Reoperation for calcific constrictive pericarditis 43 years after pericardiectomy.

The case of calcific pericarditis in a 63-year-old male with a 1-year history of progressive right heart failure 43 years after pericardiectomy in 1960 is reported. The recurrent constriction was caused by the calcification of the residual posteroinferior pericardium and the impressing sternum of a pectus excavatum encasing the heart like a skull. The patient underwent total pericardiectomy of his residual pericardium through median sternotomy using cardiopulmonary bypass resulting in complete relief of his symptoms.

[Pleuro-pneumo-phreno-pericardiectomy with pericardial and diaphragmatic plasty].

Aim of the paper is to evaluate 43 extrapleural pneumonectomy performed from 1988 to May 2000. Criteria for extrapleural pneumonectomy were pleural biopsy by thoracoscopy, potentially completely resectable unilateral disease by computed tomography and predicted postresection forced expiratory volume >1,3 L/sec. The resections regarded 33 pleural mesothelioma, 9 pleural lung-carcinosis and 1 pleural melanoma effusion. The perioperative mortality rate was 2,2% (1 death) and morbidity 21,4%.

Pericardiectomy in post-traumatic suppurative constrictive pericarditis: case report.

A 13-year-old male was seen at the Hospital with a 5-months history of right chest swelling, pain and recurrent fever and breathlessness on mild exertion. There was a history of gunshot to his chest two and half years before presentation. On admission he was febrile with a temperature of 39 degrees C. The chest wall swelling measuring 6 cm/6 cm was tender and fluctuant and needle aspiration yielded purulent fluids. His blood pressure and pulse were 110/60 mmHg (14.6/8 Kpa) and 100 per minute respectively. The chest radiograph showed massive cardiomegaly with pellets lodging in the right 5(th )c

[Long-term late results of pericardiectomy for constrictive pericarditis].

Twenty-four cases of the constrictive pericarditis were operated on pericardiectomy from 1966 to 1990. One case died because of massive bleeding, LOS, and respiratory failure immediately after the operation. A long-term follow up study was performed up to 27 years (mean 13.1 years) in other cases. Ten cases died during the follow up period (cardiac: 6, non cardiac: 4). Five cases of the cardiac deaths had major complications: tabes dorsalis, chronic respiratory failure, pacemaker implantation, mitral regurgitation, emergency operation at high age (78 years old). The survival rates of 5, 10, an

[Cardiac tamponade and rheumatoid arthritis: medical approach or pericardiectomy?].

The authors report the case of a sixty-seven-year-old man with seronegative rheumatoid arthritis since 1967. After the treatment was discontinued, a symptomatic pericardial effusion developed during an exacerbation of rheumatoid arthritis. Histological findings suggested a rheumatoid origin. Consecutive pericardiocentesis and a concomitant adequate treatment resolved cardiac tamponade, at least during short-term follow-up. However, a long term observation will be necessary to exclude recurrent effusion or evolutive constrictive pericarditis.

Radical Pericardiectomy and Use of Cardiopulmonary Bypass for Constrictive Pericarditis.

BACKGROUND: Pericardiectomy is a definitive treatment for constrictive pericarditis. However, the extent of resection and cardiopulmonary bypass (CPB) use remain debated. We therefore compared hemodynamic changes, complications, long-term survival, and use of CPB in propensity-matched patients after radical vs partial pericardiectomy. METHODS: From 2000 through 2022, 534 consecutive adults with constrictive pericarditis underwent pericardiectomy, comprising radical in 425 (345 [81%] on CPB) and partial in 109 (68 [62%] on CPB) at Cleveland Clinic. Cardiac index, central venous pressure, proce

Pericardiectomy using an ultrasonic scalpel in patients with constrictive pericarditis.

OBJECTIVES: Pericardiectomy using an ultrasonic scalpel is the effective operative treatment of chronic pericarditis. However, no consecutive case series of radical pericardiectomy using an ultrasonic scalpel has been reported. METHODS: We retrospectively reviewed the records of 10 patients who underwent a radical pericardiectomy with an ultrasonic scalpel between April 2015 and March 2024. Preoperative and postoperative variables were reviewed. RESULTS: The median age was 65 years, and 3 patients were female. Three patients had preoperative atrial fibrillation. Forty percent of patients wer

Pericardiectomy for Constrictive and Recurrent Pericarditis: State of the Art Update.

