Phenoxybenzamine in the treatment of hypoplastic left heart syndrome: a core review.

Perioperative management of neonates after the Norwood procedure is extremely complex. Limited reserve of the neonatal single ventricle and the parallel arrangement of the pulmonary and systemic circuits result in a tenuous balance between pulmonary and systemic blood flows. Precise manipulations of both pulmonary and systemic vascular resistance are necessary to prevent excessive pulmonary blood flow at the expense of systemic oxygen delivery. An emerging treatment strategy aimed at improving early mortality is the intraoperative administration of phenoxybenzamine, a profound systemic vasodil

Phenoxybenzamine is indicated in treatment of hypoplastic left heart syndrome: pro.

OBJECTIVE: The first aim of this study was to estimate the impact of anal sphincter laceration during the first delivery on the risk of recurrence in the second delivery. The second aim was to estimate the absolute risk of anal sphincter laceration in the second delivery according to the history of anal sphincter laceration and birth weight. METHODS: In this population-based cohort study, the study sample comprised all women included in the Norwegian Medical Birth Registry with 2 consecutive singleton vaginal deliveries during the period 1967-1998 (n = 486,463). The impact of prior anal sphin

Phenoxybenzamine in the management of neuropathic bladder following spinal cord injury.

BACKGROUND: The present study aims to show the clinical and urodynamic effects of phenoxybenzamine on the neuropathic bladder of spinal cord-injured patients who failed to be free of catheter by attaining satisfactory voiding function, despite initial bladder training. METHODS: Forty-six spinal cord-injured patients were subjected to pharmacological manipulation with phenoxybenzamine. It was used as an adjunct in the management of neuropathic bladder dysfunction that caused failure of the bladder to empty, by tapping or crede to achieve satisfactory residual urine volume of < 100 mL. Phenoxyb

Effect of an alpha-blocking agent (phenoxybenzamine) in the management of premature ejaculation.

Phenoxybenzamine has been extremely effective in treating patients with vesical dysfunctions, its minimal side effects include anejaculation and delay and difficulty in ejaculation. Fifteen men were treated with phenoxybenzamine (PBZ) for premature ejaculation. Eight patients reported a subjective improvement of the time from penetration to ejaculation. PBZ is a well tolerated drug. The only side effect of the treatment was dry ejaculation. The Authors suggest its use in selected patients.

Acceleration of beta-receptor desensitization in combined administration of antidepressants and phenoxybenzamine.

The delayed therapeutic effects of antidepressants (usually between 10 and 14 days) and of the tricyclic antidepressants in particular, are believed (on the basis of animal experiments) to lie in a progressive decrease of the sensitivity of cortical beta-adrenergic receptors. This is thought to be due to an increase in the synaptic concentration of noradrenaline, in turn accomplished by a decrease in the sensitivity of the presynaptic alpha 2 receptors which normally regulate noradrenaline secretion by a negative feedback mechanism. This model suggests that the desensitization of postsynaptic

Phenoxybenzamine in complex regional pain syndrome: potential role and novel mechanisms.

There is a relatively long history of the use of the α -adrenergic antagonist, phenoxybenzamine, for the treatment of complex regional pain syndrome (CRPS). One form of this syndrome, CRPS I, was originally termed reflex sympathetic dystrophy (RSD) because of an apparent dysregulation of the sympathetic nervous system in the region of an extremity that had been subjected to an injury or surgical procedure. The syndrome develops in the absence of any apparent continuation of the inciting trauma. Hallmarks of the condition are allodynia (pain perceived from a nonpainful stimulus) and hyperalgesi

Short-acting urapidil compared to long-acting phenoxybenzamine in the management of pheochromocytoma.

PURPOSE: In patients with pheochromocytoma current guidelines recommend preoperative alpha-adrenoceptor blockade with selective or nonselective antagonists for at least 7-14 days. To date, no information exists about orally administered urapidil retard, a short-acting selective antagonist. METHODS: The medical records of consecutive patients with pheochromocytoma between 2010 and 2023 were reviewed. Patients received phenoxybenzamine between 2010 and 2017, intravenous urapidil was given between 2017 and 2019. Orally administered urapidil retard has been used from 2019 until present. RESULTS:

A modern rationale for the use of phenoxybenzamine in urinary tract disorders and other conditions.

BACKGROUND: Phenoxybenzamine (PBZ) is a nonselective, irreversible alpha-adrenergic receptor antagonist that is approved for the treatment of diaphoresis and hypertension associated with pheochromocytoma. It may also be useful in several chronic conditions whose pathogenesis is mediated or affected by alpha-adrenergic stimulation, such as lower urinary tract symptoms associated with benign prostatic hyperplasia (BPH) and neurogenic bladder (eg, secondary to myelomeningocele and in sphincter dyssynergia and autonomic dysreflexia); in an adjunctive role after urogenital surgery or brachytherapy

[Personal experiences with phenoxybenzamine (Dibenzyran) in the treatment of functional disorders of bladder emptying].

The influence of phenoxybenzamine on micturition was examined in 46 patients with functional bladder neck obstruction. Due to blockade of the alpha-adrenergic receptors at the bladder neck an increase of the maximal flow rate from a mean value of 13.6 to 24.6 ml s-1 was found during the treatment. In addition, there was a relaxation of the functional urethral length and a reduction of the maximal urethral closure pressure from 96.3 to 74.4 cm H2O. The most significant changes resulting from the application of Dibenzyran were observed in the neuropathic bladder with a reduction of the functiona

Liver blood flow during haemorrhagic shock in the dog treated with phenoxybenzamine.

