BACKGROUND: Electrical impedance tomography (EIT) is a non-invasive radiation-free monitoring technique that provides images based on tissue electrical conductivity of the chest. Several investigations applied EIT in the context of perioperative medicine, which is not confined to the intraoperative period but begins with the preoperative assessment and extends to postoperative follow-up.
MAIN BODY: EIT could provide careful respiratory monitoring in the preoperative assessment to improve preparation for surgery, during anaesthesia to guide optimal ventilation strategies and to monitor the hem

BACKGROUND: The majority of standard interventions in otorhinolaryngology are classified as "clean contaminated" according to international classifications; correspondingly, no generally accepted recommendations regarding perioperative antibiotic prophylaxis (pAP) exist. The value of such pAP for these interventions remains unclear. Aim of the study was to assess the effects of pAP in selected standard otorhinolaryngologic procedures.
MATERIALS AND METHODS: In August 2012 a standard operating procedure (SOP) was implemented, which lead to termination of routine pAP for the majority of standar

Background: Precision medicine represents an evolving approach to improve treatment efficacy by modifying it to individual patient's gene variation. Pharmacogenetics, an applicable branch of precision medicine, identifies patient's predisposing genotypes that alter the clinical outcome of the drug, hence preventing serious adverse drug reactions. Pharmacogenetics has been extensively applied to various fields of medicine, but in the field of anesthesiology and preoperative medicine, it has been unexploited. Although the US Food and Drug Administration (FDA) has a table of pharmacogenomics biom

PURPOSE OF REVIEW: The purpose of this review is to summarize the current state of perioperative medicine, including the perioperative surgical home (PSH) and enhanced recovery after surgery pathways (ERAS) as well as the educational implications of these concepts for current and future anesthesiology trainees.
RECENT FINDINGS: Although there is significant, ongoing discussion surrounding the structural concept of the PSH, there remains little clinical evidence to support its development. On the other hand, publications surrounding ERAS principles continue to show clinical benefit in reducing

Metabolism and immune function are tightly intertwined, and have been the focus of research in the expanding field of immunometabolism. Immunometabolism focuses on the metabolic adaptation of immune cells to their environment, allowing an efficient and targeted immune response even in hypoxic or nutrient-depleted tissue. The inflammatory response can be pathologically altered in chronic metabolic disease or acute injury as a result of poor immunometabolic adaptation, contributing to short- and long-term complications. Novel techniques, including 'omics' investigations, advanced imaging, and ph

Demographic change is leading to an increasing number of old patients both in our society and in hospitals. With increasing age, not only the number of pre-existing conditions increases, but also the postoperative complication rate and mortality. Ultimately, however, it is not age that is decisive, but the condition of the patient and his or her capacity to face the physical and mental challenges of a surgical procedure. Frail patients are particularly at risk of complications, and an essential strategy - known as prehabilitation - is to put them in a better state pre-operatively through physi

Chronic pain is a frequent comorbidity of patients in hospitals and has an influence on the clinical course and the duration of hospitalization. There is a need to have a better understanding of chronic pain as a comorbidity and it should be considered to a greater extent in understanding diseases, in treatment concepts and hospital structures to ensure a resource-oriented and high-quality care. This begins on admission by identifying pre-existing pain and related risk factors with the medical history and taking these into account in the treatment regimen. A multimodal treatment approach that

AIM: To evaluate the models of general medicine perioperative services and describe the characteristics and outcomes of perioperative referrals.
METHODS: A multicentre cross-sectional study involving 11 Victorian public hospitals was conducted over 6 weeks at each site. Adult patient (≥18 years old) referrals from all surgical services were included. Comparative analyses of outcomes based on models of care were undertaken and adjusted for covariates.
RESULTS: The overall service activity was 427 (95% CI: 403-453) referrals per 10 000 surgical admissions per year. Of 1071 referrals, 922 (86.1

Obstructive sleep apnea (OSA) is a prevalent sleep-disordered breathing with potential long-term major neurocognitive and cardiovascular sequelae. The pathophysiology of OSA varies between individuals and is composed of different underlying mechanisms. Several components including the upper airway anatomy, effectiveness of the upper airway dilator muscles such as the genioglossus, arousal threshold of the individual, and inherent stability of the respiratory control system determine the pathogenesis of OSA. Their recognition may have implications for the perioperative health care team. For exa

Ophthalmic surgery would a surgical practice with fewer complications compared to other types of surgery. This is only true in part, because of the elderly population of reference. So, in cataract surgery we are faced with a patient with heart disease which carries the signs of aging and degenerative consequences linked to it. The interventions performed in patients with heart disease are still potentially have a greater risk, although site preparation and monitoring during and after surgery in recent years have allowed greater safety for patients and an overall reduction in morbidity and mort

Australian general medicine perioperative services provide care to surgical patients with complex comorbidities, for a wide range of medical conditions. Heterogeneous models of care exist; however, their cost-consequence has not been evaluated. Cost analysis was conducted using nationally standardised cost weights for length of stay, medical emergency team call costs and labour costs. Proactive models of general medicine perioperative care likely save approximately $7500 per patient compared with reactive models.

BACKGROUND: Surgery provides definitive management of many age-related diseases, relieving symptoms or extending life. Age-related physiological decline, multimorbidity, and frailty predispose older people to postoperative complications and incomplete functional recovery, with resultant health and social care costs. These age-related conditions can be optimized using Comprehensive Geriatric Assessment (CGA), thus mitigating perioperative risk to improve clinical outcomes with cost-effectiveness. National organizations advocate CGA-based services for older surgical patients. However, there is v

BACKGROUND: High flow nasal therapy (HFNT) is a technique in which humidified and heated gas is delivered to the airways through the nose via small nasal prongs at flows that are higher than the rates generally applied during conventional oxygen therapy. The delivered high flow rates combine mixtures of air and oxygen and enable different inspired oxygen fractions ranging from 0.21 to 1. HFNT is increasingly used in critically ill adult patients, especially hypoxemic patients in different clinical settings.

Health care delivery in the United States continues to balance on the tight rope that connects its transition from volume to value. Value in economic terms can be defined as the amount something exceeds its commodity price and is determined by extraordinary reputation, quality, and/or service, whereas its destruction can be a consequence of poor management, unfavorable policy, decreased demand, and/or increased competition. Going forward, payment for health care delivery will increasingly be based on services that contribute to improvements in individual and/or population health value, and fun

A 72-year-old woman underwent surgery for a distal radius fracture with lower jaw fracture under general anesthesia. Preoperative laboratory data showed hypokalemia (3.1 mEq · l(-1)), hypertension, and leg edema. The suspected cause of all of these symptoms was the licorice component of the multiple herbal medicines which she was taking. In addition, the ephedra and aconite tuber components of the Maobushisaishinto were suspected to be contributing to the hypertension. She was therefore taken off all of her herbal medicines. The patient underwent regular blood tests and her potassium levels we