PURPOSE OF REVIEW: This review provides a contemporary surgical view on constrictive and recurrent pericarditis. RECENT FINDINGS: Cardiac magnetic resonance imaging, echocardiography, and heart catheterization are complementary tools for diagnosis and management of patients considered for pericardiectomy. Radical pericardiectomy, unlike total/complete, represents the most extensive resection. Radical or total/complete pericardiectomy via median sternotomy is preferred over partial for both recurrent and constrictive pericarditis, to minimize the risk of symptoms and/or constriction recurrence

Constrictive pericarditis in a mare: attempted treatment by partial pericardiectomy.

Chronic constrictive pericarditis was diagnosed in a 6-year-old Thoroughbred mare based on the clinical findings of right congestive heart failure, hyperechoic pericardium without pericardial effusion, and a dip-and-plateau shape of the right ventricular pressure curve with equilibration of the diastolic pressures in all cardiac chambers. Treatment was attempted by partial pericardiectomy using a right lateral thoracotomy approach. Because of severe epicardial involvement recurrence of the constrictive pathology was noted 6 weeks after the surgical procedure. However, in selected cases in whic

Partial pericardiectomy for refractory acute tuberculous pericarditis: A case report.

INTRODUCTION: Tuberculosis is an infectious disease that usually manifests in the lungs but can also affect other organs, including the cardiovascular system. In this article, we present a rare case of purulent pericarditis caused by Mycobacterium tuberculosis. PRESENTATION OF CASE: A 67-year-old man was admitted to the emergency department with a large pericardial effusion with evidence of cardiac tamponade caused by acute pericarditis. The patient underwent surgical pericardial drainage, and a total volume of 500 mL of purulent fluid was collected with a positive culture for Mycobacterium t

Chronic constrictive tuberculous pericarditis: risk factors and outcome of pericardiectomy.

OBJECTIVE: Data on long-term outcome of pericardiectomy are limited. This retrospective study aimed to investigate risk factors and early and late outcomes of pericardiectomy for constrictive tuberculous pericarditis. DESIGN: Seventy patients with chronic constrictive pericarditis who underwent pericardiectomy between January 1990 and August 2005 were reviewed for perioperative and long-term survival. RESULTS: Patients (49 males; median age 40 years) had a median duration of symptoms of 24 months (range 8-72) before surgery. Perioperative mortality was 8.6%. During follow-up (mean 66.4 +/- 5

Transsternal bilateral thoracotomy for pericardiectomy after coronary artery bypass grafting.

Surgery for constrictive pericarditis was conducted through a transsternal bilateral thoracotomy in a 45-year-old man who developed the condition 12 months after coronary artery bypass grafting with left internal thoracic artery and vein grafts. The grafts ran just beneath the sternum. To avoid injury to the bypass grafts during sternotomy and mediastinal dissection, we conducted a transsternal bilateral thoracotomy, which provided excellent exposure of the heart. Complete pericardiectomy was done safely without cardiopulmonary bypass. Constrictive pericarditis following cardiac surgery is an

Cardiac tamponade in systemic juvenile rheumatoid arthritis requiring emergency pericardiectomy.

A 9-year-old boy with systemic onset juvenile rheumatoid arthritis (JRA) presented with fever and chest pain and rapidly developed pericarditis and cardiac tamponade. Despite corticosteroid treatment and pericardiocentesis, rapid deterioration necessitated the emergency placement of a pericardial window. This is the first reported instance of this type of emergency surgical intervention for JRA. Of 220 children with JRA followed for 12 years (7% systemic onset), 8 had evidence of acute pericarditis but no other had definite evidence of tamponade.

Total pericardiectomy for chronic constrictive pericarditis using femoro-femoral bypass.

A technique of total pericardiectomy for chronic constrictive pericarditis is presented. The authors have used a femoro-femoral cardiopulmonary bypass in nine cases of constrictive pericarditis to prevent the complications of the traditional approach. The advantages of this technique in comparison to the performance of pericardiectomy without the use of extracorporeal circulation (ECC) or with standard ECC are discussed.

A five-year-old male castrated domestic shorthair cat was referred for respiratory distress and pleural effusion. Echocardiography identified a moderate to severe volume of echogenic pericardial effusion (PCE), pericardial thickening, and a mild volume of anechoic pleural effusion. Computed tomography with intravenous contrast confirmed a large volume of PCE with concurrent pericardial thickening; a pericardiectomy was subsequently performed. Cytological analysis of the PCE was consistent with predominantly neutrophilic inflammation. Histopathology of the pericardium showed severe, pyogranulom
