The development of the obligatory education and certification programme of the Royal Australian College of Obstetricians and Gynaecologists: a practical response to the increasing challenges of a modern society.

A career in medicine should involve a commitment to a lifetime of learning. Rapid advances in medical knowledge, as well as the increasing knowledge and sophistication of consumers, make this commitment more important than ever before. The establishment of a new Specialist College in Australia in 1979 provided the opportunity to frame Articles which made this commitment obligatory for those who wished to continue as Fellows in the new College. The implementation of this philosophy called for a wide ranging involvement of all obstetricians joining the College, to ensure that the final form of t

The impact of Recovery Colleges on mental health staff, services and society.

AIMS: Recovery Colleges are opening internationally. The evaluation focus has been on outcomes for Recovery College students who use mental health services. However, benefits may also arise for: staff who attend or co-deliver courses; the mental health and social care service hosting the Recovery College; and wider society. A theory-based change model characterising how Recovery Colleges impact at these higher levels is needed for formal evaluation of their impact, and to inform future Recovery College development. The aim of this study was to develop a stratified theory identifying candidate

Improving urinary catheterisation practices in a rural hospital in Ontario.

INTRODUCTION: A urinary catheter constitutes a one-point patient restraint, can induce deconditioning and may lead to patient mortality. An audit performed at Winchester District Memorial Hospital revealed that 20% of patients had a urinary catheter, of whom 31% did not meet the criteria for catheterisation. The main objective of this study was to use the Influencer Change Model and the Choosing Wisely Canada toolkit to create a bundle of interventions that would reduce the unnecessary use of urinary catheters in hospitalised patients. METHODS: In a rural teaching hospital, a time-series quas

When disaster strikes: death of a living organ donor.

Donor safety is of paramount importance in living donor transplantation. Yet, living donor deaths occur. We believe that problems exist in our system of live donor transplantation that can be summarized in a series of simple statements: (1) Donor mortality can never be completely eliminated; (2) Live donor risk has not been mitigated so that it is as low as possible; (3) After a donor death, systematic reviews are not routinely performed to identify correctable causes; (4) The lessons learned from any donor death are not adequately communicated to other programs and (5) The administrative mech

Parents' Active Role and ENgagement in The review of their Stillbirth/perinatal death 2 (PARENTS 2) study: a mixed-methods study of implementation.

OBJECTIVE: When a formal review of care takes places after the death of a baby, parents are largely unaware it takes place and are often not meaningfully involved in the review process. Parent engagement in the process is likely to be essential for a successful review and to improve patient safety. This study aimed to evaluate an intervention process of parental engagement in perinatal mortality review (PNMR) and to identify barriers and facilitators to its implementation. DESIGN: Mixed-methods study of parents' engagement in PNMR. SETTING: Single tertiary maternity unit in the UK. PARTICIP

Quality of care for cardiovascular disease-related conditions in patients with and without mental disorders.

OBJECTIVE: We compared the quality of care for cardiovascular disease (CVD)-related risk factors for patients diagnosed with and without mental disorders. METHODS: We identified all patients included in the fiscal year 2005 (FY05) VA External Peer Review Program's (EPRP) national random sample of chart reviews for assessing quality of care for CVD-related conditions. Using the VA's National Psychosis Registry and the National Registry for Depression, we assessed whether patients had received diagnoses of serious mental illness (schizophrenia, bipolar disorder, or other psychoses) or depressio

Assessment of Healthy Start Fetal and Infant Mortality Review recommendations.

OBJECTIVES: We examine the scope and nature of the recommendations that emerged from the Healthy Start Fetal and Infant Mortality Review (FIMR) projects and explore their use to promote systems change. METHODS: The FIMR process of 16 of the 22 federal Healthy Start projects was reviewed. We analyzed data from a June 1996 survey developed and administered by the MCH Bureau which gathered information about recommendations produced by the FIMRs. We supplemented these data with information gathered through follow-up telephone interviews and by abstracting information from grant documents. RESULT

Exploring new avenues to assess the sharp end of patient safety: an analysis of nationally aggregated peer review data.

