A detailed analysis of the CT findings in 16 surgically verified cases of meningioma of the posterior surface of the petrous bone is presented. The results indicate that a correct preoperative diagnosis is possible in almost every case. Frequently occurring specific CT criteria for meningioma of the posterior surface of the petrous bone include: hyperdense, homogeneously enhancing, extra-axial, CPA mass; inverse relationship between precontrast attenuation value and degree of contrast enhancement of the tumor; oval shape; obtuse angle between lateral tumor border and posterior surface of petro

Eight patients with extensive petrous bone cholesteatomas that invaded the labyrinth and fallopian canal are presented. The eight cases are added to a tabulation of prior literature reports to elucidate concepts of routes of extension of medially invasive temporal bone cholesteatoma. Medially invasive petrous bone cholesteatoma develops insidiously, often without symptoms other than facial palsy and/or unilateral deafness. Typically, a history of chronic ear disease can be obtained. While hearing is unlikely to be preserved in this group of patients, facial nerve function can usually be preser

The present article reports the clinical cases of the surgical intervention on 20 patients presenting with petrous bone cholesteatoma. We have identified several clinical variants of petrous bone cholesteatoma based on the results of multispiral computed tomography (MSCT) of the temporal bones and categorized them into the following types in accordance with the classification proposed by Moffat-Smith an M. Sanna for this pathological condition: supralabyrinthine (n=8), supralabyrinthine-apical (n=2), infralabyrinthine (n=3), infralabyrinthine-apical (n=5), massive (n=1), and massive - apical (

Four patients, 3 woman and 1 man, with giant petroclival meningiomata are presented. In all cases the tumor size exceed 44 mm in its greater diametre. Main clinical symptoms were endocranial hypertension and cranial nerves deficit. A modified transcochlear approach in two stages was used. A total petrosectomy was performed, using the petrosygmoidal via for opening the dura. Transversus and sygmoid sinus were spared. Total removal was achieved in 2 cases, subtotal and partly in each of the other 2. No operative mortality occurred (hospital exitus) and neurologic morbility related to V-VI-VII an

An unusual case of a congenital petrous bone epidermoid which resulted in a sudden deafness is reported. The patient was a 25-year-old man who suddenly developed a right deafness and tinnitus. Neurological examination on admission revealed slight hearing disturbance in the high sound area. Plain craniogram showed a destructive bone lesion in the middle portion of the petrous bone on the right side. Bone-window CT scan disclosed a bony destructive mass in the petrous bone, and enlargement of the internal auditory canal. The mass lesion was almost isodense on plain CT scan and was not enhanced b

High resolution transverse axial computerized tomograph (CT) encompassing the lateral semicircular canal (LSC) was used to measure the size of the petrous bone in ears with bilateral (n = 13) and unilateral (n = 38) Menière's disease, and in ears with unilateral chronic otitis media (n = 25) as a control group. Measurements were made of the following 4 distances in the petrous bone; A: the distance between the posterior semicircular canal (PSC) and the posterior petrous surface (PPS) (P-P distance), B: the distance between the PSC and the LSC (P-L distance), C: the distance between the vestibu

Two cases of histiocytosis X in young adults with involvement of only the petrous bone are reported. In the first symptoms and signs consisted of a seventh nerve palsy of gradual onset over 3 years, and in the second, of pulsatile tinnitus associated with otorrhoea of clear fluid and impaired vestibular function. CT revealed large lytic lesions of the petrous bone. T1-weighted MRI before and after gadolinium confirmed the presence of a mixed tumour of the petrous bone with marked uptake of contrast medium. Spin echo images demonstrated the absence of extradural extension, and the patency of th

Cystic lesions in the petrous apex are nearly always either epidermoid cysts, with a soft but solid content, or cholesterol cysts, containing brown fluid. They may be symptomless, produce progressive cranial nerve palsies or, occasionally, reach the CSF pathways thus leading either to meningitis or a CSF leak. Thirteen cases and four unverified cases have been encountered amongst a series of more than 500 cerebellopontine angle tumours. Their clinical features, radiological diagnosis and difficulties of surgical approach are discussed.

Aneurysmal bone cysts (ABC) were originally described by Jaffe and Lichtenstein [Arch Surg 1942;44:1004-1025] in 1942 as nonneoplastic benign lesions with obscure pathogenesis. ABC occurring in the temporal bone are uncommon. Those arising from the petrous portion of the temporal bone are exceedingly rare. We report a right petrous ABC in a 16-year-old girl who presented clinically with hearing loss, facial weakness, and facial numbness and tingling. Her symptoms were preceded by a coincidental traumatic concussion 4 months earlier. Preoperative magnetic resonance imaging and computed tomograp

This study aimed to explore the surgical strategies and outcomes of petrosal bone cholesteatoma (PBC) surgery under ear endoscope. A retrospective analysis was conducted on the clinical data of PBC patients who underwent endoscopic surgery for PBC and were followed-up for≥1 year at West China Hospital, Sichuan University, from January 2017 to January 2024. The characteristics of the lesions, surgical procedures, and prognoses were evaluated. A total of seven patients were included, consisting of four males and three females, with 28-59 (36.4±11.0) years. According to the Sanna classification,

BACKGROUND: The term petrous bone cholesteatoma (PBC) refers to lesions extending deep to the bony labyrinth via superior, inferior, and posterior cell tracts. PBC is a rare incidence accounting for only 4-9% of petrous bone lesions. Lesions of petrous bone represent a real surgical challenge due to its complex relationship with critical neurovascular structures.
OBJECTIVE: To demonstrate our 40-plus years' experience in the management of PBC, depict the clinical features of PBC according to Sanna's Classification, evaluate the postoperative follow-up of surgically treated PBC patients, and d

OBJECTIVE: To raise awareness of the possibility of spontaneous temporal bone cerebrospinal fistula in case of clear retrotympanic effusion.
CASE REPORT: A 63-year-old man with no particular history presented with unilateral spontaneous right retrotympanic clear effusion. CT found defects in the posterior part of the right temporal bone, in contact with arachnoid granulations, with no other visible abnormalities.
DISCUSSION/CONCLUSION: Unilateral clear retrotympanic effusion in an adult subject should, apart from serous otitis media, suggest possible cerebrospinal fistula. In the absence of

A unique case of primary cholesteatoma of the petrous bone is described. The patient presented with a cutaneous fistula of the anterior cervical triangle and was initially diagnosed and treated as sebaceous cyst or imflammatory lympha node for 3 months. Later examination showed white mass in the right external auditory canal and a severe conductive hearing loss. Radiological findings revealed a massive soft tissue consistent with cholesteatoma in the right petrous bone. The lesion was successfully managed with an endoscopy-assisted technique leaving the neurovascular structures in situ without

Cellular schwannoma, a variant of benign schwannomas characterized by a high pseudosarcomatous cellularity, rarely involves the cranial nerves. In this report, we present the case of a 74-year-old woman with a cellular schwannoma of the VIIIth cranial nerve, which recurred from an ordinary schwannoma resected 9 years before. The tumor has been controlled for 35 months by a simple re-excision, indicating the benign nature of this tumor, although the tumor showed bone destruction and a high MiB-1 labeling ratio.

Direct measurements on tomograms of the petrous bone in Menière's disease revealed a decreased thickness of the retrolabyrinthine part of the petrous bone. This could help to confirm previous theories that the cause of Minière's disease is developmental.
