Managing the Operating Room Course for the Complex Pain Patient.

Patients with chronic pain require special attention when formulating the perioperative anesthetic plan. Understanding of the patient's preoperative medication regimen, physical and psychological limitations, type and location of pain, and presence of implanted analgesic devices is necessary to optimize the recovery process and minimize iatrogenic injuries. In general, it is recommended to continue the preoperative analgesic plan if not contraindicated and consider regional anesthetic techniques, multimodal analgesic adjuncts, and protocolized postoperative care plans (such as Enhanced Recover

On Whether the Sickest Patient in the Emergency Department Is in the Waiting Room.

One common maxim in clinical medicine is that the sickest patient in the emergency department may be in the waiting room (termed the waiting-room maxim). The purpose of this commentary is to discuss 5 pitfalls related to the waiting-room maxim that can sometimes transform a fundamentally smart idea into a popular misconception. Brevity makes a maxim powerful, yet also creates limitations by leaving out elaboration, logic, and evidence.

[Epidemiology of patients presenting to the emergency room for dyspnea in Belgium].

Studies about patients' profile presenting to the emergency department for dyspnea are scarce in Europe, and even more in Belgium. We analyze here a cohort of patients with acute dyspnea in three Belgian centers, as to the epidemiology of this dyspnea, the tests carried out, the diagnoses retained, the treatments administered, the hospitalization and survival rates. The secondary objective is to compare this Belgian cohort with a European population resulting from a large multicenter study. The analysis of 131 patients showed that acute dyspnea in the emergency department corresponds to four m

Characteristics of Hypoglycemic Diabetic Patients Visiting the Emergency Room.

INTRODUCTION: Severe hypoglycemia can be life-threatening; therefore, it is important to identify the characteristics of the hypoglycemic patients. The aim of this study is to analyze the type and characteristics of diabetic patients with hypoglycemia who visited an emergency room. METHODS: We included diabetic patients with hypoglycemia who visited the emergency room of St. Mary's Hospital in Seoul from January 2009 to August 2018 in the study. Hypo_S group patients visited the emergency room once whereas Hypo_M group patients visited twice or more. We also compared the incidence of cardiova

A profile of acute asthma patients presenting to the emergency room.

We conducted a three-month prospective study on the profile of 70 acute adult asthmatic patients presenting to the Accident and Emergency Unit of a general hospital. The overall background asthmatic activity, clinical features of current exacerbation, maintenance drug treatment, gross psycho-social problems, previous experience of near-fatal asthma, and admission and relapse rates were documented. Ninety percent of the presentations were within 24 hours of the initial attack, with an average pre-hospital therapy duration of 6.5 hours. Sixty-seven percent sought further treatment after failure

Point-of-care ultrasound for stroke patients in the emergency room.

Stroke requires rapid determination of the cause to provide timely and appropriate initial management. Various ultrasonographic techniques have been evaluated as ways to determine the cause of stroke; among them, carotid artery ultrasonography is particularly useful since it provides considerable information within a short time period when used to evaluate a specific site. In the emergency room, carotid artery ultrasonography can be used to diagnose internal carotid artery stenosis, predict an occluded vessel, and infer the cause of ischemic stroke. Additionally, carotid artery ultrasonography

Special Considerations for the Septic Patient Going to the Operating Room.

Perioperative care of the septic patient presents unique challenges for the nurse. Understanding the physiology of sepsis is important for the nurse to anticipate the physiologic changes associated with the disorder and to prioritize the patient's needs. Ensuring adequate intravenous access by peripheral or central venous access as well as anticipating potential invasive monitoring lines allows the nurse to quickly administer antibiotic therapy, initiate fluid resuscitation, and prepare for potential vasopressor use.

Patient safety in the operating room: II. Intraoperative and postoperative.

