Phrenic Nerve Stimulation for Acute Respiratory Failure.

Diaphragm inactivity during invasive mechanical ventilation leads to diaphragm atrophy and weakness, hemodynamic instability, and ventilatory heterogeneity. Absent respiratory drive and effort can, therefore, worsen injury to both lung and diaphragm and is a major cause of failure to wean. Phrenic nerve stimulation (PNS) can maintain controlled levels of diaphragm activity independent of intrinsic drive and as such may offer a promising approach to achieving lung and diaphragm protective ventilatory targets. Whereas PNS has an established role in the management of chronic respiratory failure,

Spontaneous Recovery Following Traumatic Phrenic Nerve Palsy.

This is a case report of unilateral traumatic phrenic nerve palsy with spontaneous recovery over a period of 31 months. This adds to the literature, demonstrating that extended conservative management can lead to successful resolution even in the setting of traumatic aetiology.

[Phrenic nerve stimulation in biventricular cardiac pacemakers].

BACKGROUND: Phrenic nerve stimulation (PNS) complicates the positioning of the left ventricle lead. We present a step-by-step approach to correct PNS during implantation, as established in our daily routine. METHODS: The incidence of PNS, its successful correction, and long-term results (median 27 months) were analyzed retrospectively in 266 lead positions. RESULTS: Phrenic nerve stimulation occurred in 13.9% of the lead positions. Multivariate analysis (P<0.02) showed that PNS only depended on the place of stimulation (coronary sinus side branch). Lead type, CRT indication, and patient's se

Insidious phrenic nerve involvement in postpolio syndrome.

A 49-year-old woman with amyotrophic sequelae of poliomyelitis experienced progressive left upper limb weakness and breathing discomfort while walking that had developed over one year prior to presentation. She had flaccid quadriplegia which was more marked in the left upper limb, with the C4- and C5-innervated muscles being most severely affected. Chest radiographs double exposed at maximal inspiration and expiration revealed poor respiratory movements in the left hemidiaphragm. Phrenic nerve conduction study demonstrated conspicuous diaphragmatic paralysis on the left side. The electrophysio

Comparison of the effects of mercuric chloride and methylmercuric chloride on the isolated phrenic nerve and on the phrenic nerve-diaphragm preparation from the rat.

This study compared the effects of HgCl(2) and CH(3)HgCl on the compound action potential of the isolated rat phrenic nerve and the twitch contractions of rat phrenic nerve-diaphragm preparations during direct and indirect stimulation. Both HgCl(2) and CH(3)HgCl, at a concentration of 3.7 x 10(-5)m, inhibited the twitches during indirect stimulation. CH(3)HgCl was slightly more potent than HgCl(2). The inhibition might be explained by a simultaneous inhibition of the phrenic nerve. CH(3)HgCl caused an additional curare-like inhibition at the neuromuscular junction. HgCl(2) potentiated the indi

Remedē Systems: Transvenous Pacing of the Phrenic Nerve.

The remedē System (ZOLL Medical, Minnetonka, MN; Fig 1), which was approved by the Food and Drug Administration in October of 2017, is a transvenous device that stimulates the phrenic nerve for the treatment of central sleep apnea, which is often associated with heart failure and atrial fibrillation. Given the similarity in implantation procedure to pacemakers and implantable cardioverter/defibrillators, the remedē System implantation often occurs in the electrophysiology laboratory. Despite the transvenous nature and close proximity to cardiac structures on radiographic imaging, the remedē Sy

Transvenous Phrenic Nerve Stimulation for Central Sleep Apnea.

RespiCardia remedē System (Minnetonka, MN 2017), a transvenous phrenic nerve stimulator, is indicated to treat central sleep apnea (CSA) by stimulating the phrenic nerve to cause diaphragmatic contraction to restore normal breathing during sleep. CSA is associated with decreased patient quality of life and worsens cardiovascular outcomes. Systematic review was conducted according to the Preferred Reporting of Systematic Reviews and Meta-Analysis guidelines. PubMed/MEDLINE, Cochrane, EBSCO CINAHL, and Ovid databases were queried by 2 independent reviewers for English-language studies published

Diaphragmatic paralysis after phrenic nerve injury in newborns.

BACKGROUND: Phrenic nerve injury (PNI) from birth trauma is a recognized phenomenon, generally occurring with ipsilateral brachial plexus palsy (BPP). In severe cases, PNI results in diaphragm paresis (DP) and respiratory insufficiency. Surgical diaphragmatic plication (SDP) is a potential management strategy for patients with PNI and DP, but timing and outcomes associated with SDP have not been rigorously studied. METHODS: Records from 49 tertiary United States pediatric hospitals in the Pediatric Health Information System from 2004 to 2018 were analyzed. The study cohort included patients d

Phrenic nerve involvement in Charcot-Marie-Tooth disease.

