Primary leiomyoma of the pleura is extremely rare. A 45-year-old man presented with a complaint of right chest pain. Chest computed tomography demonstrated a solid, round pleural mass in the right anterior chest wall. The mass was completely resected, and histopathological examination revealed a localized primary pleural leiomyoma. The patient was followed and has been disease-free for over 15 months. This is the first report of primary leiomyoma of the pleura in China. A review of the literature on primary leiomyoma of the pleura is presented.

Castleman's disease is a rare benign disease of the lymph nodes. Its origin from the pleura is rare. Surgical excision, when feasible, appears to provide good results. We encountered a patient who had the disease arising from the parietal pleura. We present the clinical scenario, investigations, and our management of the patient.

Malignant mesenchymomas are rare soft tissue tumors of mesenchymal origin. We report a case of pleural localization with liposarcomatous, leiomyosarcomatous and osteocartilaginous elements. The treatment associated surgical resection, chemotherapy and radiotherapy. Sixteen months after the diagnosis a metastatic retroperitoneal sarcoma with osteosarcomatous differentiation appeared without any sign of thoracic recurrence. A surgical resection was performed, but a rapid retroperitoneal recurrence led to death.

We report two cases of small pleural nodules showing the distinctive histologic appearance of adenomatoid tumor. Both lesions were discovered incidentally during surgery in patients undergoing lung resection for unrelated intrapulmonary masses: lung carcinoma in one case and histoplasmosis in the other. A total of 171 pleural needle biopsies were carried out in 123 patients with pleural effusion. The sensitivity of the method proved to be 51% for malignant diseases and 75% for tuberculotic pleurisy, respectively. Performing biopsy simultaneously with the cytological examination of the pleural fluid, malignancy was proved only by needle biopsy in 20% of the patients, while cytology was found negative in these cases. Simultaneously performed thoracocentesis and pleural needle biopsy is suggested.

A case is presented of 41 year old patient with a very rare form of clinical manifestation of adenocarcinoma with a large mucinous ascites with abdominal distention and a massive left hemithoracic content with same characteristics. Two years prior the patient suffered from acute abdomen and underwent apendectomy with slow wound healing and a stomach edema 6 months later with a finding of smaller changes on the left side of the thorax. This heterogeneous group of immunologic causes of pleural effusions includes connective tissue diseases, a syndrome related to tissue injury (postcardiac injury syndrome), a disease of unknown etiology (sarcoidosis), a malignancy, and adverse reactions to drugs. Except in rheumatoid pleurisy and lupus pleuritis, the pleural fluid findings are either nonspecific or have not been characterized fully. The clinical presentation is essential in the presumptive diagnosis of these causes of pleural effusions.
