Euphorigenic drugs: effects on the reward pathways of the brain.

This report summarizes a number of experiments designed to examine the changes in the threshold for intracranial self-stimulation (ICSS) in the rat after the administration of morphine and a number of narcotic agonist-antagonists, as well as three nonnarcotic drugs that have extensive nonmedical use (cocaine, d-amphetamine, and phencyclidine). The results of these experiments clearly indicate that morphine lowers the threshold for ICSS and, furthermore, there appears to be little or no tolerance to this effect. The only mixed agonist-antagonist that consistently lowered the ICSS threshold was

Case report of a fatal 3-hydroxyphencyclidine intoxication, including blood and hair results.

3-Hydroxyphencyclidine (3-OH-PCP) is a hydroxy derivative of phencyclidine, synthesized in 1978 to investigate the structure-activity relationship of phencyclidine derivates. In vitro studies have shown that 3-OH-PCP, like phencyclidine, acts on the N-methyl-D-aspartate receptor and has a higher affinity for this receptor than phencyclidine. The authors report the case of a 38-year-old man, known for drug addiction, found dead at home with two plastic bags of powders found near his body. Using liquid chromatography coupled to tandem mass spectrometry, peripheral blood toxicological analysis re

Phencyclidine and chemical "stroking".

The current wave of drug abuse is but the latest manifestation of the real human need for "stroking." Drugs which affect the pleasure center of the brain are prime objects of abuse. Phencyclidine ("PCP" or "angel dust") is a cyclohexylamine which has properties of the amphetamine-stimulants, the narcotic-depressants and the hallucinogens. Previously unrecognized population groups include children less than six years of age and pregnant women and their intoxicated offspring. Successful treatment of present and future patients must be multidisciplinary and will ultimately depend upon a clear und

[Advances in research of ketamine addiction mechanism].

Ketamine is a phencyclidine derivative acting primarily as a noncompetitive antagonist of N-methyl-D-aspartate (NMDA) excitatory glutamate receptors. As a common intravenous anaesthetic in clinic, it is also increasingly abused because of its hallucination and addiction effects. Based on the pharmacological and toxicologic characteristics of ketamine and the acknowledged addiction mechanism of other abused drugs, this article reviews the possible addiction mechanism of the ketamine in the aspects of its enhanced effects and reward systems, the anatomic structures, the related receptors and the

Drugs of abuse in saliva: a review.

There has been substantial interest in the use of saliva as a diagnostic medium for drugs of abuse because it can be obtained noninvasively. Although drugs of abuse have been investigated in saliva for more than a decade, the role of saliva remains uncertain. A clear picture is difficult to obtain because of variations in (1) the analytical methods used; (2) the dose regimen of subjects, which was either unknown or differed between studies; and (3) the elapsed time between drug intake and sample collection. This communication summarizes the studies on the quantitative determination of differen

Extended EMIT-DAU phencyclidine screen.

In a public hospital emergency room, 580 urines were screened for phencyclidine (PCP) with the routine EMIT-DAU PCP screen, the extended EMIT-DAU PCP screen, and a gas chromatograph/mass spectrometer/computer (GC/MS/COMP) in selected ion mode, which was chosen as the reference method. The extended method produced a 38.5% increase in positives detected over the routine EMIT-DAU PCP screen and allowed 66.4% of the specimens to be signed out as negative without confirmation by GC/MS/COMP. This ability to provide a rapid, relatively inexpensive screen for PCP in urine and, in particular, to elimin

Improvement of phencyclidine-associated psychosis with ECT.

Phencyclidine-associated psychosis may mimic classic forms of both schizophrenia and affective psychosis. Treatment of phencyclidine-associated psychosis may prove very difficult for some patients. A patient who developed a severe phencyclidine-associated psychosis and failed to respond to high doses of antipsychotics is described. The patient responded dramatically to electroconvulsive therapy.

Stability of Drugs of Abuse in Biological Specimens.

Knowledge about the stability of drugs in biological specimens is critical to proper interpretation of test results. Literature pertaining to the stability of drugs of abuse in blood, urine, saliva, and tissue specimens is reviewed in this article. Ethanol-related information are also included as it is an abused drug and its stability in blood and urine has been the subject of many studies. Other drug classes for which literature data were reviewed are barbiturates, benzodiazepines, cannabinoids, cocaine and benzoylecgonine, lysergic acid diethylamide, and phencyclidine. PCP intoxication in seven young children.

