Dynamic factors involved in common peroneal nerve entrapment neuropathy.

BACKGROUND: Common peroneal nerve (CPN) entrapment neuropathy (CPNEN) is the most common peripheral neuropathy of the lower extremities. The pathological mechanisms underlying CPNEN remain unclear. We sought to identify dynamic factors involved in CPNEN by directly measuring the CPN pressure during stepwise CPNEN surgery. METHODS: We enrolled seven patients whose CPNEN improved significantly after CPN neurolysis. All suffered intermittent claudication, and the repetitive plantar flexion test, used as a CPNEN provocation test, was positive. During decompression surgery we directly measured the

High-resolution 3-T MR neurography of peroneal neuropathy.

The common peroneal nerve (CPN), a major terminal branch of the sciatic nerve, can be subject to a variety of pathologies, which may affect the nerve at any level from the lumbar plexus to its distal branches. Although the diagnosis of peripheral neuropathy is traditionally based on a patient's clinical findings and electrodiagnostic tests, magnetic resonance neurography (MRN) is gaining an increasing role in the definition of the type, site, and extent of peripheral nerve disorders. Current high-field MR scanners enable high-resolution and excellent soft-tissue contrast imaging of peripheral

Sonographic evaluation of common peroneal neuropathy in patients with foot drop.

The common peroneal nerve arises from the sciatic nerve and is subject to a variety of abnormalities. Although diagnosis is often is based on the clinical findings and electrodiagnostic tests, high-resolution sonography has an increasing role in determining the type and location of common peroneal nerve abnormalities and other peripheral nerve disorders. This article reviews the normal sonographic appearance of the common peroneal nerve and the findings in 21 patients with foot drop related to common peroneal neuropathy.

Chiropractic care for foot drop due to peroneal nerve neuropathy.

Peroneal nerve palsy is the most common entrapment neuropathy in the lower extremity, and the presentation of foot drop is a frequent initial manifestation of this disorder. This condition can have a marked influence on the patient's activities of daily living, and is classified as 'Impairment of Body Structure.' Current literature provides little direction to its evaluation and management, and the importance to differentially diagnose the causes of foot drop. Therefore, the purpose of this case report is to describe the application of chiropractic manipulative therapy, for a 14-year-old femal

Compression neuropathy of the common peroneal nerve secondary to a ganglion cyst.

There are various causes of the common peroneal nerve palsy. However, common peroneal nerve palsy caused by ganglia are uncommon. We hereby present a case of a 55-year-old man with a 1 week history of foot drop and swelling in the region of the right leg. Physical examination and nerve conduction study studies confirmed a diagnosis of common peroneal nerve palsy. Magnetic resonance imaging (MRI) revealed a lobulated, elongated cystic-appearing mass anterior to the head of fibula. Surgical decompression of the nerve with removal of the mass was performed. Surgical pathology reports confirmed th

Common peroneal neuropathy in patients after first-time stroke.

BACKGROUND: Common peroneal neuropathies, usually located at the fibular head, are one of the causes of drop foot, a condition often evaluated in the electromyography laboratory. OBJECTIVES: To study the motor conduction properties of the common peroneal nerve and its branches of distribution in patients with paralyzed drop foot, several weeks after their first stroke, assuming that its inversion position can cause neuropathy around the fibular neck. METHODS: We performed peroneal nerve conduction study on 76 legs of 38 patients, 12-73 days after their first stroke. All the patients had flac

High sciatic lesion mimicking peroneal neuropathy at the fibular head.

We report 4 patients with severe, axon-loss, high sciatic mononeuropathies affecting exclusively the peroneal fibers: a boy with a slowly-growing nerve tumor, a woman with an injection injury, and 2 patients who had undergone proximal femur surgery. Clinically, the findings mimicked those seen with common peroneal neuropathy at the fibular head. The peroneal conduction studies were very low in amplitude/unelicitable. Conversely, the tibial studies and H-responses were normal in all; the sural responses were normal in one while low in amplitude/unelicitable in the remaining three. The biceps fe

Compound nerve action potential of common peroneal nerve and sural nerve action potential in common peroneal neuropathy.

