Primary Localised Endobronchial Amyloidosis Mimicking Aspiration Pneumonia.

A female in her 70s originally presented with a mechanical fall and a consolidation on a chest x-ray, thought to be secondary to aspiration. Failure to response to medical treatment prompted further investigations which revealed a calcific lesion obstructing the left lower lobe bronchus, histologically confirmed as endobronchial amyloidosis. Amyloidosis is a rare condition characterized by insoluble fibril protein deposition in various organs. Pulmonary amyloidosis can be a manifestation of systemic amyloid disease, or otherwise being isolated in nature. Tracheobronchial amyloidosis is a subty

Descriptive Analysis of Combat-Associated Aspiration Pneumonia.

BACKGROUND: Airway obstruction is the second leading cause of potentially preventable death on the battlefield. The treatment for airway obstruction is intubation or advanced airway adjunct, which has a known risk of aspiration. We sought to describe the variables associated with aspiration pneumonia after prehospital airway intervention. METHODS: This is a sub-analysis of previously described data from the Department of Defense Trauma Registry (DoDTR) from 2007 to 2020. We included casualties that had at least one prehospital airway intervention with documentation of subsequent aspiration pn

Aspiration pneumonia: a review of modern trends.

PURPOSE: The purpose was to describe aspiration pneumonia in the context of other lung infections and aspiration syndromes and to distinguish between the main scenarios commonly implied when the terms aspiration or aspiration pneumonia are used. Finally, we aim to summarize current evidence surrounding the diagnosis, microbiology, treatment, risks, and prevention of aspiration pneumonia. MATERIALS AND METHODS: Medline was searched from inception to November 2013. All descriptive or experimental studies that added to the understanding of aspiration pneumonia were reviewed. All studies that pro

Dysphagia and aspiration pneumonia in older adults.

PURPOSE: To review the risks, diagnosis, management, and prevention of aspiration pneumonia in adults over age 65, and discuss the major role of dysphagia as a risk factor in this population. DATA SOURCES: A comprehensive search of the literature using PubMed (MedLine) and CINAHL. CONCLUSIONS: Aspiration pneumonia is a common diagnosis in older adults and is associated with significant morbidity and mortality. Oropharyngeal and esophageal dysphagia, often related to stroke, dementia, poor oral hygiene, or multiple chronic illnesses, increases the risk of aspiration. Nurse practitioners (NPs)

Olive oil aspiration pneumonia (lipoid) in children.

In the Asir region of south-western Saudi Arabia, nasal instillation of olive oil to infants and children in the recumbent position is practised to relieve nasal congestion. Aspiration of olive oil results in lipoid pneumonia resistant to antimicrobial treatment. A series of 5 children, aged 4-72 months, with olive oil-induced lipoid pneumonia is presented. Clinical presentation included persistent coughing, tachypnoea, recurrent febrile illness and chest infections. The pulmonary radiological picture was mainly right middle lobar and perihilar infiltrates. Bronchial lavage and microscopic exa

Aspiration pneumonia due to diffuse cervical hyperostosis.

Diffuse idiopathic skeletal hyperostosis previously has been reported to cause a number of extraspinal manifestations including dysphagia, respiratory distress, dysphonia and cervical myelopathy. We report a case of cervical DISH so extensive as to interfere with the swallowing mechanism and lead to aspiration. Patients with DISH who have mechanical compression of the posterior pharynx may be at high risk for aspiration.

Aspiration pneumonia, anaerobic infections, and lung abscess.

Anaerobic pleuropulmonary infections are common in the setting of aspiration. Treatment is with penicillin or in mixed infections, penicillin and other appropriate antimicrobial agents. Four syndromes are commonly recognized. Simple pneumonitis resolves promptly with antibiotic therapy. If treatment is delayed, simple pneumonitis may evolve into a more fulminant process of necrotizing pneumonia with a significant mortality despite antibiotic therapy. Lung abscess, typically a subacute or chronic infection, responds well to antibiotic therapy so long as the cavity is drained via the communicati

The nasogastric feeding tube as a risk factor for aspiration and aspiration pneumonia.

PURPOSE OF REVIEW: Aspiration is one of the most common complications in enterally fed patients. The source of aspiration is due to the accumulation of secretions in the pharynx of reflux gastric contents from the stomach into the pharynx. The true prevalence of aspiration is difficult to determine because of vague definitions, poor assessment methods, and varying levels of clinical recognition. RECENT FINDINGS: There is evidence in the literature showing that the presence of a nasogastric feeding tube is associated with colonization and aspiration of pharyngeal secretions and gastric content

[A case of recurrent aspiration pneumonia by achalasia].

A 56-year-old woman came to our hospital with the symptoms of anorexia, body weight loss and sustained cough. Chest radiography showed diffuse, rounded, high-attenuation areas in both lung fields. The diagnosis was difficult, but, because of the symptoms and chest radiograph, we suspected miliary tuberculosis. Finally, we diagnosed her illness as achalasia with aspiration pneumonia, because we found a dilated esophagus and diffuse, rounded, high attenuation areas in chest CT scan films. Neither Mycobacterium tuberculosis nor tuberculous granulation was present in transbronchial lung biopsy spe

Aspiration pneumonia in nursing literature-a mapping review.

INTRODUCTION: Aspiration pneumonia (AP) is an infection of the lungs caused by inhalation of material. The reported incidences vary across literature and clinical populations and is associated with high morbidity and mortality. Management of AP is best carried out by a multidisciplinary team. METHODS: This aim of this review was to collate and describe the available evidence on AP to develop a greater understanding of the concept of AP as it is represented in the nursing literature. As a collaborative team, we undertook the six stages of a systematic mapping review. We searched for the term a

Recurrent aspiration pneumonia precipitated by obstructive sleep apnea.

OBJECTIVE: The clearance of the pharynx by deglutition and the respiratory phase patterns associated with deglutition are important in protecting airways and lungs against aspiration. The deglutition and respiratory phase patterns during sleep in patients (without swallowing disorders while awake) with obstructive sleep apnea (OSA) precipitating recurrent intractable aspiration pneumonia were investigated. METHODS: After videoendoscopic and videofluorographic examinations of swallowing showed subjects had no swallowing disorders while awake, two adults with recurrent intractable aspiration pn

Herpes Simplex Virus Pneumonia: Importance of Aspiration Etiology.

Herpes simplex virus (HSV) pneumonia caused by aspiration from the oropharyngeal exudates is described. An 89-year-old Japanese male in a poor performance state complained of appetite loss followed by difficulty in swallowing, and bilateral pulmonary infiltrates with interstitial reactions were radiologically pointed out. Antibiotics administration was ineffective, and he died on the 6th day of hospitalization. At autopsy, HSV-induced multiple mucosal erosions were observed on the tongue, pharynx, epiglottis, and trachea. In bilateral lower lobes of the lung, HSV infected bronchiolar and type-

Risk Factors for Aspiration Pneumonia in Older Adults.

BACKGROUNDS: Aspiration pneumonia is a dominant form of community-acquired and healthcare-associated pneumonia, and a leading cause of death among ageing populations. However, the risk factors for developing aspiration pneumonia in older adults have not been fully evaluated. The purpose of the present study was to determine the risk factors for aspiration pneumonia among the elderly. METHODOLOGY AND PRINCIPAL FINDINGS: We conducted an observational study using data from a nationwide survey of geriatric medical and nursing center in Japan. The study subjects included 9930 patients (median age:
