Early Ocular Response to Visual Motion in Patients With Persistent Postural-Perceptual Dizziness.

Persistent postural-perceptual dizziness (PPPD) patients with visual dependency rely more on visual cues and may have altered oculomotor responses to visual motion. This study examined early eye movement responses to large-field motion stimuli-the ocular following responses (OFRs)-in 30 PPPD patients and 29 healthy controls. Participants viewed horizontal motion in peripheral and/or central visual fields while eye movements were recorded. OFR velocity 200 ms after stimulus onset and suppression efficacy were analyzed across four central field sizes from 5° to 42° radius. Both groups exhibited

Attentional dysfunction in persistent postural-perceptual dizziness: a narrative review of mechanistic evidence.

Persistent postural-perceptual dizziness (PPPD) is a common chronic functional neuro-otological disorder characterized by persistent non-spinning dizziness, unsteadiness, and heightened sensitivity to upright posture, motion, and visually complex environments. Several conceptual models have been proposed to explain its persistence, including failure of readaptation after a destabilizing event, maladaptive multisensory integration with excessive visual or somatosensory weighting, and prediction-actual sensory mismatch with amplified prediction errors. This narrative review synthesizes neuroimag

Reduced cortical folding in multi-modal vestibular regions in persistent postural perceptual dizziness.

Persistent postural perceptual dizziness (PPPD) is a common functional vestibular disorder that is triggered and sustained by a complex interaction between physiological and psychological factors affecting spatial orientation and postural control. Past functional neuroimaging research and one recent structural (i.e., voxel-based morphometry-VBM) study have identified alterations in vestibular, visuo-spatial, and limbic brain regions in patients with PPPD and anxiety-prone normal individuals. However, no-one thus far has employed surface based morphometry (SBM) to explore whether cortical morph

Predictors of treatment response to pharmacotherapy in patients with persistent postural-perceptual dizziness.

The study aimed to identify the predictors of response to selective serotonin reuptake inhibitors (SSRIs) for 12 weeks in patients with persistent postural-perceptual dizziness (PPPD). Short-term treatment responses were studied in 197 outpatients [127 (64.5%) women, mean age ± SD = 51.7 ± 15.9] diagnosed with PPPD. Clinical and self-rated assessments were analyzed at the baseline and at the end of pharmacotherapy for 12 weeks. Multivariable logistic regression analysis was used to determine the following variables as the potential predictors of treatment response as measured by scoring in cli

Neurotransmitter regulatory networks in persistent postural-perceptual dizziness: potential mechanisms and therapeutic implications.

Persistent postural-perceptual dizziness (PPPD) is a chronic functional vestibular disorder characterized by persistent non-spinning dizziness, subjective unsteadiness, and symptom exacerbation induced by upright posture, movement, and complex visual motion. Abnormal vestibular sensory reweighting, increased visual dependence, postural hypervigilance, and comorbid anxiety are considered to contribute to the persistence of symptoms. Neurotransmitter systems, including serotonin, norepinephrine, dopamine, gamma-aminobutyric acid, glutamate, histamine, acetylcholine and neuropeptides, may partici

Postural motion perception during vestibular stimulation depends on the motion perception threshold in persistent postural-perceptual dizziness.

Patients with persistent postural-perceptual dizziness (PPPD) perceive postural instability larger than the observed sway. It is unknown whether the concept of postural misperception prevails during vestibular stimulation and whether it may account for the unsteadiness patients complain during body movements. We tested the hypothesis of an abnormal sensory-perceptual scaling mechanism in PPPD by recording objective, perceived, and the reproduced postural sway under various standing conditions, modulating visual and proprioceptive input, by binaural galvanic vestibular stimulation (GVS). We rel

Vestibular-Visual Reweighting in Persistent Postural-Perceptual Dizziness: A Multilevel Resting-State fMRI Study.

Persistent postural-perceptual dizziness (PPPD) is a disabling functional vestibular disorder characterized by chronic dizziness and visually and motion-induced unsteadiness that markedly impairs daily activities, yet it lacks objective neurobiological markers. We acquired resting-state functional MRI (rs-fMRI) in 52 patients with PPPD and 50 age- and sex-matched healthy controls (HCs) and analyzed the data using a three-tier approach: (i) functional network connectivity (FNC) of independent component analysis (ICA-FNC), (ii) voxel-wise measures of spontaneous amplitude of low-frequency fluctu

Misdiagnosis of spinocerebellar ataxia type 3 as persistent postural-perceptual dizziness: A case report.

RATIONALE: Spinocerebellar ataxia type 3 (SCA3) is the most common autosomal dominant inherited spinocerebellar disorder. The disease can easily be misdiagnosed because the symptoms of SCA3 are diverse and not obvious in the early stages. PATIENT CONCERNS: A 55-year-old woman had experienced dizziness and an unstable gait since 2017. She was misdiagnosed with persistent postural-perceptual dizziness at another hospital because of normal brain magnetic resonance imaging and emotional abnormalities. In our hospital, there were no abnormalities in routine laboratory tests and brain magnetic reso

Effects of Kampo medicine hangebyakujutsutemmato on persistent postural-perceptual dizziness: A retrospective pilot study.

BACKGROUND: Persistent postural-perceptual dizziness (PPPD) is a functional disorder, typically preceded by acute vestibular disorders. It is characterized by a shift in processing spatial orientation information, to favor visual over vestibular and somatosensory inputs, and a failure of higher cortical mechanisms. To date, no therapies for PPPD have been approved. Kampo medicine hangebyakujutsutemmato (HBT) has been reported to alleviate disturbances of equilibrium. We hypothesized that HBT would be a beneficial treatment for PPPD. AIM: To examine the efficacy of HBT for the treatment of PPP
