Acute progressive neurological decline in an elderly man.

The patient was an 88-year-old man referred to a physical therapist by his primary care physician for a 1-week history of severe neck pain of insidious onset. Based upon the history and physical examination, the physical therapist concluded that the patient's neck pain was mechanical in nature. Initial physical therapist intervention included cervical taping, cervical collar use and instruction in home exercise. At his follow-up visit 4 days after his initial physical therapy visit, the patient reported no improvement. The patient's son, who accompanied him to this visit, also reported that hi

Asymptomatic carriage of Klebsiella pneumoniae producing extended-spectrum beta-lactamase by patients in a neurological early rehabilitation unit: management of an outbreak.

During 11 months 58 extended-spectrum beta-lactamase-producing Klebsiella pneumoniae (ESBL-Kp) isolates were grown from 10 patients on a neurological early rehabilitation unit. The patients had no signs of infection but were colonized in the nose and trachea, and unusually only one had colonization in the gut. A single clone of ESBL-Kp was identified by pulse field gel electrophoresis. Strong hygienic precautions similar to those for Methicillin-resistant Staphylococcus aureus patients prevented spread of the bacteria to other wards. However, rehabilitation for patients with severe neurologica

She also presented with multiple scars, skin grafts on the extremities, and a missing left lower leg, the causes for all of which were unexplained. The results of her blood test were normal, except for the hemoglobin level. Histology of the skin biopsy specim

Using Rehabilitation Therapies to Reduce Antipsychotic Use and ED Visits in Individuals With Dementia.

Alzheimer's disease and related dementias represent a significant and growing health care burden in the United States. This article describes the implementation of a rehabilitation-driven, interdisciplinary care model and its impact on antipsychotic medication use and emergency department visits among individuals with dementia in skilled nursing facilities. The approach integrates a research-based dementia staging protocol into occupational, physical, and speech therapies to create individualized care plans that leverage preserved function and the development of customized care strategies. Cervical Tuberculous Lymphadenitis in a Healthy Individual Following Latent Tuberculosis Treatment: A Case Report.

A 33-year-old healthy female, employed as a hospital-based physical therapist, was exposed to a colleague diagnosed with pulmonary tuberculosis. Screening revealed a positive interferon-gamma release assay (IGRA) result. She was diagnosed with latent tuberculosis infection (LTBI) and completed a 6-month isoniazid (INH) regimen. Nine months post-treatment, she presented to the otolaryngology department with right cervical swelling and ulceration. Polymerase chain reaction (PCR) of the pus confirmed Mycobacterium tuberculosis, establishing a diagnosis of cervical tuberculous lymphadenitis. She w

[Management of mucopolysaccharidosis type VI in adults].

Mucopolysaccharidosis type VI (Maroteaux-Lamy syndrome, MPS VI) is a progressive multisystemic lysosomal storage disease. Physical symptoms generally include growth retardation, and bone dysplasia. Enzyme replacement therapy is the treatment of choice and is done with recombinant version of enzyme N-acetylgalactosamine 4-sulfatase (galsulfase) which is administered intravenously. The enzyme replacement therapy should be applied once a week as a life-long treatment. Division of metabolic diseases, Department of internal medicine, University Hospital Center Zagreb continues with the treatment of

Inhospital management of common lumbosciatic syndrome. An opinion survey among French rheumatologists.

To determine which treatments are most commonly offered to patients younger than 70 years of age admitted for uncomplicated lumbosciatica suggestive of disk herniation, we sent a 43-item questionnaire to 106 rheumatology departments in France. The questionnaire collected information on hospital stay duration, modalities of rest, drug treatments, local glucocorticoid injections, physical treatments, sick leave duration and advice given to the patients. The response rate was 77% (84 questionnaires). Substantial variations were found across departments. Hospital stay duration ranged from five to

The effect of triheptanoin treatment on clinical and laboratory outcomes in patients with long-chain fatty acid oxidation disorder.

UNLABELLED: Long-chain fatty acid oxidation disorders (LC-FAOD) are a rare metabolic condition that results in impaired fatty acid utilization, leading to metabolic crises, hospitalization, and reduced quality of life. Despite dietary management, many patients experience ongoing complications. Triheptanoin, a seven-carbon triglyceride, has emerged as a therapeutic alternative by providing an energy source and supporting metabolic stability. This study aims to evaluate the clinical outcomes of LC-FAOD patients receiving triheptanoin therapy in Türkiye. A retrospective nationwide study was condu

Effectiveness of a structured physical therapist-led intervention compared to usual care in people with longstanding hip and groin pain referred to orthopaedic care in Sweden: protocol for a randomised controlled trial (the HIPSTER trial).

INTRODUCTION: People with longstanding hip and groin pain (LHGP) are often referred to orthopaedic care. Physical therapist-led interventions are recommended in consensus statements as the first line of treatment, but it is unknown if structured interventions are more effective than usual care. The aim of this trial is to evaluate the effectiveness of a structured physical therapist-led treatment model (HIPSTER) compared with usual care on hip-related quality of life at 4 months for people with LHGP referred to orthopaedic care. METHODS AND ANALYSIS: This is a preregistered (clinicaltrials.go
