Consensus on identifying and ranking ventilator asynchronies in invasively ventilated ICU patients: a modified Delphi study (SYNAPsE).

PURPOSE: Despite extensive research, it remains unclear which patient-ventilator asynchronies are reliably detectable in clinical practice, most clinically relevant, and how they rank in severity. METHODS: Multiple-choice questions and 5-point Likert-scale statements were used in iterative Delphi rounds. Feedback was incorporated until stable consensus or dissensus was reached for all items. First series of rounds focused on identifying and classifying patient-ventilator asynchronies detectable from ventilator waveforms, second series assessed their associations with outcomes in three patient

Classification of patient-ventilator asynchronies, development of an updated conceptual framework: Scoping review.

PURPOSE: Patient-ventilator asynchrony (PVA), a mismatch between a patient's respiratory effort and the ventilator's support, affects up to 85% of patients receiving invasive mechanical ventilation (IMV). It is associated with prolonged ventilation, increased complications, and higher mortality. Existing classification systems for PVA are inconsistent, difficult to apply in clinical settings, and not validated for integration with advanced technologies. This scoping review aimed to evaluate current PVA classification systems and propose an operational, flexible framework suitable for intensive

Parasternal electromyography to determine the relationship between patient-ventilator asynchrony and nocturnal gas exchange during home mechanical ventilation set-up.

INTRODUCTION: Patient-ventilator asynchrony (PVA) can adversely affect the successful initiation of non-invasive home mechanical ventilation (HMV). The aim of this observational study was to quantify the prevalence of PVA during initiation of HMV and to determine the relationship between PVA and nocturnal gas exchange. METHOD: Type and frequency of PVA were measured by surface parasternal intercostal muscle electromyography, thoracoabdominal plethysmography and mask pressure during initiation of HMV. Severe PVA was defined, as previously, as asynchrony affecting ≥10% of breaths. RESULTS: 28

Emergency and critical care medicine residents' competency to identify patient ventilator asynchrony using a mechanical ventilator waveform analysis in Addis Ababa, Ethiopia: a multicenter cross-sectional study.

BACKGROUND: patient-ventilator asynchrony (PVA) describes a condition in which a suboptimal interaction occurs between a patient and a mechanical ventilator. It is common and often undetected, with a negative impact on patient outcomes if unrecognized and addressed. Mechanical ventilator waveform analysis is a non-invasive and reliable way of identifying PVAs for which advanced methods of identifying PVA are lacking; however, it has not been well studied in residents working in developing setups like Ethiopia. OBJECTIVES: to assess Emergency and Critical Care Medicine (ECCM) Residents' compet

Effect of patient-ventilator asynchrony on lung and diaphragmatic injury in experimental acute respiratory distress syndrome in a porcine model.

BACKGROUND: Patient-ventilator asynchrony during mechanical ventilation may exacerbate lung and diaphragm injury in spontaneously breathing subjects. We investigated whether subject-ventilator asynchrony increases lung or diaphragmatic injury in a porcine model of acute respiratory distress syndrome (ARDS). METHODS: ARDS was induced in adult female pigs by lung lavage and injurious ventilation before mechanical ventilation by pressure assist-control for 12 h. Mechanically ventilated pigs were randomised to breathe spontaneously with or without induced subject-ventilator asynchrony or neuromus

Application of a cloud platform that identifies patient-ventilator asynchrony and enables continuous monitoring of mechanical ventilation in intensive care unit.

BACKGROUND: Patient-ventilator asynchrony (PVA) frequently occurs in mechanically ventilated patients within the ICU and has the potential for harm. Depending solely on the health care team cannot accurately and promptly identify PVA. To address this issue, our team has developed a cloud-based platform for monitoring mechanical ventilation (MV), comprising the PVA-RemoteMonitor system and the 24-h MV analysis report. We conducted a survey to evaluate physicians' satisfaction and acceptance of the platform in 14 ICUs. METHODS: Data from medical records, clinical information systems, and ventil

Challenges and strategies for the identification and correction of patient-ventilator asynchronies in daily practice: a scoping review.

Patient-ventilator asynchronies (PVA) are associated with adverse clinical outcomes, including prolonged ventilatory support and increased mortality both in the intensive care unit (ICU) and at the hospital level. Despite their clinical relevance, the detection and management of PVA remain challenging in daily practice because of multiple factors, such as the lack of continuous monitoring and the low recognition by healthcare professionals. The aim of this review is to map and synthesize the available evidence on the main challenges in the identification and management of PVA and to describe t

Comparison of patient-ventilator asynchrony during pressure support ventilation and proportional assist ventilation modes in surgical Intensive Care Unit: A randomized crossover study.

