Levator plate descent angle in pelvic floor disorders.

BACKGROUND: The levator plate descent angle (LPDA) quantifies the levator plate position with reference to the pubic bone and perineal body at rest. Unfortunately, research on this notable new parameter is lacking, but it is clear that levator ani deficiency (LAD) will undermine the fundamental role of the levator ani muscle (LAM) in organ support. The aim of this study was to establish the relationship between the LPDA and LAD in patients with pelvic floor disorders. METHODS: This retrospective study was conducted at Seoul Songdo Hospital, Korea between August 2019 and August 2020 on women w

Joint hypermobility, obstetrical outcomes, and pelvic floor disorders.

INTRODUCTION AND HYPOTHESIS: Benign joint hypermobility syndrome may be a risk factor for pelvic floor disorders. It is unknown whether hypermobility impacts the progress of childbirth, a known risk factor for pelvic floor disorders. Our objective was to investigate the association between joint hypermobility syndrome, obstetrical outcomes, and pelvic floor disorders. Our hypotheses were: (1) women with joint hypermobility are less likely to experience operative delivery and prolonged second-stage labor; and (2) pelvic floor disorders are associated with benign hypermobility syndrome, controll

Management of bladder, prostatic and pelvic floor disorders.

Since its introduction in the late 1970s for the treatment of strabismus and blepharospasm, botulinum toxin (BoNT) has been increasingly used in the interventional treatment of several other disorders characterized by excessive or inappropriate muscle contractions. Over the years, the number of primary clinical publications has grown exponentially, and still continues to increase. It has been shown that BoNT blocks cholinergic nerve endings in the autonomic nervous system but does not block non-adrenergic non-cholinergic responses mediated by nitric oxide (NO). The present paper reviews a numb

Pudendal reflexes in women with pelvic floor disorders.

The objective of this work was to study two pudendal anal reflexes: Deep Pudendal Reflex (DPR) and classical Bulbocavernosus Reflex (BCR) in women with primary and recurrent genital prolapse to obtain support to the hypothesis of pelvic nerve damage in patients with pelvic floor disorders. 124 women were studied: 68 were normal; 38 with genital prolapse (GP); and 18 with recurrent GP. Clinical and electrophysiological studies were carried out. Delayed reflex responses were found in 44/56 of patients (79%). [27/38 in genital prolapse group (71%); 17/18 in recurrent GP group (94%)]. Unifying concept of pelvic floor disorders and incontinence.

Denervation of pelvic floor sphincter muscles is a feature of pelvic floor disorders. When severe, it may be accompanied by stress incontinence of faeces, or of urine. The extent of chronic partial denervation of these pelvic floor muscles can be quantified by electromyography (EMG), and its cause identified by electrophysiological studies of the motor innervation of these striated muscles. Damage to this innervation is often initiated by childbirth, but appears to progress during a period of many years so that the functional disorder usually presents in middle life. Incontinence develops in s

[Assessing knowledge of pelvic floor disorder and pelvic floor muscle training among young women].

Introduction: Although pelvic floor disorders affect millions of women worldwide, healthcare seeking is low. Adequate knowledge regarding pelvic floor disorders could lead to increased healthcare-seeking behavior, which may help to reduce the economic burden of these disorders. Objective: The present study aims to evaluate knowledge about pelvic floor disorders and pelvic floor muscle training and to assess healthcare seeking behavior among young women. Method: 221 young women (mean age: 20.65 ± 1.67 years) were included in the present cross-sectional study. The Prolapse and Incontinence Knowl

Beyond the Binary: Pelvic Pain and Pelvic Floor Disorders in Transmasculine Patients.

IMPORTANCE: Transmasculine and gender-nonconforming (TMGNC) people who were assigned female at birth also experience pelvic floor disorders (PFDs) and chronic pelvic pain (CPP). This group of patients interacts with the health care system uniquely and has different barriers to care compared with cisgender women. OBJECTIVES: The objectives of this study were to (1) qualitatively analyze how TMGNC individuals experience PFDs and CPP and (2) to explore participants' interaction with the health care system while obtaining PFD- and CPP-related treatment. Progress in the genetics and epigenetics of pelvic floor disorder.

