OBJECTIVE: Hypoparathyroidism is a known complication of total laryngectomy, although parathyroid preservation and/or reimplantation are not routine. Autofluorescence is a new technique for identifying parathyroid glands intraoperatively. The aim of this study was to evaluate the feasibility of autofluorescence in this context.
MATERIALS AND METHODS: A retrospective study of patients undergoing total laryngectomy/pharyngectomy with concomitant thyroidectomy using the Fluobeam® (Fluoptics, Grenoble, France) and frozen section of a parathyroid fragment in case of reimplantation. The rates of id

A retrospective study has been performed on 155 cases of surgically treated hypopharyngeal carcinomas in order to bring to light what effect the type of surgery has on survival and the reasons for failure. The following operations were performed: 103 hemipharyngectomies with total laryngectomy (HPTL); 28 total circular pharyngectomies with total laryngectomy (CPTL); 16 hemipharyngectomies with partial laryngectomy (HPPL); 4 exereses in lateral pharyngectomy; 2 total laryngectomies and 2 hemipharyngectomies in lateral pharyngotomy. Analysis of the results is only performed on the three most num

This report details the diagnosis and treatment of a patient with a large T3NoMo laryngopharyngeal cancer by combined surgical excision and planned postoperative irradiation therapy. Surgical excision included a horizontal subtotal supraglottic laryngectomy combined with a vertical hemilaryngectomy and partial pharyngectomy. The patient was reconstructed with a free sternocleidomastoid muscle graft. The results and implications of this therapy are discussed.

The principle, the technique and the usual uses of pectoralis major and latissimus dorsi myocutaneous flaps in head and neck oncology are called again. Three examples of unusual use are described: reconstruction of the tongue after total glossectomy with preservation of the larynx, mediastinal tracheostomy, pharyngo-esophageal reconstruction with muscle flap alone after total laryngo-pharyngectomy.

AIM: This study reports the results of surgical treatment of patients with carcinoma of the base of tongue-supraglottic complex. The base of the tongue, the epiglottic valleculae, preepiglottic space and supraglottic part of the larynx develop from one central region. The proper term to refer to these structures is tongue base-supraglottic complex. Carcinoma of this complex is a challenge for the surgeons working in the field of head and neck surgery.
METHODS: Patients with squamous-cell carcinoma of the tongue base present usually in advanced stage of the disease. The treatment includes surg

The authors describe a technique of entry into the vallecula with the guide of a small Deaver retractor inserted transorally, beyond the tongue base, into the vallecula. The tip of the Deaver directs the surgeon to the pharyngotomy site and allows pharyngeal entry and access without damaging structures in the region. This technique is particularly useful in total laryngectomy with or without pharyngectomy and is applicable whenever entry to the pharynx through the vallecula is indicated.

It is not uncommon for patients with hypopharyngeal cancer to present at an advanced stage of disease. Surgical treatment provides a cure for the tumour with immediate relief from obstruction to the airway and the swallowing passage. Careful planning of surgery is important to ensure good outcome of treatment and prevent complications, some of which may be fatal. The shape of the hypopharynx resembles that of a funnel, with a wide circumference above in continuity with the oropharynx, and a small circumference below where it joins with the cervical oesophagus. As a result, while small tumours

Background and Objectives: Total laryngectomy with partial pharyngectomy is traditionally the principal curative treatment for hypopharyngeal cancer; however, conservative surgical approaches that minimize functional disability are attracting increasing interest. Thus, we evaluated the appropriateness and oncological outcomes of open conservation surgery for such patients. Materials and Methods: We reviewed the medical records of 49 patients who underwent vertical hemipharyngolaryngectomy from 1998 to 2018 at a single institution. Results: Locoregional recurrences developed in 19 patients (38.

PURPOSE: Post-laryngectomy hypoparathyroidism is associated with significant short- and long-term morbidities. This systematic review aimed to determine incidence, risk factors, prevention and treatment of post-laryngectomy hypoparathyroidism.
METHODS: Medline, EMBASE and the Cochrane library were searched for relevant articles on hypocalcaemia and/or hypoparathyroidism after laryngectomy or pharyngectomy. Two authors independently screened titles and abstracts from the search. Data from individual studies were collated and presented (without meta-analysis). Quality assessment of included stu

