[Poliomyelitis in literature, cinema and television].

INTRODUCTION: Poliomyelitis is an infectious disease whose initial symptoms are fever, fatigue, headaches, vomiting, a stiff neck and pains in the limbs. In many cases, the sequelae are irreversible paralysis and may result in death if there is bulbar or respiratory compromise. A set of symptoms, called post-polio syndrome, which appears years after the acute infection, are also described. AIM: To analyse the way poliomyelitis has been dealt with in literature, cinema and television. DEVELOPMENT: Film and television writers and directors have shown an interest in poliomyelitis and have portr

Vocal cord paresis and glottic stenosis: a late complication of poliomyelitis.

A patient with spinobulbar poliomyelitis had residual dysfunction of the ninth and tenth cranial nerves, which produced bilateral vocal cord paresis and recurrent aspiration. Critical glottic stenosis developed 28 years after the initial episode of poliomyelitis; this course appeared to be explained by fibrosis of the intrinsic laryngeal muscles and ankylosis of the right cricoarytenoid joint. Thus it appears that significant upper airway obstruction may develop as a late complication in patients with stable neurologic deficits and chronic immobility of the vocal cords.

Hypokalaemic muscle paresis in children.

Seven children with muscle paralysis due to hypokalaemia are reported. In four the weakness was of asymmetrical and patchy distribution invoking an initial diagnosis of poliomyelitis, while the other three children had symmetrical pareses, a more widely recognized presentation. Involvement of bulbar muscles occurred in three, one of whom also had intercostal weakness. Head lag was present in all. The cause of hypokalaemia was malnutrition and/or diarrhoea, but in only one case was the diarrhoea severe enough to prompt the mother to seek medical advice.

Rehabilitation for postpolio sequelae.

BACKGROUND: Postpolio sequelae (PPS) are new, late manifestations that occur many years after the initial poliomyelitis infection. Recurrence of symptoms and fear of reactivation of the polio virus is particularly distressing to polio survivors. OBJECTIVE: This article outlines the diagnosis, pathophysiology, and management of PPS disabilities using a case vignette. DISCUSSION: Clinical features of PPS include fatigue, joint and muscle pain, new muscular weakness and bulbar symptoms. Diagnosis can be complicated particularly in nonparalytic cases of poliomyelitis. Disabilities in PPS may not

Myasthenia gravis masquerading as post-poliomyelitis syndrome.

A 79-year-old man with previous bulbar poliomyelitis developed dysphagia and was diagnosed as having post-polio syndrome. Over 2 years, his swallowing deteriorated and he suffered an aspiration pneumonia. Only after the subsequent development of fatigue and facial weakness was myasthenia gravis diagnosed. Diagnostic criteria for post-polio syndrome include the exclusion of all other neurological conditions such as myasthenia gravis. Moreover, in any instance where a patient develops new symptoms, it is advisable to reconsider the original diagnosis.

Respiratory function during wakefulness and sleep among survivors of respiratory and non-respiratory poliomyelitis.

The purpose of this study was to determine whether there is a difference in respiratory mechanics and gas exchange between polio survivors and healthy, age-matched controls during wakefulness and sleep. Polio survivors were divided into four groups. The first group included those who had evidence of respiratory muscle involvement originally (PRM) and the second group included those who had bulbar muscle involvement originally (PBM). The third and fourth groups had only limb involvement originally but were separated by absence (PSL) or presence of a scoliosis (PSS) at the time of their evaluati

Pulmonary dysfunction and its management in post-polio patients.

Respiratory dysfunction is extremely common and entails considerable risk of morbidity and mortality for individuals with past poliomyelitis. Although it is usually primarily due to respiratory muscle weakness, post- poliomyelitis individuals also have a high incidence of scoliosis, obesity, sleep disordered breathing, and bulbar muscle dysfunction. Although these factors can result in chronic alveolar hypoventilation (CAH) and frequent pulmonary complications and hospitalizations, CAH is usually not recognized until acute respiratory failure complicates an otherwise benign upper respiratory t

[Proximal spinal muscular atrophy in the adult].

Based on personal experience and recent advances in the molecular genetics of spinal muscular atrophies (SMA), the author reviews the main features of adult SMA predominantly affecting the proximal musculature. Hereditary SMA forms linked to characteristic chromosome abnormalities are distinguished from those that have not been linked to any known gene. Among the first type, recessive autosomal inherited forms linked to chromosome 5 are mentioned and the description is extended to progressive bulbar-spinal amyotrophy and Kennedy syndrome, linked to the X chromosome, whose gene also codifies an

[Familial poisoning with Karwinskia humboldtiana].