Pelvic floor disorder (PFD) is a common gynecological disorder, and with the ageing of the population, PFD has a serious impact on the physical and mental health of patients and their quality of life. The most prominent of these are pelvic organ prolapse (POP) and urinary incontinence (UI), about which the etiology is still unclear, and it is urgent to explore their pathogenesis. Advances in genetics and epigenetics have provided new insights into the pathophysiology of PFD. Candidate genes and genome-wide association studies have identified susceptibility genes for POP and UI. These susceptib

The Role of MRI in the Diagnosis of Pelvic Floor Disorders.

PURPOSE OF REVIEW: This article provides an overview of MRI as a dynamic imaging modality as well as recent updates on the role of MRI in the evaluation of female pelvic floor disorders, including pelvic organ prolapse (POP), rectal prolapse, and defecatory dysfunction. RECENT FINDINGS: Pelvic floor disorders in women are common, and demand for treatment is high. Recently, multidisciplinary guidelines for dynamic MRI have been published to reflect this need. Despite criticisms that MRI is an expensive modality, cost-analysis studies are demonstrating utility for MRI in surgical decision-makin

Influence of pelvic floor disorders on sexuality in women.

OBJECTIVE: To determine the association between different pelvic floor disorders and the presence of sexual dysfunction in women. METHOD: An observational study of non-pregnant women was carried out in Spain in 2021 and 2022. To assess the presence of pelvic floor problems, the Pelvic Floor Distress Inventory (PFDI-20) was used, consisting of the subscales Pelvic Organ Prolapse Distress Inventory-6 (POPDI-6; prolapse symptoms), Colorectal-Anal Distress Inventory (CRADI-8; colorectal symptoms), and Urinary Distress Inventory-6 (UDI-6; urinary symptoms). Depression and anxiety in patients with pelvic floor disorders.

Pelvic floor disorders are very common health problems in adult women affecting their quality of life in many aspects. One of them, still poorly recognised, is depression as well as anxiety. As the main goal of treatment is achievement of improvement of life quality we have to be aware of the incidence and severity of mood disorders in urogynecological patients. It is very important to be sure whether treatment of main disease is enough to solve depression and anxiety or we have to cope with them separately. The review sums up current knowledge on that very important topic.

Is Bariatric Surgery a Prophylaxis for Pelvic Floor Disorders?

INTRODUCTION: Obesity is one of the well-documented risk factors of pelvic floor disorders (PFDs). The PFDs include urinary and fecal incontinence (UI, FI) and pelvic organ prolapse (POP). Surgery-induced weight loss improves different kinds of incontinence as well as POP symptoms. However, there is a lack of evidence how bariatric surgery influences pelvic floor anatomy and function in women without previous PFDs and whether it may be concerned as PFD prophylaxis tool. MATERIALS AND METHODS: The present analysis is a prospective, non-randomized case-control study from January 2014 to Septemb

Breaking news in the prediction of pelvic floor disorders.

Urinary incontinence (UI) and fecal incontinence (FI) together with pelvic organ prolapse (POP) constitute a huge global health problem affecting millions of women throughout the world. These pelvic floor disorders (PFDs) can have a negative influence on a woman's well-being, quality of life, and sexual function and prevent many women from participating in recreational and sporting activities. The global costs of PFDs to health care systems and society are enormous and approximately one in five women will undergo surgery for genital prolapse or UI by the age of 85 years.

Laparoscopic surgery for rectal prolapse and pelvic floor disorders.

Pelvic floor disorders are different dysfunctions of gynaecological, urinary or anorectal organs, which can present as incontinence, outlet-obstruction and organ prolapse or as a combination of these symptoms. Pelvic floor disorders affect a substantial amount of people, predominantly women. Transabdominal procedures play a major role in the treatment of these disorders. With the development of new techniques established open procedures are now increasingly performed laparoscopically. Operation techniques consist of various rectopexies with suture, staples or meshes eventually combined with si

Sexual abuse history and pelvic floor disorders in women.

OBJECTIVES: Sexual abuse rates in the general female population range between 15% and 25%, and sexual abuse is known to have a long-term impact on a woman's health. The aim of this study was to report the prevalence of sexual abuse history in women presenting to clinicians for pelvic floor disorders (PFD) and to determine whether a history of sexual abuse is associated with a specific type of PFD. METHODS: We conducted a retrospective chart review of new urogynecology patients seen at the University of New Mexico Hospital. All women underwent a standardized history and physical examination an
