[The potentials of virtual reality in pediatric perioperative care].

Each year, millions of children worldwide undergo general anesthesia for various surgical or diagnostic procedures. In Hungary, approximately 20,000 to 30,000 children are treated in pediatric surgical centers annually, and additional 5,000 to 10,000 children undergo operation in adult otolaryngology departments. Treating children is a challenge, as their cognitive and emotional development levels may pose difficulties for them in understanding the procedures ahead and may make the processing of the stressful experiences of the intervention uncertain. Preoperative anxiety can often lead to inc

Standardization in Perioperative Care for Children with Medical Complexity.

Children with medical complexity undergoing surgery face significant risks for intraoperative and postoperative complications. Although research is limited, most studies suggest that a deliberate, patient- and family-centered, multidisciplinary approach throughout the perioperative process can reduce these risks. Strategies such as enhanced recovery after surgery protocols and perioperative checklists may offer opportunities to provide standardized and patient- and family-centered care for children with medical complexity undergoing surgery. These strategies allow us to best advocate for these

Perioperative Care of a Pediatric Patient With Beals Syndrome.

The trismus pseudocamptodactyly syndrome (Beals syndrome) is an uncommon autosomal dominant condition first described in 1971. The disorder shares phenotypic similarities with Marfan syndrome. Affected patients classically present with two main physical features: limited excursion of the mandible and flexion deformity of the fingers that occurs with wrist extension (pseudocamptodactyly). The primary cellular defect is a mutation of the fibrillin-2 (FBN2) gene on chromosome 5q23. Mutations to this gene change the structure of the FBN2 protein, decreasing the elasticity and altering the strength

Tramadol Use in Perioperative Care and Current Controversies.

PURPOSE OF REVIEW: The aim of this review is to discuss the use of tramadol in the perioperative period. There is no doubt that tramadol has revolutionized pain treatment, making it important to understand the pharmacokinetics and pharmacodynamics in order to provide patients with the safest and most effective analgesia. RECENT FINDINGS: Tramadol is a centrally acting synthetic analgesic with a multimode of action used to help treat moderate to severe pain. Pharmacologically, the unique opioid acts as a serotonin-norepinephrine reuptake inhibitor, while its metabolite, O-desmethyltramadol, ac

Gabapentinoid Use in Perioperative Care and Current Controversies.

PURPOSE OF REVIEW: This review summarizes the risks and benefits of gabapentinoids (gabapentin and pregabalin) for perioperative pain control and the controversies surrounding their use in a variety of settings. We review current literature with the goal of providing patient-centric and procedure-specific recommendations for the use of these medications. RECENT FINDINGS: Gabapentinoids are among the most prescribed medications in the USA, and typically for off-label indications such as postoperative pain. In the perioperative setting, multimodal analgesic or "opioid-sparing" regimens have bec

[Intensive care and perioperative monitoring in liver transplantation].

Perioperative intensive care has several objectives in the context of liver transplantation. Prior to the operation, it is essential for the management of fulminant hepatic failure in order to limit the consequences of cerebral oedema. During the operation, intensive care consists of monitoring and correction of the occasionally severe circulatory and metabolic disorders induced by vascular exclusion of the liver and revascularization of the cold, ischaemic, orthotopic transplant. The possibility of major haemorrhage requiring massive transfusion with stored blood aggravates these disorders. C

Perioperative care in older adults: a narrative review.

PURPOSE: The perioperative care of older patients is challenging due to clinical complexity. In this narrative review, we summarized the most important aspects of perioperative care of very old adults. METHODS: We performed a non-systematic narrative review of available studies to summarize evidence to support best clinical practice in perioperative care of very old patients. When appropriate, evidence was supplemented with data from broader population of older adults. RESULTS: The preoperative assessment should take into consideration comorbidities and cardiac risk as well as geriatric synd

Optimizing perioperative care in esophagectomy: a narrative review.

BACKGROUND AND OBJECTIVE: Perioperative management of esophageal cancer surgery is continuously evolving. Advances in patient preparation, surgical techniques, and the implementation of enhanced recovery after surgery (ERAS) protocols have collectively improved postoperative outcomes and recovery. Minimally invasive esophagectomy has been widely adopted due to its association with reduced surgical trauma, less blood loss, shorter hospital stay, and, in some studies, lower complication rates. Despite these advances, esophagectomy remains a high-risk procedure with substantial morbidity, undersc

Advancing perioperative care: introducing patient-centered comfort management.

The perioperative patient-centered comfort management (2P2CM) model introduces a transformative approach to surgical care, prioritizing patient comfort, psychological well-being, and satisfaction across the perioperative continuum. Unlike traditional models that focus primarily on clinical outcomes, the 2P2CM model emphasizes a holistic framework that integrates preoperative, intraoperative, and postoperative interventions tailored to individual patient needs. This review synthesizes the emerging evidence supporting the 2P2CM model, which aims to alleviate preoperative anxiety, optimize postop

Perioperative Care of Patients Using Wearable Diabetes Devices.

The increasing prevalence of diabetes mellitus has been accompanied by a rapid expansion in wearable continuous glucose monitoring (CGM) devices and insulin pumps. Systems combining these components in a "closed loop," where interstitial glucose measurement guides automated insulin delivery (AID, or closed loop) based on sophisticated algorithms, are increasingly common. While these devices' efficacy in achieving near-normoglycemia is contributing to increasing usage among patients with diabetes, the management of these patients in operative and procedural environments remains understudied wit

Clinical hypnosis: implications in anaesthesia and perioperative care.

A randomised trial published in the British Journal of Anaesthesia describes hypnosis compared with general anaesthesia in 60 children undergoing superficial surgery. We describe a definition of clinical hypnosis; the goals and conduct of hypnotic communication; and its potential as both an adjunct and, in suitable cases, alternative to traditional pharmacological anaesthesia.

Multidisciplinary Perioperative Care for Children with Neuromuscular Disorders.

Children with neuromuscular diseases present unique challenges to providing safe and appropriate perioperative care. Given the spectrum of disease etiologies and manifestations, this is a population that often requires specialized multidisciplinary care from pediatricians, geneticists, neurologists, dieticians, and pulmonologists which must also be coordinated with surgeons and anesthesiologists when these patients present for surgery. Several of these diseases also have specific pharmacologic implications for anesthesia, most notably mitochondrial disease and muscular dystrophies, which put t

[Perioperative care of older patients with hip fractures].

The number of osteoporotic fractures is increasing along with population aging. Most patients with these type of fractures are older than 65 years, with multiple chronic conditions and different degrees of disability. Hip fracture is the most relevant osteoporotic fracture due to its frequency, costs, severity and complications. Multidisciplinary management is of the utmost importance to obtain good therapeutic results. We herein review the management of this fracture. Orthogeriatric joint management should be incorporated in fragility fracture treatment. We contribute with general recommendat