BACKGROUND: The patient-ventilator asynchrony is almost observed in all modes of ventilation, and this asynchrony affects lung mechanics adversely resulting in deleterious outcome. Innovations and advances in ventilator technology have been trying to overcome this problem by designing newer modes of ventilation. Pressure support ventilation (PSV) is a commonly used flow-cycled mode where a constant pressure is delivered by ventilator. Proportional assist ventilation (PAV) is a new dynamic inspiratory pressure assistance and is supposed to be better than PSV for synchrony and tolerance, but rep

Respiratory muscle activity and patient-ventilator asynchrony during different settings of noninvasive ventilation in stable hypercapnic COPD: does high inspiratory pressure lead to respiratory muscle unloading?

INTRODUCTION: High-intensity noninvasive ventilation (NIV) has been shown to improve outcomes in stable chronic obstructive pulmonary disease patients. However, there is insufficient knowledge about whether with this more controlled ventilatory mode optimal respiratory muscle unloading is provided without an increase in patient-ventilator asynchrony (PVA). PATIENTS AND METHODS: Ten chronic obstructive pulmonary disease patients on home mechanical ventilation were included. Four different ventilatory settings were investigated in each patient in random order, each for 15 min, varying the inspi

Convolutional long short-term memory neural network integrated with classifier in classifying type of asynchrony breathing in mechanically ventilated patients.

BACKGROUND AND OBJECTIVE: Asynchronous breathing (AB) occurs when a mechanically ventilated patient's breathing does not align with the mechanical ventilator (MV). Asynchrony can negatively impact recovery and outcome, and/or hinder MV management. A model-based method to accurately classify different AB types could automate detection and have a measurable clinical impact. METHODS: This study presents an approach using a 1-dimensional (1D) of airway pressure data as an input to the convolutional long short-term memory neural network (CNN-LSTM) with a classifier method to classify AB types into

Effects of sedatives and opioids on trigger and cycling asynchronies throughout mechanical ventilation: an observational study in a large dataset from critically ill patients.

BACKGROUND: In critically ill patients, poor patient-ventilator interaction may worsen outcomes. Although sedatives are often administered to improve comfort and facilitate ventilation, they can be deleterious. Whether opioids improve asynchronies with fewer negative effects is unknown. We hypothesized that opioids alone would improve asynchronies and result in more wakeful patients than sedatives alone or sedatives-plus-opioids. METHODS: This prospective multicenter observational trial enrolled critically ill adults mechanically ventilated (MV) > 24 h. We compared asynchronies and sedation d

Asynchrony: Clinical Relevance, Detection, and Resolution.

Patient-ventilator asynchrony is defined as a mismatch between the patient's intrinsic timing of inspiratory and expiratory efforts and the ventilator's insufflation and exhalation, reflecting a discrepancy between the patient's needs and the support delivered by the ventilator. Although asynchrony is frequently observed in mechanically ventilated patients, it remains unclear whether patient-ventilator asynchrony directly contributes to adverse outcomes, under what conditions some degree of asynchrony may be tolerated or beneficial, and how this knowledge should guide clinical management. Patient-Ventilator Synchrony.

Patient-ventilator asynchrony develops when the ventilator output does not match the efforts of the patient and contributes to excess work of breathing, lung injury, and mortality. Asynchronies are categorized as trigger (breath initiation), flow (delivery of the breath), and cycle (transition from inspiration to expiration). Clinicians should be skilled at ventilator waveform analysis to detect patient-ventilator asynchronies and make informed ventilator adjustments. Ventilator overdrive suppresses respiratory drive and reduces asynchrony, while other adjustments specific to the asynchrony ar

Correction of Patient-Ventilator Discordance.

Optimizing the interaction between the patient and the ventilator should be a cornerstone for clinicians to pursue. Disruptions in these interactions can lead to what is called patient-ventilator discordance. Patient-ventilator discordance is a mismatch between what the patient desires in a breath and what the ventilator provides. Asynchrony is also a common term used to describe these unhealthy interactions. Different types of asynchronies make up the more global term of patient-ventilator discordance. Asynchronies can occur at the onset of a breath (trigger asynchrony), during breath deliver

Patient-ventilator Dyssynchrony: Detection, Importance, and Management.

Patient-ventilator dyssynchrony (PVD) is frequent during assisted mechanical ventilation and reflects a mismatch between the patient's respiratory demands and ventilatory assistance. PVD can be classified according to their predominant mechanisms into high respiratory drive/underassistance, low respiratory drive/overassistance, and reverse triggering, the last emerging during the transition to lighter sedation. Their clinical impact is influenced by their physiologic consequences, particularly the effort magnitude and the presence of breath stacking. Although PVD has been associated with worse
