Erythropoietin resistance in patients on continuous ambulatory peritoneal dialysis.

Anemia is one of the most serious complications in patients on dialysis. Erythropoietin improves the anemia. However, erythropoietin resistance is sometimes encountered from causes such as functional iron deficiency, secondary hyperparathyroidism, blood loss, or interaction with other drugs. To clarify the interaction between erythropoietin and the renin-angiotensin system, we studied the maintenance dose of recombinant human erythropoietin (rHuEPO) in patients on continuous ambulatory peritoneal dialysis (CAPD) with and without angiotensin converting enzyme inhibitor (ACEIs), angiotensin II t

Mesothelial paracrystalline inclusions in continuous ambulatory peritoneal dialysis patients.

OBJECTIVE: Uremia is known to be followed by changes in the serous membranes of pleura, pericardium, and peritoneum. During continuous ambulatory peritoneal dialysis (CAPD), the peritoneum is exposed to altered body conditions as well as to the influence of dialysate. The aim of the present study was to examine the ultrastructure of the mesothelial cells in CAPD patients, and to compare the findings with those from studies of the peritoneum in uremic controls. Paracrystalline intracytoplasmic inclusions in mesothelial cells were objects of special interest. METHODS: Biopsies of human parietal

Unexpected nonocclusive mesenteric infarction during continuous ambulatory peritoneal dialysis.

An unexpected case of nonocclusive mesenteric infarction in a patient on continuous ambulatory peritoneal dialysis is described. The common clinical finding in this entity is a low cardiac output state or even circulatory collapse. Nonocclusive mesenteric infarction has been reported very rarely during continuous ambulatory peritoneal dialysis, and only in the setting of cardiac dysfunction or prolonged hypotension. The current patient had none of the typical features commonly described in the clinical setting of nonocclusive mesenteric infarction; potential mechanisms, including the role of e

Histoplasma capsulatum infection associated with continuous ambulatory peritoneal dialysis.

Fungal infection has become increasingly more important in patients undergoing continuous ambulatory peritoneal dialysis. We report here a case of Histoplasma capsulatum infection in such a Hong Kong Chinese patient who presented with fever and peritonitis. Histoplasma capsulatum was isolated from the dialysis fluid and histoplasma antibody was detected in the serum. The patient responded to the combined treatment of fluconazole, 5-flurocytosine and amphotericin B. This is the first reported case of histoplasmosis in Hong Kong.

Vancomycin-associated exfoliative dermatitis during continuous ambulatory peritoneal dialysis.

Vancomycin is commonly prescribed to patients undergoing continuous ambulatory peritoneal dialysis (CAPD) for catheter-related infections and acute episodes of peritonitis. Although adverse dermatological reactions have been reported secondary to the rapid intravenous infusion of vancomycin, the intraperitoneal route of administration has been used routinely during CAPD without these effects. This case report describes a CAPD patient with systemic lupus erythematosus who developed erythema multiforme that progressed to exfoliative dermatitis during intermittent intraperitoneal vancomycin thera

Recurrent hemoperitoneum in women receiving continuous ambulatory peritoneal dialysis.

Of 27 women in the reproductive age group receiving continuous ambulatory peritoneal dialysis for more than 3 months, 4 of 7 who menstruated developed recurrent hemoperitoneum. Tubal ligation had been done in 3 of these 4 women. There were 37 episodes of hemoperitoneum; 22 occurred at midcycle and 15 with menstruation. One patient required repeated blood transfusion, but after oral anovulant therapy no further bleeding occurred and no transfusion was required. Two patients needed laparotomy: one for heavy intraperitoneal bleeding originating from a luteal cyst, and the other for severe lower a

Renal osteodystrophy in children undergoing continuous ambulatory peritoneal dialysis.

This paper describes a retrospective evaluation of the course of renal bone disease in 14 children undergoing treatment with continuous ambulatory peritoneal dialysis (CAPD) for an average of 11.9 +/- 1.5 months (mean +/- SE). The patients were divided in two groups according to the changes in serum alkaline phosphatase activity during the period of observation: five patients had alkaline phosphatase activity that decreased or was relatively stable (group I), and nine patients exhibited a rising serum alkaline phosphatase activity (group II). Serial radiological examinations showed adequate co

Progress of renal osteodystrophy during continuous ambulatory peritoneal dialysis.

At present, only a few data are available on the effects of continuous ambulatory peritoneal dialysis (CAPD) on renal osteodystrophy. Twelve patients on CAPD were studied for one year, in order to evaluate the progress of renal osteodystrophy. Our results showed a downward trend of plasma calcium, a good control of phosphatemia, a significant increase of alkaline phosphatase and parathyroid hormone. We also found an important decrease of plasma vitamin D metabolites (25(OH)D3 and 1,25 (OH)2D3) and bone mineral content. Bone biopsy showed a general worsening of both secondary hyperparathyroidis

Growth in children treated with continuous ambulatory peritoneal dialysis.

Adequate growth continues to be a major problem in the treatment of renal failure in children and adolescents. During this study data were obtained which indicate that patients treated with continuous ambulatory peritoneal dialysis (CAPD) can grow at a normal or even accelerated rate if renal osteodystrophy and acidosis is controlled, caloric intake exceeds 100% RDA, and protein intake exceeds 2 gm/Kg. Since young infants can be treated with CAPD it provides the potential to produce adequate growth to children born with renal lesions leading to early onset renal insufficiency.