BACKGROUND: The perioperative environment can be hazardous to patients and providers alike. Although many risks are best addressed preoperatively, some hazards require constant attention by the surgeon, anesthesiologist, and staff in the operating room. In a previous article, the authors discussed preoperative aspects of patient safety. In this article, the authors review intraoperative and postoperative risks and techniques to decrease these risks. METHODS: The authors reviewed the literature regarding operating room safety, both primary research and secondary reviews, via multiple PubMed qu

[Epidemiology and management in emergency room patients with maxillofacial fractures].

OBJECTIVE: To minimize overall mortality and optimise reconstructive and cosmetic outcome in severely injured patients with maxillofacial injuries the interdisciplinary coordination of several surgical disciplines is required. It is still discussed controversy whether patients with maxillofacial fractures benefit from early fracture repair or if delayed operative management also yields in good results. METHODS: Herein we analysed the data of 1252 severely injured patients between May 1998 through June 2002 in our trauma department regarding fractures of the maxillofacial region, injury severi

Stroke in young patients: a diagnostic challenge in the emergency room.

Stroke in young adults has been related to mechanisms different to those found in older individuals. Cardiogenic embolism, arteritis, atherosclerosis, fibromuscular dysplasia, pregnancy-related angiopathy, migrainous stroke, anaemia, antiphospholipid syndrome, arterial dissection, the consumption of toxic substances and head trauma have been described. We present a young man with a case history of tobacco and cocaine abuse who suffered a mild head trauma, with normal neurological examination, and a computed tomography scan image of a right anterior cerebral infarction. Brief treatment of emergency room patients with panic attacks.

OBJECTIVE: Most research on treatment for panic disorder has involved chronic forms of the illness. To determine the efficacy of early intervention, the authors examined the effects of treatment for patients with panic attacks who were seen in the emergency room, which is the first point of contact with the health delivery system for many persons with panic attacks. METHOD: The subjects were 33 patients with panic attacks seen in two emergency rooms. The presence of panic attacks was confirmed with a modified version of the Structured Clinical Interview for DSM-III-R; approximately 40% of the

Dual diagnosis patients in the urban psychiatric emergency room.

Substance abuse among the mentally ill has become increasingly prominent. This article describes the problems presented by dual diagnosis patients in the urban, publicly funded, psychiatric emergency room. When such patients become acutely ill, neither the public health system nor the mental health clinician is adequately prepared to provide care. The inability to effectively treat these patients has become strikingly clear in recent years. In 1986, the Psychiatric Emergency Service at San Francisco General Hospital began to overflow with patients who required overnight stays. The intoxicated

[Suspected acute coronary events in emergency room patients].

Identification of patients in whom chest pain is due to an acute coronary event is among the most challenging problems in emergency medicine. Because of the dangers of missing the diagnosis in patients with a possibly fatal condition, emergency room (ER) physicians often admit patients for 'observation' or 'to rule out MI.' As a result of such a policy, only 30-50% of such patients admitted to the CCU may finally be diagnosed as having an acute myocardial infarction (AMI), resulting in deleterious medical, psychological and economic consequences for the others. A series of 2280 patients who we

Patients who bring weapons to the psychiatric emergency room.

The personal characteristics of patients who brought weapons to a university hospital-based psychiatric emergency room were examined. During 14 months following implementation of a routine weapon-screening procedure, 37 (4%) of 1012 psychiatric emergency room patients were found to have weapons. These patients did not differ significantly from a randomly selected control group of non-weapon-carrying patients on a variety of demographic and clinical variables often associated with violence potential, although the weapon-carrying patients were more likely to be male and have a history of substan

An emergency room's patients: their characteristics and utilization of hospital services.

Utilization of the emergency room at an urban community hospital is studied in a format designed to accomplish three complementary objectives: 1) to characterize a sample of individual patients, rather than an unweighted sample of visits, 2) to estimate the number of individuals served during a specified period and the magnitude of the relationship between these patients and the utilization of other hospital services, and 3) to introduce the patient's "frequency-of-visit" as an important variable in the analysis of emergency room utilization. Some specific findings are: 1) the vast majority of
