A novel in vitro model of villitis of unknown etiology demonstrates altered placental hormone and cytokine profile.

PROBLEM: Placental dysfunction is present over 50% of cases of stillbirth and fetal growth restriction (FGR). Villitis of unknown etiology (VUE), an inflammatory condition of the placenta characterized by maternal T cell infiltrates in the villous stroma and dysregulation of inflammatory cytokines, is more frequent in FGR and stillbirth. METHOD OF STUDY: A novel in vitro model of placental inflammation was developed to test the hypothesis that inflammatory cells seen in VUE and/or cytokines impair placental function. RESULTS: Coculture of placental explants with maternal leukocytes resulted

Plasma angiotensin I and serum placental leucine aminopeptidase (P-LAP) in pre-eclampsia.

A study was undertaken on serial measurements of plasma angiotensin I (A-I) and serum placental leucine aminopeptidase (P-LAP) activities in normal and pre-eclamptic pregnancy. There was the significant difference in A-I levels between normal and mild pre-eclamptic pregnancy at weeks 30, 35, 37, between normal and severe pre-eclamptic pregnancy at week 37. There were no differences in serum P-LAP between normal and mild pre-eclamptic pregnancy up to week 33, but thereafter the levels for the mild pre-eclampsia were significantly higher than for the normal pregnancy. The P-LAP activity for the

Isorhynchophylline Inhibits NLRP3 Inflammasome and Improves Gestational Diabetes.

This study aims to investigate the effects and underlying mechanisms of isorhynchophylline (IRN) on gestational diabetes mellitus (GDM). The db/+ mice were randomly divided into four groups: GDM, GDM + IRN (20 mg/kg), and GDM + IRN (40 mg/kg). Blood glucose and insulin tolerance were assessed using intraperitoneal glucose tolerance tests (IPGTTs) and intraperitoneal insulin tolerance tests (IPITTs) on gestational day 10. On gestational day 20, placental inflammation (tumor necrosis factor [TNF]-α, interleukin [IL]-6, IL-1β), oxidative stress markers (malondialdehyde [MDA], SOD, glutathione per

Prenatal imaging features suggestive of liver gestational allo immune disease.

We report prenatal imaging features of four cases of neonatal hemochromatosis due to an alloimmune disease. All cases exhibited intra uterine growth restriction (IUGR) without arguments for a vascular etiology, associated with oligohydramnios. Placental hydrops was present in 75% of cases. Splenomegaly was identified in one case. Other causes of NH have been ruled out during diagnostic workup including karyotype, detection of IGFBP-1 to evaluate a premature rupture of membranes, maternal serologic tests. MRI was performed in two cases and showed an atrophic liver associated with a low signal i

Is the placental disk really an ellipse?

The mean surface shape of placenta is round and common abnormalities of shape are associated with vascular abnormalities and reduced placental functional efficiency. A long-standing approach is to describe shapes as elliptic, and to quantify them by "length" and "breadth". We test this description in two cohorts: National Collaborative Perinatal Project and Pregnancy, Infection and Nutrition Study. We conclude that quantifying placental shape as elliptic is ambiguous and problematic. The "breadth" of the placenta should be interpreted as a combination of two different measurements: placental s

Novel use of proton magnetic resonance spectroscopy (1HMRS) to non-invasively assess placental metabolism.

BACKGROUND: Placental insufficiency is a major cause of antepartum stillbirth and fetal growth restriction (FGR). In affected pregnancies, delivery is expedited when the risks of ongoing pregnancy outweigh those of prematurity. Current tests are unable to assess placental function and determine optimal timing for delivery. An accurate, non-invasive test that clearly defines the failing placenta would address a major unmet clinical need. Proton magnetic resonance spectroscopy ((1)H MRS) can be used to assess the metabolic profile of tissue in-vivo. In FGR pregnancies, a reduction in N-acetylasp

Wharton's jelly area and its association with placental morphometry and pathology.

INTRODUCTION: Wharton's jelly (WJ) is the mucoid connective tissue that surrounds the vessels in the human umbilical cord and provides protection from compression and torsion in response to fetal movement. WJ is known to be altered in the presence of pregnancy complications such as gestational diabetes mellitus and preeclampsia. The present study examined associations between the cross-sectional area of WJ measured by ultrasound and postpartum placental pathology and morphometry. METHODS: The area of WJ was measured by ultrasound in 156 eligible participants between 23 and 37 weeks' gestation

[Relations between the oxytocin test and morphometry of the placenta].

We have studied placental morphometry after delivery in 23 cases in which the oxytocin challenge test was carried out towards the end of the pregnancy. Thus we have been able to compare the results of the two and establish details of placental measurement that have the greatest influence on its function. The following parameters seem to have the most direct influence, by statistical relationship, with placental insufficiency: the total surface of the villi, the concentration of villi per unit sectioned, the surface of fetal capillaries and the mean diameter of these capillaries. On the other h

T2*-Relaxometry MRI to Assess Third Trimester Placental and Fetal Brain Oxygenation and Placental Characteristics in Healthy Fetuses and Fetuses With Congenital Heart Disease.

BACKGROUND: Congenital heart disease (CHD) has been linked to impaired placental and fetal brain development. Assessing the placenta and fetal brain in parallel may help further our understanding of the relationship between development of these organs. HYPOTHESIS: 1) Placental and fetal brain oxygenation are correlated, 2) oxygenation in these organs is reduced in CHD compared to healthy controls, and 3) placental structure is altered in CHD. STUDY TYPE: Retrospective case-control. POPULATION: Fifty-one human fetuses with CHD (32 male; median [IQR] gestational age [GA] = 32.0 [30.9-32.9] we

Placental pathology and fetal growth outcomes in pregnancies complicated by maternal obesity.