Renal osteodystrophy in children undergoing continuous ambulatory peritoneal dialysis.

Fifteen children undergoing continuous ambulatory peritoneal dialysis for 0.3 to 2.4 years were evaluated longitudinally for renal osteodystrophy. Immunoreactive parathyroid hormone, 25-OHD, total and ionized calcium, inorganic phosphate, and alkaline phosphatase levels were measured regularly. Skeletal radiographic studies were performed at the onset and conclusion of CAPD and at six-month intervals during therapy. All children received 1,25(OH)2D3 and aluminum hydroxide, and nine received supplemental calcium. Plasma 25-OHD concentrations were normal to elevated, and calcium increased steadi

Eosinophilic peritonitis in a patient with continuous ambulatory peritoneal dialysis (CAPD).

Eosinophilic peritonitis is defined as when there are more than 100 eosinophils present per milliliter of peritoneal effluent, of which eosinophils constitute more than 10% of its total WBC count. Most cases occur within the first 4 weeks of peritoneal catheter insertion and they usually have a benign and self-limited course. We report a patient of eosinophilic peritonitis that was successfully resolved without special treatment. An 84-year-old man with end stage renal disease secondary to diabetic nephropathy was admitted for dyspnea and poor oral intake. Allergic history was negative. and ph

The case for oral treatment of peritonitis in continuous ambulatory peritoneal dialysis.

Among several regimes used in the oral treatment of continuous ambulatory peritoneal dialysis (CAPD)--related peritonitis, ciprofloxacin and ofloxacin showed the higher treatment efficacy, with an overall cure rate of 80% over a treatment duration of 10-16 days. The antimicrobial activity of these agents against gram-positive episodes was equal to or even higher than that against other peritonitis episodes, and the effectiveness of oral ciprofloxacin can be further increased either by an additional intraperitoneal dose for the first 1-5 days, or by a simultaneous intraperitoneal dose of vancom

Host defences in continuous ambulatory peritoneal dialysis and the genesis of peritonitis.

Continuous ambulatory peritoneal dialysis (CAPD) has come to be extensively used for the treatment of end-stage renal failure in children, and especially infants, such that now more than half of children on dialysis worldwide receive treatment by this means. Peritonitis, however, is commoner in children than in adults receiving treatment, and is a major source of morbidity and treatment failure in children started on CAPD. Only recently has the immunology of the normal peritoneum been studied extensively, with the need to assess the impact of the installation of large volumes of fluid into the

Treatment of peritonitis in patients undergoing continuous ambulatory peritoneal dialysis.

The epidemiology, etiology, pathogenesis, diagnosis, and pharmacotherapy of peritonitis in patients with end-stage renal disease treated with continuous ambulatory peritoneal dialysis (CAPD) are reviewed. CAPD-associated peritonitis is a localized infection of the peritoneal cavity. Approximately 70% of the cases are caused by a single gram-positive microorganism indigenous to the patient's skin or upper respiratory tract that infects the peritoneal cavity. Gram-negative microorganisms cause 25% of the cases; fungi, anaerobes, and mycobacteria cause approximately 5%. Clinical manifestations in

Dyspepsia in Turkish patients on continuous ambulatory peritoneal dialysis.

BACKGROUND AND AIMS: Although dyspepsia is very common in uremic patients, there is not much data on the prevalence of dyspepsia undergoing continuous ambulatory peritoneal dialysis (CAPD). The aims of this study are to assess the prevalence of dyspepsia, Helicobacter pylori, and the association between dyspeptic symptoms and diabetes. METHODS: One hundred and thirty CAPD patients were included in the study. The presence or absence of dyspepsia was assessed by using the Hong Kong Index of Dyspepsia (HKID) Questionnaire. A score of >or=16 indicates the presence of dyspepsia. The patients who w

MR peritoneography in complications of continuous ambulatory peritoneal dialysis.

BACKGROUND: We evaluated the usefulness of magnetic resonance (MR) peritoneography for the examination of complications from continuous ambulatory peritoneal dialysis (CAPD). METHODS: MR peritoneography was performed in 20 patients who had suspected CAPD-related complications. Patients who had active peritonitis were not included in the study. Before MR imaging, a mixture of 2000 mL of peritoneal dialysis solution and 20 mL of gadopentetate dimeglumine was instilled into the peritoneal cavity. MR imaging was performed on a 1.0-Tesla system using a body coil. Axial, coronal, and sagittal fat-s

Anti-viral drugs in continuous ambulatory peritoneal dialysis (CAPD).

The prevalence of HIV-positive subjects in dialysis (hemodialysis and peritoneal dialysis) population varies from 0.13 to 0.36% in italian and french studies, respectively. Most drugs used in HIV therapy are primarily excreted by the kidney. In patients with renal insufficiency, careful dosage adjustment is mandatory to optimize drug exposure and reduce the risk for adverse events. We review the impact of peritoneal dialysis on the pharmacokinetics of antiviral drugs, and discuss on the dosage recommendations needed to achieve efficacy and avoid toxicity in patients with end-stage renal diseas