Diaphragmatic weakness in Charcot-Marie-Tooth (CMT) disease may be more common than generally acknowledged. Recognizing diaphragmatic weakness in a patient with CMT is important since recognition of this process may help explain the symptoms of breathlessness and orthopnea, as well as the development of hypercarbia. Although most patients with CMT have a normal lifespan, the subset of patients with diaphragmatic weakness may be at a higher risk of pulmonary morbidity and early mortality. Elevation of one or both hemidiaphragms on chest roentgenogram should raise the suspicion of diaphragmatic

Pulmonary function after complete unilateral phrenic nerve transection.

OBJECT: The status of pulmonary function following phrenic nerve transfer surgery is still largely unknown because of the high degree of variability in the accessory phrenic nerve that may be involved. In the present study, pulmonary functions were assessed in patients before and after full-length phrenic nerve transfer surgery, in whom the phrenic nerve was severed at a location just before its entry into the diaphragm. METHODS: Fifteen patients (average age 27.4 years) with complete brachial plexus palsy underwent full-length phrenic nerve transfer. The phrenic nerve was harvested from the

Transient phrenic nerve paralysis associated with status asthmaticus.

Phrenic nerve paralysis is a condition typically occurring after invasive procedures in the chest and neck. Here we describe a case of transient unilateral diaphragmatic paralysis in a child with status asthmaticus complicated by complete right lung atelectasis. Common causes of this disorder and possible implications for our case are discussed.

Repetitive phrenic nerve stimulation study in normal subjects.

In patients with myasthenia gravis (MG), it may be difficult to determine by clinical methods if respiratory insufficiency is due to a defect in neuromuscular transmission. We therefore studied the technique of repetitive electrical stimulation of the phrenic nerve in 6 healthy subjects. It was easily performed and quite reproducible. Responses at 3-Hz stimulation were recorded from surface electrodes of the ipsilateral hemidiaphragm, before and after exercise. We analyzed the percent decrement of the negative peak (NP) amplitude, area, and duration of the diaphragmatic compound muscle action

Phrenic nerve transfer for brachial plexus motor neurotization.

We report a series of 164 patients who underwent phrenic neurotization to elements of the brachial plexus with root avulsion injuries. Recipient nerves included musculocutaneous nerve in 125 patients (78 direct neurotizations and 48 with intervening autograft), median nerve in 10 patients, and a variety of other nerves in 28 patients. Sixty-five patients presented a follow-up period of 2 or more years. Of this group, 55 patients (84.6%) achieved a recovery of M-3 or better. We observed no long-term deleterious effects on respiratory function.

Is phrenic nerve-sparing worthwhile for phrenic-involved thymic epithelial tumors? A retrospective cohort study.

OBJECTIVES: To determine whether phrenic nerve (PN) sparing offers a worthwhile trade-off between functional preservation and oncologic safety compared with resection in PN-involved thymic epithelial tumors (TETs), and to establish an objective tool for postoperative respiratory risk stratification. METHODS: In this retrospective cohort of patients with TETs with PN invasion, 122 propensity score-matched cases were analyzed from 2016 to 2024. PN preservation or resection was determined intraoperatively based on the extent of invasion. The diaphragmatic elevation (DE) ratio was calculated as [

Neuropathy of the phrenic nerve associated with antiganglioside antibodies.

BACKGROUND AND PURPOSE: Antiganglioside antibodies have been implicated in several autoimmune-mediated neuropathies, and binding of these antibodies can result in inflammatory changes of the nerves. Diaphragmatic paralysis is a rare condition, mostly arising from diseases affecting the phrenic nerve, neuromuscular junction, or skeletal muscle. OBJECTIVES: In this case series, we identified five patients with diaphragmatic paralysis due to unilateral or bilateral neuropathy of the phrenic nerve associated with the presence of antiganglioside antibodies (immunoglobulin G anti-GT1a antibodies an

Is the preservation of the phrenic nerve important after pneumonectomy?

A best evidence topic in cardiac surgery was written according to a structured protocol. The question addressed was: is the preservation of the phrenic nerve important after pneumonectomy? Altogether more than 49 papers were found using the reported search, of which four represented the best evidence to answer the clinical question. The authors, journal, date and country of publication, patient group studied, study type, relevant outcomes and results of these papers are tabulated. We conclude that care should be taken to preserve the integrity of the phrenic nerve wherever possible. The abnorm