BACKGROUND: The rising prevalence of maternal obesity presents a significant health concern because of the possible implications for obstetric complications and neonatal outcomes. Understanding the impact of obesity on placental structure and function as well as fetal growth and infant outcomes is important to improve the care of these potentially high-risk pregnancies. This study aimed to determine the effect of elevated maternal BMI on histopathologic patterns of placental injury and its consequences on fetal growth. METHODS: Data were collected from an ongoing cohort of maternal-infant dya

Placental characteristics and reduced risk of maternal breast cancer.

BACKGROUND: Women who have preeclampsia during pregnancy are at reduced risk of subsequent breast cancer. We examined whether other markers of reduced placental size or function, including increased blood pressure during pregnancy, predict a reduction in maternal breast cancer. METHODS: The Child Health and Development Studies is a 40-year follow-up of pregnant women enrolled in the Kaiser Permanente health plan between 1959 and 1967. We identified 3804 white women for whom data were available on placental examinations and other study variables. As of 1997, 146 women had developed invasive br

Towards early diagnosis of Preeclampsia: Emerging roles of circulating microRNAs.

Preeclampsia (PE) is a pregnancy-specific hypertensive disorder and a major contributor to maternal and fetal morbidity and mortality worldwide. Its pathophysiology involves placental dysfunction, immune dysregulation, heightened inflammatory activation, imbalance between angiogenic and antiangiogenic factors, and metabolic alterations. Despite extensive research, early diagnosis of PE remains challenging, as current approaches rely on specialized professionals, costly biochemical tests, and advanced equipment, which are not always widely accessible. Emerging evidence indicates that microRNAs

Placental Features of Late-Onset Adverse Pregnancy Outcome.

OBJECTIVE: Currently, no investigations reliably identify placental dysfunction in late pregnancy. To facilitate the development of such investigations we aimed to identify placental features that differ between normal and adverse outcome in late pregnancy in a group of pregnancies with reduced fetal movement. METHODS: Following third trimester presentation with reduced fetal movement (N = 100), placental structure ex vivo was measured. Placental function was then assessed in terms of (i) chorionic plate artery agonist responses and length-tension characteristics using wire myography and (ii)

Oxidative stress and endothelial function in normal pregnancy versus pre-eclampsia, a combined longitudinal and case control study.

BACKGROUND: Pre-eclampsia (PE) is related to an impaired endothelial function. Endothelial dysfunction accounts for altered vascular reactivity, activation of the coagulation cascade and loss of vascular integrity. Impaired endothelial function originates from production of inflammatory and cytotoxic factors by the ischemic placenta and results in systemic oxidative stress (OS) and an altered bioavailability of nitric oxide (•NO). The free radical •NO, is an endogenous endothelium-derived relaxing factor influencing endothelial function. In placental circulation, endothelial release of •NO dil

Subsequent physical and mental development in infants with positive contraction stress tests.

Little knowledge exists concerning long-term sequelae in children born of pregnancies complicated by chronic uteroplacental insufficiency. This follow-up study evaluates 12 children with positive contraction stress tests (CSTs) 5.5 months to 4.75 years following birth. Subsequent physical growth was uniformly normal in the four infants classified as appropriate for gestational age at birth; however, continued postnatal growth deficiency was present in 50% of the eight infants who suffered from intrauterine growth retardation. No major neurologic abnormalities were detected, and 10 of the 12 di

Is heparin a placental anticoagulant in high-risk pregnancies?

Randomized control trials show beneficial effects of heparin in high-risk pregnancies to prevent preeclampsia and intrauterine growth restriction. However, the lack of placental pathology data in these trials challenges the assumption that heparin is a placental anticoagulant. Recent data show that placental infarction is probably associated with abnormalities in development of the placenta, characterized by poor maternal perfusion and an abnormal villous trophoblast compartment in contact with maternal blood, than with maternal thrombophilia. At-risk pregnancies may therefore be predicted by

Plasma estrogen response to dehydroepiandrosterone sulphate injection in normal and complicated late pregnancy.

Dehydroepiandrosterone sulphate (DHAS) was administered intravenously to 70 women in the third trimester of pregnancy. By measuring the increase in the concentrations of estrogens in maternal plasma, it was hoped to assess the reserve capacity of the placenta. In normal pregnancies, unconjugated estradiol invariably showed a marked increase within 60 minutes of the injection of DHAS, reaching 174 to 478% above basal levels. Changes in unconjugated estrone were more variable, but significant increases usually occurred 2 to 3 hours after the peak concentration of estradiol. The concentrations of

[Antigens of the human placenta (author's transl)].

The immunologic relations between mother, foetus and placenta are discussed and those antigens of the human placenta already characterized by their physico-chemical and immunochemical properties described. In diagnostics the detection and determination of placental proteins is used in pregnancy tests, for the evaluation of placental functions and the detection of malignant diseases. Antibodies to placental antigens have an abortive effect; immunization against placental specific proteins may have implications as an immunologic contraceptive method and for the immunotherapy of choriocarcinoma.

[Evaluation in the influence of pregnancy induced hypertension on the growth and development of fetus and infant].

170 cases of premature with birth weight less than 2,000 g were analysed. The results demonstrated that the incidence of small for gestational age infant (SGA) was significantly higher in women associated with pregnancy induced hypertension (PIH) than that in normal pregnancy. As compared with appropriate for gestational age infant (AGA), SGA group showed higher perinatal mortality and apparent retardation in physical and mental development as well. The significance of difference between infant groups was correlated with percentile of SGA other than the gestational age. Therefore monitoring pl
