A narcoleptic patient exhibiting hallucinations and delusion.

A female narcoleptic patient with persistent auditory hallucinations and delusion is reported. These psychotic symptoms appeared before the diagnosis of narcolepsy and before the use of pemoline and clomipramine. On admission to the Department of Neuropsychiatry at the Kurume University school of Medicine, the patient's auditory hallucinations were almost the same as the previous hallucinations. The patient's delusional state improved smoothly after haloperidol administration. After that, the patient related well socially. The persistent auditory hallucinations were correlated with rapid eye m

Clinical trials in multiple sclerosis.

In this past year, there has only been modest progress in the search for an effective treatment for multiple sclerosis and its complications, although a number of carefully designed trials are in progress. No treatment predictably slows the course of active disease. The marginal benefits previously claimed for azathioprine have been strengthened by a meta-analysis of previously published work. Methylprednisolone may have a minor role in the treatment of very severe, acute optic neuritis but prednisone use may predispose patients to recurrent optic neuritis. 4-Aminopyridine and 3,4-diaminopyrid

[Narcolepsy].

Narcolepsy is a disorder of sleep with uncontrolled sleepiness and cataplexy. Its prevalence in Japanese is estimated to be 0.16-0.59%. The handicap of narcoleptic patients is even larger than that of epileptic patients. The etiology is still unknown, however, a tight association between specific HLA (DR15 and QQ6) or its genotypes (DRB1*1501, DRB5*0101, DQB1*0602, DQA1*0102) and narcolepsy was recently established. This finding raises the possibility that narcolepsy might be an immunological disorder. At present, any concrete evidence supporting this possibility is lacking. Methylphenidate an

Drug therapy in the treatment of minimal brain dysfunction.

The pharmacotherapy of minimal brain dysfunction (MBD) is reviewed. Studies using central nervous system (CNS) stimulants (amphetamines and methylphenidate, deanol, pemoline, caffeine), antidepressants (imipramine and desipramine), anticonvulsants (phenytoin and primidone), antianxiety agents (chlordiazepoxide, hydroxyzine, meprobamate), antipsychotic agents (phenothiazines, thioxanthenes, butyrophenones) and miscellaneous agents (benztropine, thyrotropin-releasing hormone, megavitamins) are discussed. When drugs are indicated, the CNS stimulants are the agents of choice in the treatment of MB

Dramatic favorable responses of children with learning disabilities or dyslexia and attention deficit disorder to antimotion sickness medications: four case reports.

Responses of four learning disabled children who showed dramatic improvements to one or more antimotion-sickness-antihistamines and -stimulants are described qualitatively. These cases were selected from a prior quantitative study in which three antihistamines (meclizine, cyclizine, dimenhydrinate) and three stimulants (pemoline, methylphenidate, dextroamphetamine) were tested in variable combinations (using a specific clinical method) for favorable responses by 100 children characterized by diagnostic evidence of learning disabilities and cerebellar-vestibular dysfunctioning. Pharmacologic management of daytime sleepiness.

Excessive daytime sleepiness and abnormal sleep-wake patterns are becoming increasingly pervasive in modern society. The major causes of excessive daytime sleepiness include pathologic abnormalities of the central nervous system, such as narcolepsy and idiopathic hypersomnia; deficiencies in quality or quantity of sleep, such as those caused by sleep apnea and poor sleep hygiene; disturbances to the body's natural circadian rhythm, such as those caused by shift work or jet lag; and drugs, which can increase sleepiness either therapeutically or as a side effect. Determining the cause of daytime

Enhancing medical image segmentation with a multi-transformer U-Net.

BACKGROUND: The PARP inhibitor olaparib was recently granted Food and Drug Administration (FDA) accelerated approval in patients with advanced BRCA1/2 mutation ovarian cancer. However, antitumor responses are observed in only approximately 40% of patients and the impact of baseline clinical factors on response to treatment remains unclear. Although platinum sensitivity has been suggested as a marker of response to PARP inhibitors, patients with platinum-resistant disease still respond to olaparib. RESULTS: 108 patients with advanced BRCA1/2 mutation ovarian cancers were included. The interval

CNS stimulant potentiation of monoamine oxidase inhibitors in treatment-refractory depression.

