Pacemaker process of ureteral peristalsis in multicalyceal kidneys.

The mechanical activity of isolated strips from different areas of the pyeloureteral system was investigated in 10 pigs: calyx, renal pelvis, pyeloureteral junction and ureter. Additionally, electrical activity was measured in some pyeloureteral preparations using the sucrose-gap technique. Regular spontaneous activity with an average frequency of 9.5/min was recorded in calyceal strips, decreasing to 5.4/min in renal pelvis, 5.7/min in pyeloureteral preparations and to 1.2/min in ureteral preparations. The activity of renal pelvis, pyeloureteral and ureteral preparations was less regular, and

Initiation and propagation of canine renal pelvic peristalsis.

A new in vitro method was employed which enabled detailed examination of the initiation and propagation of the peristaltic contraction of the renal pelvis and ureter. Both the kidney and its attached ureter were carefully removed and the renal parenchyma concealing the renal pelvis and calyces removed under the dissection microscope. At the pelvicalyceal border, ripple-like peristaltic contraction of a constant frequency were observed. At the same time, electromyograms with a slow rising phase were recorded at constant intervals from the pelvicalyceal border, leading us to deduce that these mi

The renal pelvis and ureteral peristalsis. I. Pelveometry.

The renal pelvis is a visco-elastic organ and serves the double purpose of acting as a pump function and a reservoir. With a "pig-tail" catheter ("William Cook Europe" aps, Søborg, Denmark), placed transrenally in the canine pelvis and external blocking of the pelveoureteral junction, it is possible to change the volume of the canine renal pelvis and register the resulting pelvic pressure. This examination is called pelveometry. In 24 adult, female dogs pelveometry showed a linear relation within the range of 0-5.5 ml and 1-30 mmHg, respectively. The correlations between the measurements and t

Biomechanical increase in cervical esophageal wall tension during peristalsis.

During pharyngeal phase of swallowing, circumferential tension of the cervical esophagus (CTE) increases caused by a biomechanical process of laryngeal elevation pulling the cervical esophagus orad. The esophagus contracts longitudinally during esophageal peristalsis, therefore, we hypothesized that CTE increases during esophageal peristalsis by a biomechanical process. We investigated this hypothesis using 28 decerebrate cats instrumented with electromyographic (EMG) electrodes on the pharynx and esophagus, and esophageal manometry. We recorded CTE, distal esophageal longitudinal tension (DET

Myomectomy decreases abnormal uterine peristalsis and increases pregnancy rate.

BACKGROUND: The relationship between fibroids and infertility remains a critical and unresolved question. During the implantation phase, it is known that uterine peristalsis is dramatically reduced, which is thought to facilitate implantation of the embryo to the endometrium. In the previous study, using a cine MRI mode, we found that less than half of the patients with intramural fibroids exhibited abnormal uterine peristalsis during the mid-luteal phase. In the present study, we further investigated whether myomectomy for patients in the high peristalsis group is a constructive method to nor

Role of hyperpolarization-activated cation channels in pyeloureteric peristalsis.

Although it has been recognized for more than 140 years that the pacemaker driving pyeloureteric peristalsis resides within the kidney, the cellular mechanisms underlying this autorhythmicity have been little investigated. The demonstration by Hurtado et al. of a role of hyperpolarization-activated cation channels in the maintenance of coordinated propagating contractions in the mouse renal pelvis raises a number of intriguing possibilities in addition to the provided correlation with 'pacemaker' function as in the heart and neurons.

Secondary oesophageal peristalsis in patients with non-obstructive dysphagia.

Secondary peristalsis was investigated in 30 patients with non-obstructive dysphagia and 20 age matched controls. Oesophageal motility was recorded at 3 cm intervals along the oesophageal body. Primary peristalsis was tested with 5 ml water swallows. Secondary peristalsis was stimulated with 10 ml boluses of air and water injected in the mid-oesophagus and by distensions (5 seconds duration) with a 3 cm balloon at the same level. Primary peristalsis was normal in 19 of the 20 control subjects and in nine of the 30 patients with dysphagia; 11 patients had diffuse spasm and 10 had non-specific a

Control of ureteral peristalsis during changes in intravesical pressure.

Studies of the ureteral response to changes in intravesical pressure (IP) have been conducted by surgically altering the neural and hydrodynamic integrity of canine ureters. Ureteral function was assessed by acute and chronic recordings of ongoing electrical activity. Intact control ureters responded (acute to 56 per cent; chronic to 83 per cent) to elevated IP with immediate increases and to decreases in IP with slowing or cessation of activity. Acute ureteral ligation mimicked the effects of increased IP, except the responses were much slower. Ureterotomy and the resultant extravasation of u

Does Intestinal Peristalsis Cause Suture Failure After Instrument Suture?

