Correct execution of hearing tests is essential for audiologic diagnostics and selection of treatment by the ENT physician. Especially in the case of pure-tone and speech audiometry, incorrectly performed audiometric masking can lead to false measurement results. Often, hearing that is too good is feigned by overhearing in the contralateral ear. Herein, a masking strategy is described by which errors in the execution of masking can be recognized and corrected. The aim of this paper is to identify and prevent the most common errors made during the masking process in pure-tone and speech audiome

BACKGROUND: The integration of medical professionals into management—so-called hybrid management—has emerged as a key strategy in public healthcare reforms. Clinical directorates (CDs) are often seen as vehicles for this integration. While prior studies have explored hybrid management roles and decentralization, less is known about how multiple hybrid-managerial levels interact within one organization. This study examines the perceptions of clinical directors in a large, decentralized healthcare provider in Sweden, following the introduction of a divisional hybrid-managerial level intended to

PURPOSE: To conduct the first of a series of pilot projects of the clinical competence assessment (CCA) of the Educational Commission for Foreign Medical Graduates (ECFMG) in order to provide profiles of clinical competencies of graduates of foreign medical schools for residency directors in the United States and for governments and institutions in other countries.
METHOD AND RESULTS: In September 1992 the first pilot project of the ECFMG CCA was conducted for a program director who wanted to evaluate ten first-year residents in a midwestern U.S. program. The CCA consists of integrated clinic

The involvement of physicians as an executive tool in the excesses of medical experimentation in prisons continues to raise questions and incomprehension. From the depths of the extermination camps under the Nazi regime to the iterative exploitation scandals after the Second World War, the ability of certain physicians to overstep the foundations of the Hippocratic Oath of «do no harm» and associate themselves with so many atrocities will always surprise. The answers to this question seem ambiguous because the excesses of medical experimentation in prisons should be reconsidered in their assoc

A 2023 workforce engagement survey of employees at the Houston Methodist Physician Organization (HMPO) identified a widening engagement gap between physician and nonphysician staff members. A greater number of physicians reported disengagement, mirroring a national trend in declining engagement scores, than other employees. Recognizing that low levels of physician engagement can negatively impact physician retention, patient care, and organizational success, in 2024, HMPO launched a focused Joy in Medicine Initiative (JIMI) aimed at bridging the engagement gap. The JIMI workplan, led by the Of

Idiopathic clubfoot affects approximately 100,000 children each year and is one of the leading causes of disability worldwide. The Ponseti method is an inexpensive, non-surgical treatment that, when executed correctly, is more than 95% effective; however, in Peru, a developing country where a low-cost alternative is greatly needed, physicians report up to 30% of patients do not complete treatment. This study involved semi-structured interviews with 25 physicians who practice the Ponseti Method in Peru to discuss obstacles for the method in their country. The most frequent obstacles to the Pons

Part-time practice--practicing fewer clinical work hours than one's HMO norm--is common at Harvard Community Health Plan. Physicians choose part-time practice for diverse reasons: to allow time for other professional activities, such as management or teaching, to facilitate their lives as working mothers, or to ease their work lives. A great potential for difficulties with practice management exists when physician colleagues who devote different proportions of their time to clinical practice collaborate. This can be resolved by a facilitated approach that allows team members to identify proble

Recognition that human immunodeficiency virus could be transmitted by blood transfusion has brought the practice of transfusion medicine into the public consciousness with great force. Patients are informed about the dangers of receiving a blood transfusion and are expressing their preferences about the source of the blood they may receive and the conditions under which they may receive it. Government, blood bank organizations, and physicians are responding to the public demand for a safe blood supply. As a result, profound changes are underway in hospital blood donor services, transfusion ser

BACKGROUND: Concurrent with the evolving role of the department chair in academic medicine is the entry of women physicians into chair positions. Because implicit biases that stereotypically masculine behaviors are required for effective leadership remain strong, examining faculty members' perceptions of their chair's leadership in medical school departments with women chairs can provide insight into the views of women leaders in academic medicine and the complex ways in which gender may impact these chairs' leadership style and actions.
METHODS: We conducted semistructured interviews with 13

This Note argues that prisoners, whether executed or living, should not become organ donors. The introduction acknowledges the shortage of transplantable organs in the United States and the steps that have been taken to ameliorate the crisis. Part I discusses the procurement of organs from executed prisoners, beginning with a brief examination of China, a country where this type of procurement is routinely practiced. Part I also examines organ procurement legislation pertaining to executed prisoners. Finally, Part I asserts the reasons that prisoners should not become donors, including the dea

The state of Florida prepares for hurricanes annually, particularly from late summer to fall. These hurricanes put immense pressure on public services, especially emergency medical services (EMS). This paper highlights the critical role of the EMS medical director in Sarasota, Florida, within the community during disaster response. It outlines the medical director's responsibilities, situations, protocols, and limitations encountered during Hurricane Milton. Specifically, this paper will illustrate a series of cases and their responses during periods of severe winds, which forced 9-1-1 service

The basic definitions of obstructive sleep apnea (OSA), its epidemiology, its clinical features and complications, and the morbidity and mortality of OSA are discussed. Included in this treatise is a discussion of the various symptomatic and polysomnographic phenotypes of COPD that may enable better treatment and impact mortality in persons with OSA. The goal of this article is to serve as a reference for life and disability insurance company medical directors and underwriters when underwriting an applicant with probable or diagnosed sleep apnea. It is well-referenced (133 ref.) allowing for m

This article provides an executive summary of the 2019 Guideline for Management of Wounds in Patients with Lower-Extremity Venous Disease (LEVD) published by the Wound, Ostomy and Continence Nurses Society (WOCN). The executive summary presents an overview of the systematic process used to update and develop the guideline. It also lists the specific recommendations from the guideline for assessment, prevention, and management of LEVD and venous leg ulcers (VLUs). In addition, the guideline includes a new section regarding implementation of clinical practice guidelines. The LEVD guideline is a

This article discusses the diagnosis and management of toxoplasmosis. Congenital toxoplasmosis continues to be a tragic outcome of a preventable and treatable infection. Education of patients, physicians and health policy makers on the primary and secondary preventive measures of the disease, and their execution, will undoubtedly result in lower incidence, morbidity, and mortality rates from congenital disease due to Toxoplasma gondii.

The term twice-exceptional (2e) describes individuals who combine high intellectual potential with neurodevelopmental or psychiatric conditions that may impair performance in traditional environments. While this concept has been increasingly recognized in education, its relevance to medical training and practice has received little attention. This case series presents five physicians across different stages of training who exemplify twice-exceptionality, highlighting both their cognitive strengths and the barriers they faced in clinical environments. The first case involves a surgical resident
