

OBJECTIVE: The investigation aimed to compare two approaches to the delivery of care for hospitalized injury survivors, a patient-centered care transition intervention versus enhanced usual care. METHOD: This pragmatic comparative effectiveness trial randomized 171 acutely injured trauma survivors with three or more early postinjury concerns and high levels of emotional distress to intervention (I; n = 85) and enhanced usual care control (C; n = 86) conditions. The care transition intervention components included care management that elicited and targeted improvement in patients' postinjury c

Patient-Centered Perioperative Care for a Victim of Military Sexual Trauma.

INTRODUCTION: The patient population seen in our nation's Veterans Affairs Healthcare system is increasingly female and an alarming percentage of our veterans, male and female alike, report a history of military sexual trauma (MST), which is associated with an increased burden of morbidities including post-traumatic stress disorder (PTSD) and substance abuse. The experience of surgery can produce symptoms of PTSD in a clinically significant percentage of patients. This article describes the challenges of achieving a patient-centered perioperative care plan in the case of a female veteran who s

A digital, patient-centered carepath for cardiac surgical care with continuous remote patient monitoring.

Postoperative complications and readmissions remain common after cardiac surgery, and intermittent manual vital-sign checks leave patients unmonitored for much of the recovery period. We implemented a phygital care pathway combining Enhanced Recovery After Surgery (ERAS) protocols, and 21-day continuous remote monitoring (heart rate, respiratory rate, temperature, blood pressure, oxygen saturation) with daily review and escalation to a cardiac surgeon when indicated. Quality of Recovery (QoR-15) was assessed on preoperative days -1 and postoperative days 3 and 7, and patient-reported experienc

Association Between Patient-Reported Outcomes and Health Care Resource Utilization: Toward Developing Patient-Centered Value Measures in Cancer Care.

PURPOSE: Value of care is defined as patient-relevant outcomes achieved per dollar spent. We examined the association between patient-reported outcomes (PROs) and health resource utilization in the year after cancer diagnosis, with a view to develop PRO-based measures of value in cancer care. METHODS: This was a population-based cohort study of adults with cancer in the province of Ontario, Canada, between 2008 and 2019. The exposure was symptom burden measured using the Edmonton Symptom Assessment System (ESAS) scores. The outcome was total health care cost 30 days following ESAS reporting,

Cultural sensitivity and patient-centered care regarding incense and candle smoke emissions.

Incense and candles are commonly utilized for relaxation, deodorization, aesthetics, and religious observance. Both are used ubiquitously and viewed innocuously. However, both modalities release a multitude of compounds, including carbon monoxide, benzene, acrolein, and polycyclic aromatic hydrocarbons, that carry documented risks for cardiovascular disease, pulmonary dysfunction, and cancer. Incense particulate matter generates 45 mg/g vs. 10 mg/g from cigarette smoke. The following two clinical cases illustrate this challenge: an 87-year-old woman with chronic obstructive pulmonary disease a

Patient-Centered Chronic Wound Care Mobile Apps: Systematic Identification, Analysis, and Assessment.

BACKGROUND: The prevalence of chronic wounds is predicted to increase within the aging populations in industrialized countries. Patients experience significant distress due to pain, wound secretions, and the resulting immobilization. As the number of wounds continues to rise, their adequate care becomes increasingly costly in terms of health care resources worldwide. eHealth support systems are being increasingly integrated into patient care. However, to date, no systematic analysis of such apps for chronic wounds has been published. OBJECTIVE: The aims of this study were to systematically id