OBJECTIVES: The current trend in managing laryngeal and hypopharyngeal cancer is to perform organ preservation therapy, which improves quality of life and decreases treatment-related morbidity. Transoral robotic surgery (TORS) can overcome the limit of "line of sight" often met in classic transoral procedure. We utilized a robotic surgical system to evaluate the feasibility and oncologic safety of transoral partial pharyngectomy and laryngectomy.
MATERIALS AND METHODS: TORS was performed by using a "da Vinci surgical robot" (Intuitive Surgical, Inc., Sunnyvale, CA). An FK retractor (Gyrus Med

The tracheoesophageal fistula (TOF) is defined as a pathological connection between the trachea and esophagus, both congenital and acquired. The latter is often associated with malignancies or their treatments. Reconstruction in this context is particularly challenging due to the defect's location, size, and compromised tissue quality. Here, we present for the first time the surgical treatment of TOF using a free superficial circumflex iliac artery perforator (SCIP) flap. Three patients were included in the present report (84-year-old female, 74-year-old male, 62-year-old male). Two patients d

Bicomponent cancer of the piriform fossa and larynx of 55 years old man has been described. The patient was treated surgically and by radiotherapy. The performance of laryngectomy and partial pharyngectomy with conservative bilateral neck dissection was used as surgical treatment. In histopathological findings of postoperative specimens two types of neoplasms: squamous cells and adenoid-cystic cancers were stated. The authors presented opinions on the affection arising of multicomponent tumours of the aerodigestive truct.

Two further indications for the use of a musculocutaneous flap from pectoralis major are reported: mucosal reconstruction after posterior pharyngectomy and vertical hemi-laryngopharyngectomy. This method was used in 7 cases, after previous surgery in 6 of these patients. Caution is necessary when interpreting carcinologic results because of the lack of long-term follow up results, but postoperative course was uncomplicated and tube removal and subnormal feeding was possible within 3 to 6 weeks.

We present a case of a 58-year-old man with a history of laryngo-pharyngectomy including bilateral thyroidectomy due to hypopharyngeal cancer presenting with lethargy, acute kidney failure, and hypercalcemia. Milk alkali syndrome was diagnosed given the history of high-dose calcium / vitamin D supplementation after ruling out other causes of hypercalcemia. After initial treatment with normal saline, furosemide and denosumab, the patient developed severe symptomatic hypocalcemia as a rare adverse effect of denosumab.

Transoral robotic surgery (TORS) is increasingly used in laryngeal/hypopharyngeal cancer surgery. Ablative procedures described in these anatomical sites include: (i) supraglottic laryngectomy, (ii) total laryngectomy, (iii) glottic cordectomy, and (iv) partial pharyngectomy. TORS supraglottic laryngectomy remains the most commonly performed of these procedures. Initial oncologic and functional outcomes with these procedures are promising and comparable to other treatment options. As robotic instrumentation technology advances a rise in TORS laryngeal/hypopharyngeal surgery is anticipated.

Pharyngeal liposarcomas are very rare; still more rare are dedifferentiated liposarcomas in the pharynx. An 82-year-old man presented with dysphagia, voice changes, weight loss, nasal regurgitation of liquids, and coughing spells. A 3.5 cm mass was identified in the hypopharynx. The mass was biopsied and diagnosed as a benign fibroepithelial polyp. Continued symptoms and airway obstruction prompted a pharyngectomy, and the mass was then diagnosed as dedifferentiated liposarcoma. Due to infrequency and subtle histological findings, liposarcomas of the head and neck can be misdiagnosed and recur

Hypopharyngeal stenosis is a frequent complication of laryngectomy and radiotherapy in patients treated for carcinoma. In a retrospective study of patients treated in the University of Washington Affiliated Hospitals, hypopharyngeal stenosis was more frequent after laryngectomy with partial pharyngectomy than after laryngectomy alone and occurred more commonly in lesions of the pyriform fossa than in glottic tumors. Neck disease and inclusion of radical neck dissection were both significant factors in the development of hypopharyngeal stenosis. Recurrent tumor was significantly more prevalent

BACKGROUND: Reconstruction of circumferential hypopharyngeal defects is often challenging. The aim of the study is to examine the result of our experience and to formulate improved management guidelines.
METHOD: Between 1980 and 2009, all patients who have circumferential pharyngectomy were recruited. Data were recorded prospectively for analysis.
RESULTS: A total of 202 patients were recruited. Majority had primary tumour in the hypopharynx (n=165), and the remaining patients had recurrent laryngeal carcinoma. Radiotherapy was given preoperatively in 72 patients and postoperatively in 108 p