We report on our clinical experience with a combination of a CNS stimulant (either pemoline or dextroamphetamine) and a monoamine oxidase inhibitor (MAOI) for treating 32 depressed patients (mainly outpatients) refractory to standard antidepressant pharmacotherapy. This combination, though not approved by the FDA, appears to be safe and effective. Twenty-five (78%) of these patients experienced at least 6 months of symptom remission with a stimulant + MAOI combination. Many patients required adjunctive antidepressant treatment, including tricyclics and lithium. Side effects were not excessive,

Psychostimulants in psychiatry.

The use of the psychostimulants in psychiatry is reviewed. A brief historical perspective on dextroamphetamine is presented, and a brief review of the psychopharmacology of dextroamphetamine, methylphenidate and magnesium pemoline is given. The literature on the use of stimulants in the treatment of resistant depression, apathetic geriatric patients and patients medically ill with a secondary depression is summarized and two case histories given to illustrate the clinical usefulness of the stimulants. The literature on the use of stimulants as an adjunct to antidepressant therapy and as a diag

[Hyperkinetic disorders with attention deficit. Diagnostic and therapeutic approach].

Attention deficit disorder (ADD) is the most common neurobehavioral disorder in the pediatric age group. The development of diagnostic criteria based on the "diagnostic and statistical manual of Mental Disorders" represents an advance in the reliable and valid diagnosis of ADD. An examination of neuromaturational signs (soft signs) and neuropsychologic and psychometric tests to specify the behavioral functions and the origin of learning disabilities are very important. ADD must be differentiated from other conditions such as psychiatric, developmental, neurological, psychosocial and environmen

Modafinil for the treatment of diminished responsiveness in a patient recovering from brain surgery.

PRIMARY OBJECTIVE: This case report reviews the history and post-operative status of a 77-year-old woman who underwent transcallosal resection of an intra-ventricular haemorrhagic subependymoma and who remained in a deep post-operative state of lethargy and listlessness. MAIN OUTCOME MEASURES: Glasgow Coma Scale (GCS) and detailed clinical observations. RESULTS: The patient (GCS=8) was initiated on methylphenidate 20 mg per day on the 6th day after surgery, but treatment was discontinued 2 days later due to elevated blood pressure and persistent tachycardia. Pemoline 37.5 mg per day was init

Excessive daytime sleepiness and sleep disturbances in patients with neurological diseases: epidemiology and management.

Up to 12% of the general population experience excessive daytime sleepiness (EDS), with increasing prevalence in the elderly. EDS may lead to cognitive impairment, resulting in inattentiveness, poor memory, mood disorders and an increased risk of accidents. As a result, quality of life is reduced in most patients with EDS as well as in their caregiving spouses. There are a variety of causes leading to EDS, including CNS pathology, neurological dysfunction, associated sleep disorders with insufficient or fragmented sleep, and drug therapy. Since EDS accompanies many neurological disorders, such

Pediatricians' reported practices in the assessment and treatment of attention deficit disorders.

This study examined, in a national survey, the assessment and treatment practices of pediatricians who care for children with attention deficit disorders (ADD). A questionnaire was sent to randomly selected members of the American Academy of Pediatrics (AAP), stratified by state. Of 417 (52%) responses returned, there were 290 (38%) completed questionnaires. It was found that most pediatricians did not utilize specific DSM-III criteria for ADD; however, they do rely on symptoms of distractibility, overactivity, and impulsivity, which are the behaviors central to the DSM-III diagnosis. In addit

[Hyperkinetic syndrome (attention deficit-/hyperactivity disorder) in adulthood].

The clinical picture of adult hyperkinetic syndrome (HKS) or attention deficit/hyperactivity disorder is nearly unknown in Germany. It can be estimated, that approximately one third of affected children also show symptoms as adults. In the combined type of the syndrome symptoms of inattention as well as of hyperactivity and impulsivity are present, a predominantly inattentive or hyperactive-impulsive type is possible. Retrospective diagnosis of HKS in childhood can be difficult. Disorganization, emotional disturbances and stress intolerance are common in adults with HKS as well as residual sym

Narcolepsy.

Narcolepsy is a commonly under diagnosed or misdiagnosed problem that results in severe daytime sleepiness. There is a strong genetic component with some immune system involvement. It may occur in combination with other sleep disorders such as sleep apnea complicating its treatment. An objective diagnosis requires a polysomnogram and Multiple Sleep Latency Test. Management depends on the careful use of stimulant medication to control the sleepiness and other medications to control the auxiiliary symptom of cataplexy.