BACKGROUND/AIM: Gastrectomy with lymphadenectomy is a standard treatment for gastric cancer. Anastomotic leakage remains a potentially fatal complication of gastrectomy. Forceful stapler extraction may cause anastomotic complications. We focused on the duodenal peristalsis, as we hypothesized that it might cause forceful stapler extraction. We then retrospectively investigated duodenal peristalsis and reviewed videos of Da Vinci system cases to clarify the relationship between peristalsis and anastomotic complications. PATIENTS AND METHODS: Forty-nine cases with stored videos of laparoscopic

Onset of oesophageal peristalsis after surgery for idiopathic achalasia.

Twenty-seven patients were treated with a Heller myotomy and Dor fundoplication; some peristaltic contractions occurred in seven after operation. When clinical, radiological and manometric data in the seven patients developing peristalsis were compared with findings in the other 20, there were no differences in symptoms, lower oesophageal sphincter pressures and lengths, relief of dysphagia or oesophageal calibre reduction. Oesophageal resting pressure was lower and oesophageal contraction amplitudes were statistically higher in patients with restored peristalsis, which correlated only with th

Recurrent autonomous esophageal peristalsis in patients with chest discomfort.

Routine esophageal manometry revealed a novel pattern of esophageal motility, recurrent autonomous peristalsis, in five patients evaluated for chest discomfort and heartburn. In the absence of swallowing, esophageal peristalsis occurred at frequencies of 4-8 peristaltic sequences per minute for periods of 2-8 min. The recurrent peristaltic sequences developed spontaneously during manometry and were associated with the onset of vague chest discomfort in four patients; one patient reported no symptoms during recurrent peristalsis. Duration and propagation velocity of the recurrent peristaltic co

Numerical analysis of the urine flow in a stented ureter with no peristalsis.

A ureteral stenosis or occlusion causes the disturbance of normal urine flow and results in renal failure. Ureteral stents are used to relieve the stagnation of urine in the upper urinary tract. Peristalsis in the ureter, which occurs to help urine flow, becomes to weaken when a stent is inserted and effective peristalsis disappears as time goes on, and a stented ureter seems to be tubular and curved in the human body. Double J stents, which are manufactured by many medical companies and are used widely these days, have different geometries of side holes in the stent shafts. In total, 12 model

Effect of tragus clips on gastric peristalsis: a pilot study.

In the past 22 years we have used the "hunger point" on the tragus of the ear to induce aversion to cigarette smoking and inhibit appetite for weight reduction with varying degrees of success. This has generally been accomplished by inserting press needles into the tragus of both ears. Because these needles gradually lose effect over a 5- to 7-day period, they have had to be changed every 5 to 7 days. To obviate this need, ear-clips (Aculite; I-Openers, Cranston, RI) were devised to be worn on the tragus. Previous work using press needles on the tragus induced slowing of gastric peristalsis, t

Effect of dystonic movements on oesophageal peristalsis in Sandifer's syndrome.

A 5 year old girl with gross gastrooesophageal reflux had pronounced dystonic neck movements that resolved after fundoplication and resolution of the oesophagitis (Sandifer's syndrome). Oesophageal manometry before operation showed that peristalsis improved when she tilted her head to one side. Dystonic posturing may promote acid clearance from the distal oesophagus.

Mechanical simulation of renal pelvic wall peristalsis in the rat.

A urinary concentrating defect was induced in Munich-Wistar rats by removing the renal pelvis from 1 kidney. This defect was partially corrected by crudely simulating the actions of pelvic wall peristalsis with a mechanical system that cyclically compressed the exposed renal papilla.

Return of esophageal peristalsis in achalasia secondary to gastric cancer.

A 69-year-old white man developed progressive symptoms of dysphagia for solids and liquids and regurgitation of undigested food accompanied by a 12-kg weight loss over a 4-month period. Initially, radiographs of the esophagus and stomach were normal, but when repeated 4 months later, a diagnosis of achalasia was suggested. Esophageal manometry performed at that time demonstrated a motor abnormality of the esophagus and lower esophageal sphincter consistent with a diagnosis of achalasia. Upper endoscopy revealed a small ulcerated tumor in the cardia of the stomach. A biopsy specimen was interpr

Rhythmic contractions in the renal pelvis correlated to ureteral peristalsis.

Using a transparenchymal pressure catheter and a specially designed DC amplifier, rhythmic pressure variations have been demonstrated to occur in the renal pelvis of dogs. For the hydropenic dog this frequency is constant during the course of all the experiments. The interperistalic interval in the ureter constitutes a multiple of the pelvic pressure variations. The ratio of ureteral to pelvic activity is 1 during osmotic diuresis. The frequency of the ureteral activity was never found to be larger than the pelvic activity. These experiments suggest that the ureteral contractions are correlate

Distension-contraction profile of peristalsis in patients with nutcracker esophagus.

INTRODUCTION: High amplitude peristaltic esophageal contractions, that is, nutcracker esophagus, were originally described in association with "angina-like pain" of esophageal origin. However, significant number of nutcracker patients also suffer from dysphagia. High-resolution esophageal manometry (HRM) assesses only the contraction phase of peristalsis. The degree of esophageal distension during peristalsis is a surrogate of relaxation and can be measured from the intraluminal esophageal impedance measurements. AIMS: Determine the amplitude of distension and temporal relationship between di
