T-hex keratotomy surgically corrects manifest hyperopia. The typical patient presenting for this surgery is a presbyope who develops symptoms from latent hyperopia. These patients are older than typical radial keratotomy patients and are thus likely to develop senile cataracts sooner. The exact surgical approach to cataract surgery in such patients is unknown. We report a patient who had successful cataract extraction by phacoemulsification with implantation of a posterior chamber lens after a previous routine T-hex keratotomy. The preoperative workup and the surgical technique did not require

PURPOSE: Phacoemulsification has been shown to reduce intraocular pressure (IOP). The mechanism of action is thought to be via increased trabecular outflow facility. However, studies on the relationship between phacoemulsification and outflow facility have been inconsistent. This study intended to examine the change in electronic Schiotz tonographic outflow facility (TOF) and IOP measurements following phacoemulsification.
METHODS: Patients who were due to undergo a standard clear corneal incision phacoemulsification with intraocular lens (IOL) implantation, at St Thomas' Hospital, were invit

OBJECTIVE: To discuss how to choose the methods of capsularhexis and phacoemulsification in cases with mature or hypermature cataract.
METHODS: On fifty-nine mature or hypermature cataract patients, continuous circular capsularhexis or can-opener capsulotomy was performed. The nucleus was emulsified by the method of divide and conquer or phaco chop or rotatory carousel in the bag or at the plane of iris.
RESULTS: Continuous circular capsularhexis was successful in 51 cases, failed or shifted to can-opener in 8 cases. There were posterior capsular rupture in 1 cases, partial dehiscence of the

We describe a modified surgical technique of anterior capsulotomy and phacoemulsification for use in hypermature cataracts. Surgical steps included inferior linear capsulotomy with aspiration of milky cortex, inflation of the capsular sac with viscoelastic, a reverse triangular anterior capsulotomy with Vannas scissors, and in-the-bag bimanual sculpting of the nucleus. An oval 5 mm x 6 mm posterior chamber intraocular lens was used with single horizontal or no-stitch wound closure. The technique was used successfully in 20 cases of hypermature cataract. The mean time required to perform phacoe

In cases of an ectopic lens (e.g. Marfan Syndrome) with elongated zonular fibers, the lens can be partially emulsified. This can be done in such a way that it leaves behind a diaphragm consisting of zonular fibers and posterior capsule which is strong enough to support a posterior chamber lens. In cases of traumatically subluxated lenses intercapsular phacoemulsification can be done. The zonular dialysis can be fixed by suturing the capsule directly into the sulcus. The PCL can then be implanted either into the capsular bag or into the sulcus. Suture fixation of a posterior capsule is also pos

Charles D. Kelman was a brilliant American ophthalmologist who revolutionized cataract surgery by introducing phacoemulsification to replace extracapsular cataract extraction. He used an ultrasonic probe to emulsify and aspirate the lens through a small incision (3-4 mm). Kelman's technique met initial resistance at first, but it gained global acceptance after proving its safety and effectiveness in the management of cataractous eyes, and it has been the preferred technique until now. Today, the entire surgery is performed in 5-7 minutes. This technique also helped to reduce hospitalization af

BACKGROUND: A cataract is one of the underlying causes of blindness in animals. Phacoemulsification is the standard procedure in cataract surgery for humans and animals. This procedure has been used to restore vision in cataracts in a variety of animals. However, this technique is difficult in very small animals, such as rodents, due to their small eyes.
CASE DESCRIPTION: A 4-year-old male domestic chinchilla was presented with cloudiness in the lenses for 1 month. The ophthalmic examination revealed cataracts (oculus uterque: both eyes). Positive dazzle reflex oculus sinister; left eye (OS)

PURPOSE: To determine the immediate effect on corneal thickness of cataract extraction using phacoemulsification.
DESIGN: Prospective consecutive nonrandomized humans undergoing standard small-incision phacoemulsification and intraocular lens insertion.
METHODS: Corneal thickness was measured immediately preoperative and postoperatively in 58 consecutive patients undergoing phacoemulsification in the absence of other ocular abnormalities. Corneal thickness was determined centrally and in the midperiphery of four quadrants. The last 17 eyes were also evaluated for the effect of pressure from

A phacoemulsification procedure with implantation of a foldable acrylic intraocular lens in a 31-year-old man with spherophakia is described. The procedure was necessitated by anterior dislocation of the spherophakic lens, with corneal endothelial contact and development of central corneal edema. With a careful approach, the procedure was uneventful and the outcome successful. Modern small-incision cataract surgery techniques are of great benefit in this type of complicated case.

Endothelial cell counts, morphology, and 24-hour postoperative intraocular pressure (IOP) changes were measured in 55 consecutive patients (60 eyes) who had uncomplicated phacoemulsification and posterior chamber lens implantation with either aspirated (30 eyes) or retained (30 eyes) Viscoat (sodium chondroitin sulfate-sodium hyaluronate). The surgical technique was standardized in all cases. Endothelial cell loss was almost identical in the aspirated and in the retained groups (8.8% and 8.7%). Postoperative pleomorphism was significantly less in the retained Viscoat group than in the aspirate

BACKGROUND: To investigate whether phacoemulsification platform type and phaco tip design are associated with differences in unadjusted posterior capsular rupture (PCR) rates in cataract surgery.
METHODS: This retrospective observational study used centre-level data from UK National Health Service (NHS) cataract surgery centres included in the 2024 National Ophthalmology Database Cataract Audit, with a complementary analysis from two tertiary NHS centres. Phacoemulsification platform and principal tip design were identified by contacting each centre. Unadjusted PCR rates were compared using χ

Bimanual phacoemulsification has been established as a safe and efficacious means of nuclear emulsification in the past. This case report presents the use of this technique in a man in his early forties, who had subluxated cataractous lenses in both eyes. The technique allowed for effective nuclear emulsification within a closed chamber, providing better control over intraocular events. The report emphasizes the safety and effectiveness of bimanual phacoemulsification in cataract extraction, even in challenging cases like a subluxated lens.