The liver blood flow has been extensively studied in hemorrhagic shock, but considerable disagreement exists as to the nature of hemodynamic changes and their controlling mechanism. The present investigation was undertaken in order to determine the effects of hemorrhage and phenoxybenzamine (PBZ) on the participation of hepatic artery (HAF) and portal vein flow (PVF) in total liver blood flow (LBF) changes. The dynamics of LBF (H2 washout method), HAF and PVF (electromagnetic flowmeter) during 3-hours posthemorrhagic hypotension (90 min. = 50-60 mmHg; 90 min. = 25-30 mmHg) and one-hour postret

The effect of phenoxybenzamine on postoperative urinary complications during extradural morphine analgesia.

One hundred and fifty patients, post-Caesarean section, were investigated to evaluate the effect of epidural morphine analgesia and that of phenoxybenzamine on the frequency and extent of urinary complications. Forty patients (group A) underwent Caesarian section under general anaesthesia, while 110 patients received epidural anaesthesia. Of the latter patients, 40 received postoperative mild analgesics (group B) whilst in another 40, postoperative continuous epidural morphine was administered (group C). Thirty patients who received postoperative epidural morphine, also received oral phenoxybe

Treatment with phenoxybenzamine of upper urinary tract complications caused by intravesical obstruction.

Five children in whom reflux and progressive hydronephrosis persisted despite multiple surgical attempts or repair are described. In all cases cystomanometry displayed a marked elevation of the bladder outlet resistance combined with high intravesical pressure values. Therapy with phenoxybenzamine, an alpha-adrenergic blocker, was successful in all cases, restoring a free urine passage of the upper urinary tract and unimpaired voiding preventing urinary diversion which has been considered in some of these children. Although there were no signs of bladder neuropathy, a hyperfunction of the symp

Phenoxybenzamine in neurogenic bladder dysfunction after spinal cord injury. I. Voiding dysfunction.

Phenoxybenzamine is a welcomed adjunct in the pharmacologic management of patients with neurogenic bladder dysfunction resulting from spinal cord trauma. Side effects are infrequent. Bladder balance was predictably and effectively achieved and maintained in patients with 1) lower motor neuron lesions, 2) upper motor neuron lesions and coexisting autonomic dysreflexia and 3) intact sympathetic innervation and upper motor neuron bladders. The response was unpredictable in patients with upper motor neuron lesions without coexisting dysreflexia.

Phenoxybenzamine in neurogenic bladder dysfunction after spinal cord injury. II. Autonomic dysreflexia.

Phenoxybenzamine blockade of the alpha receptor site can abolish the vascular and internal sphincter responses in patients with chronic recurrent autonomic dysreflexia associated with bladder filling or contraction. The life-threatening hypertensive response is suppressed by this medication. Therefore, the drug is a beneficial adjunct in the pharmacologic management of autonomic dysreflexia. When the drug is used patients can be maintained free of a catheter. The drug had no serious side effects at the dose regimen used. One particularly significant advantage is its long duration of action--pa

Drug-protein interactions: isolation and characterization of covalent adducts of phenoxybenzamine and calmodulin.

Phenoxybenzamine, an alpha-adrenergic antagonist containing a (chloroethyl)amine group, labels calmodulin in the presence of calcium. The covalent interaction is inhibited by chlorpromazine in a concentration-dependent manner. Adducts of calmodulin and phenoxybenzamine were separated by high-performance liquid chromatography into four major fractions: two containing 0.6 and 1.2 mol of drug per mol of protein and two different fractions each containing 2.0 mol/mol. Each adduct had a reduced ability to activate cyclic nucleotide phosphodiesterase and myosin light chain kinase, and the chlorproma

Types of neurogenic bladder in children with spina bifida, and response to treatment with phenoxybenzamine.

Seventeen children with spina bifida and neurogenic bladder were treated with phenoxybenzamine, an alpha-blocking agent, and in 15 cases the treatment was successful. It is suggested that in certain types of neurogenic bladder the need for urinary diversion can be avoided by treatment with phenoxybenzamine; only if this treatment fails does urinary diversion become necessary.

Secondary bladder neck obstruction in patients with urethral valves: Treatment with phenoxybenzamine.

Two cases are reported of functionally significant bladder neck obstruction following urethral valve fulguration which resulted in inability to void. Both patients responded to the administration of phenoxybenzamine, an alpha-adrenergic blocking agent. Treatment of patients with suspected bladder neck obstruction with this agent provides a reversible method of decreasing urethral resistance without the potentially harmful effects of surgical bladder neck revision.

Phenoxybenzamine reduces bladder outlet obstruction in benign prostatic hyperplasia. A urodynamic investigation.

Nine patients with bladder outlet obstruction attributable to benign prostatic hyperplasia were treated with 15 mg of phenoxybenzamine daily. Spontaneous urinary flow, water cystometry, simultaneous pressure-flow-EMG, and the urethral closure pressure profile were measured before and at the end of 4 weeks of treatment with phenoxybenzamine. A significant reduction in residual urine volume and a significant increase in the first sensation and bladder capacity were found. All voiding pressure parameters were significantly reduced and flow rates significantly increased. Urethral closure pressure