Five infants and two young children were treated at a large children's hospital for phencyclidine intoxication. The clinical symptoms and signs were mostly neurologic, including diminished response to tactile and verbal stimuli (100%), ataxia (71%), nystagmus (57%), constricted pupils (57%), depressed sensorium, and stupor associated with a blank, expressionless stare (57%). Notably absent were the behavioral aberrations such as aggression, which are usually seen with PCP intoxication in older children and adults. The possibility of drug intoxication was denied by most of the parents or surrog

Phencyclidine overdose: an emerging concept of management.

Emergency physicians should be prepared to treat overdosage from phencyclidine, a common street drug abused throughout North America. A dearth of laboratory resources to establish the diagnosis makes it essential that diagnoses include awareness of symptomatology. A specific therapeutic approach utilizing ion-trapping by continuous gastric suctioning and acidification of urine and/or blood can be used successfully to treat these patients.

Radioimmunoassay for phencyclidine (PCP) in serum.

This accurate, sensitive radioimmunoassay for determining phencyclidine concentrations in serum specimens involves the use of anti-phencyclidine sera, 0.1 mL of serum specimen, an iodinated tracer, and a solid-phase separation. Phencyclidine metabolites do not show significant cross reactivity, but several phencyclidine analogs do cross react. Within-run coefficients of variation for human and dog serum ranged from 2.5 to 13% for concentrations from 2.0 to 500 micrograms/L. Day-to-day coefficients of variation for human and dog serum ranged from 4.3 to 16.7% for concentrations ranging from 2.0

Clinical issues associated with urine testing of substances of abuse.

Several factors may affect the validity and outcome of urine testing for abused drugs such as amphetamines, barbiturates, benzodiazepines, cannabinoids, cocaine, ethanol, opiates, and phencyclidine. Urine is used for large-scale testing because acquisition of the sample is noninvasive and because most abused drugs can be detected in urine for a reasonable duration after ingestion. Urine testing for drugs of abuse is a two-step process. In the first step, screening assays are used to identify presumably positive specimens. Common screening tests are radioimmunoassays, enzyme immunoassays, fluor

Substance abuse.

The potential for human developmental toxicity caused by chemical exposure was not generally recognized until the thalidomide tragedy of the early 1960s, at which time it was acknowledged that the placenta is not a barrier protecting the fetus. The focus of this chapter is to present the role of substance abuse screening in prenatal care, the gamut of drugs, and the clinical implications of the substances abused by pregnant women, as well as the legal and ethical considerations. Physiologic and care issues will be discussed. Legal substances discussed include the commonly used artificial sweet

Clinical implications of behavioral pharmacology research on phencyclidine.

This discussion has highlighted only some of the areas of behavioral pharmacology research with PCP, focusing largely on studies in our laboratories. Some of the areas touched upon lightly have been much more extensively investigated (e.g., PCP-like properties of psychotomimetic opioids). Some areas, such as the search for a PCP antagonist, have been studied with relatively little success so far. Two other areas, among many that are worthy of mention, are the extensive series of studies of the effects of PCP on complex learning procedures, starting with the studies by Moerschbaecher and Thomps

Evaluation of a rapid bedside toxicology screen in patients suspected of drug toxicity.

Although rapid bedside toxicological screening is reliable, it has not been validated in the emergency department (ED) setting. We assessed the accuracy of a 10-min bedside immunoassay, Triage Panel (TP), for 7 drugs of abuse and tricyclic antidepressants (TCA) in ED patients suspected of drug toxicity. This was a prospective observational study conducted at an urban teaching ED (100,000 visits/year) of patients suspected of drug toxicity during a 7-month period. The assay was compared for agreement with combined SYVA EMIT and TLC. GC/MS or HPLC was used for analyzing sensitivity and specifici

Novel psychoactive substance intoxication resulting in attempted murder.

A man in his twenties who had no previous history of violence, snorted large quantities of two substances he identified as 3-methoxyphencyclidine (3-MeO-PCP), and methylenedioxypyrovalerone (MDPV); both are recognised as novel psychoactive substances, or commonly described in the media as "legal highs". He also inhaled butane gas. He experienced vivid hallucinations and developed bizarre ideas. During this state of mind he stabbed his father multiple times and was arrested and charged with attempted murder. He had a previous history of drug induced psychosis and although he had some slight res