BACKGROUND: Many healthcare organisations (HCOs) use peer review to evaluate clinical performance, but it is unclear whether these data provide useful insights for assessing the sharp end of patient safety. OBJECTIVE: To describe outcomes of peer review within the Department of Veterans Affairs (VA) healthcare system and identify opportunities to leverage peer review data for measurement and improvement of safety. DESIGN: We partnered with the VA's Risk Management Program Office to perform descriptive analyses of aggregated peer review data collected from 135 VA facilities between October 20

Quality assurance for health supervision of the ambulatory child.

The Joint Committee on Quality Assurance (JCQA) was established in 1970 by the American Academy of Pediatrics and includes members from eight organizations concerned with primary care for children. The Committee developed criteria for assessing (1) health supervision of children, and (2) the management of three common diseases of children: tonsillopharyngitis, urinary tract infection, and bronchial asthma. These criteria were then validated by 452 academic and practitioner experts. A representative sample of primary care physicians further evaluated these criteria. The last phase of the study

Developing a Typology for Peer Education and Peer Support Delivered by Prisoners.

Peer interventions delivered for prisoners by prisoners offer a means to improve health and reduce risk factors for this population. The variety of peer programs poses challenges for synthesizing evidence. This article presents a typology developed as part of a systematic review of peer interventions in prison settings. Peer interventions are grouped into four modes: peer education, peer support, peer mentoring, and bridging roles, with the addition of a number of specific interventions identified through the review process. The article discusses the different modes of peer delivery with refer

Expert review of an approach to functional capacity evaluation.

Functional capacity evaluation (FCE) is a widely used tool in work rehabilitation, despite the limited examination of the soundness of its measurement properties. This paper outlines the development of a new approach to FCE, the GAPP FCE, and reports on the findings of an expert review of aspects of its content validity and technical adequacy and how it meets established test criteria. Five expert occupational therapists reviewed the materials of the GAPP FCE then completed a questionnaire related to the content validity, technical adequacy and safety, reliability, validity, practicality and u

[The development of clinical guidelines for the diagnosis and treatment of chronic periodontitis in Belgium].

In many disciplines of medicine guidelines are developed for the diagnosis and treatment of disease. These are essentially intended to standardize care and to optimize communication between the general practitioner and the specialist. Guidelines have already been described in the literature for chronic periodontitis. However, given the unique conditions in Belgium, these may not be appropriate for the average dental practice. In this manuscript the development of Belgian clinical guidelines for the diagnosis and treatment of chronic periodontitis is described. Basically, ten clinical questions

The impact of legislatively imposed practice guidelines on cesarean section rates: the Florida experience.

Florida legislation implemented in the fall of 1992, unique in the nation, mandated that practice guidelines regarding cesarean section deliveries be disseminated to obstetric physicians. The law also required that peer review boards at hospitals be established to review cesarean deliveries and that the exact dates of implementation of the guidelines be reported to a state agency. To determine the impact of the legislation, we conducted a retrospective analysis of 366,246 total live births occurring in Florida hospitals during 1992 and 1993, before and after formal hospital certification of th

Advanced Practitioners Are Peers in Trauma Performance Improvement Peer Review.

Advanced practitioners (APs) have been successfully integrated into the clinical care of injured patients. Given the expanding role of APs in trauma care, we hypothesized that APs can perform Performance Improvement and Patient Safety (PIPS) peer review at a level comparable with trauma surgeons. For Phase 1, cases previously reviewed by a trauma surgeon were randomly selected by the PIPS coordinator and peer reviewed by an AP. The trauma surgeons' and APs' reviews were compared. For Phase 2, cases requiring concurrent review were peer reviewed by both an AP and an MD, who were blinded to each