To enhance the accuracy for determining the precise localization, the findings of the compound nerve action potentials (CNAPs) of the common peroneal nerve (CPN) were investigated in patients with common peroneal mononeuropathy (CPM) in the knee, and the sural sensory nerve action potentials (SNAPs) were also analyzed. Twenty-five patients with CPM in the knee were retrospectively reviewed. The findings of the CNAPs of the CPN recorded at the fibular neck and the sural SNAPs were analyzed. The lesion was localized at the fibular head (abnormal CNAPs) and at or distal to the fibular head (norma

Utilizing peroneal nerve conduction studies to differentiate L5 radiculopathy and peripheral neuropathies of the lower extremity.

BACKGROUND: L5 radiculopathy, peroneal neuropathy at the fibular head (PNFH), and sciatic neuropathy (SN) often share similar clinical features, such as foot drop. The ability of peroneal nerve conduction studies (NCSs) to distinguish these three conditions was investigated. METHODS: NCS findings of the patients with L5 radiculopathy, PNFH and SN were reviewed retrospectively. Additionally, needle electromyography (EMG) findings of the tibialis anterior (TA) and peroneus longus (PL) muscles were analyzed. RESULTS: Sixty-six patients with L5 radiculopathy, 53 patients with PNFH, and 75 patien

Benign joint hypermobility with neuropathies: documentation and mechanism of median, sciatic, and common peroneal nerve compression.

We report three patients in whom carpal tunnel syndrome was due to neurapraxis as a consequence of unusual sleep postures enabled by their benign joint hypermobility syndrome (BJHS). A fourth patient also had bilateral sensory sciatic neuropathy and a common peroneal palsy, similarly due to regularly adopted unusual postures. Recognition of such cause-effect mechanisms and their simple solution in BJHS should obviate needless investigation and unnecessary surgery.

Fabella Syndrome and Common Peroneal Neuropathy following Total Knee Arthroplasty.

CaseA 62-year-old man presented with persistent lateral knee pain 15 months following an uncomplicated total knee arthroplasty. There was a tendinous snapping structure over the lateral aspect of the knee in deep flexion with positive Tinel's sign over the fibular head. The patient underwent an uncomplicated flabella excision. The patient was cleared to return to work and full duty at two months postoperatively.ConclusionFlabella syndrome is a rare but increasingly common mechanism of persistent lateral knee pain following total knee arthroplasty. Surgeons should be aware of this etiology of p

Role of Sonography in the Diagnosis and Treatment of Common Peroneal Neuropathy Secondary to Fabellae.

The fabella is a sesamoid bone situated in the posterolateral knee, which may contribute to posterolateral knee pain by impinging on the adjacent common peroneal nerve (CPN). Although anatomic studies have established an relationship between the fabella and CPN, we present 4 cases of posterolateral knee pain radiating into the anterolateral leg in which sonography was able to determine the source of the pain as CPN compression by an adjacent fabella. In 2 of these cases, resolution of symptoms was achieved with ultrasound-guided CPN blocks, whereas 1 case was surgically treated, and another wa

[Entrapment neuropathies involving the ulnar nerve at the wrist (and into the hand) and the peroneal nerve].

We report our experience with patients who underwent surgery for entrapment neuropathies involving the ulnar nerve at the wrist and into the hand and the peroneal nerve. For the ulnar nerve, the cause of the lesion was identified in all patients, generally a cyst which had developed in the Guyon canal. The patients usually recovered completely. For the peroneal nerve, there was a wide variety of causes, with mucoid cysts frequently involved. Recovery was often incomplete, because of the very marked initial axonal damage. We emphasized the need for rapid diagnosis and surgical treatment.

Entrapment neuropathy of the deep peroneal nerve associated with the extensor hallucis brevis.

The authors report a case of entrapment neuropathy of the deep peroneal nerve associated with the extensor hallucis brevis. This entrapment neuropathy was found distal to the inferior retinaculum that causes the anterior tarsal tunnel syndrome. Surgical decompression of the deep peroneal nerve that was entrapped by the extensor hallucis brevis relieved the symptoms. This condition, like the anterior tarsal tunnel syndrome, deserves attention.

Peroneal neuropathy with foot drop after bariatric surgery: A neurosurgical perspective.

OBJECTIVE: Peroneal neuropathy is the most common entrapment neuropathy of the lower extremity and most commonly results from compression of the peroneal nerve at the fibular head. This study aimed to evaluate the relationship between peroneal neuropathy and rapid weight loss after bariatric surgery and to assess the outcomes of surgical decompression in patients presenting with foot drop. METHOD: Between 2020 and 2025, five patients underwent surgical treatment for foot drop due to peroneal neuropathy following bariatric surgery. The diagnosis was confirmed by clinical evaluation, imaging, a