The ingestion of ripe fruit of the Karwinskia humboldtiana, a shrub commonly known as tullidora or coyotillo, produces an intoxication described in the literature as a symmetric flaccid paralysis of the hind limbs, progressive and ascendent, that in severe cases may cause bulbar paralysis and death. The cause of an acute accidental intoxication of an entire family is presented here, wherein ten out of thirteen members ingested the ripe fruit of the tullidora. Three died, the father and two daughters. For the first time the toxins determination in blood by thin layer chromatography method is de

Neuropathological findings in West Nile virus encephalitis: a case report.

A 67-year-old woman had fever, myalgias, progressive weakness, and respiratory insufficiency. In 9 days, flaccid areflexic quadriparesis and bulbar palsy developed. She died 26 days after the onset of her illness. Serum and cerebrospinal fluid serology were positive for West Nile virus. Neuropathological study showed changes consistent with a viral encephalomyelitis, similar to poliomyelitis. The brainstem showed neuronal loss and multiple foci of necrosis. The spinal cord showed severe loss of anterior and posterior horn neurons. Immunohistochemistry identified West Nile virus antigens in the

Respiratory paralysis as a presenting symptom in Japanese encephalitis--a case report.

In the literature there are rare reports on Japanese encephalitis (JE) mimicking poliomyelitis or presenting as respiratory paralysis. A case of JE presenting as respiratory paralysis is described. The 22-year-old male was well until five days earlier, when he experienced the acute onset of headache and fever. Four days after onset he developed dysphagia and respiratory difficulty, and a tracheotomy was performed. The following day he lapsed into semicoma and the respiration had to be maintained by an artificial ventilator. He regained consciousness by the 10th day after onset. He became able

Intoxication with buckthorn (Karwinskia humboldtiana): report of three siblings.

A variety of plants and seeds, some of which are used for therapeutic and nutritional purposes, can cause neurotoxic symptoms. The ingestion of the green or ripe fruit of the Karwinskia humboldtiana (buckthorn), a bush known in Mexico as coyotillo or tullidora, causes a flaccid, symmetric, progressive, and ascending palsy of the lower limbs, which, in severe cases, can cause bulbar palsy and death. The neurologic symptoms are similar to those of poliomyelitis, Guillain-Barré syndrome, and other polyradiculoneuritis syndromes, for which it is often mistaken. The clinical diagnosis of intoxicate

[Post-polio syndrome with isolated dysphagia].

The case of a 69 year old, male patient with slowly progressive dysphagia and an acute exacerbation is presented. He was referred to rule out an organic origin to his disorder. Apart from mild residual weakness of both legs after an acute paralytic poliomyelitis at the age of 20, the physical examination was essentially normal. No upper motor neuron pathology was found. Electromyography showed prevalent spontaneous and chronic denervation of bulbar and skeletal muscles. X-ray cinematography revealed abnormalities of bolus passage. On the basis of history and the benign clinical course over a 2

Long term non-invasive ventilation in the community for patients with musculoskeletal disorders: 46 year experience and review.

BACKGROUND: A study was undertaken to assess the long term physiological and clinical outcome in 79 patients with musculoskeletal disorders (73 neuromuscular, six of the chest wall) who received non-invasive ventilation for chronic respiratory failure over a period of 46 years. METHODS: Vital capacity (VC) and carbon dioxide tension (PCO(2)) before and after initiation of ventilation, type and duration of ventilatory assistance, the need for tracheostomy, and mortality were retrospectively studied in 48 patients who were managed with mouth/nasal intermittent positive pressure ventilation (M/N

[Genetic changes in strains of poliovirus type 2 isolated from patients with vaccine-associated paralytic poliomyelitis].

INTRODUCTION: Poliomyelitis is currently a rare disease in developed countries, where only vaccinal strains seem to be in circulation, which replace wild poliovirus. Nevertheless, it is still a serious disease for children in underdeveloped countries of Asia and Africa. PATIENTS AND METHODS: We analysed nine strains of poliovirus type 2 isolated from the faecal matter of patients with vaccine associated paralytic poliomyelitis (VAPP), from the beginning of anti polio vaccination campaigns in our country. These strains were submitted to sequencing of a fragment of 114 base pairs from the 5 NTR

ENTEROVIRAL SYNDROMES IN TORONTO, 1964.

Virological or serological investigations of 72 children in Toronto and environs, who were hospitalized between January and October 1964 with a variety of syndromes, revealed evidence of enteroviral infection in 29 subjects. Coxsackie B2 was the dominant enterovirus, being isolated from feces and/or cerebrospinal fluid (CSF) of three children with aseptic meningitis, three with pleurodynia, one with myalgia and one with pericarditis; four additional patients showed rising antibody titres to this virus. Coxsackie B1 virus, which has not been isolated in Toronto since 1950, was recovered from fe

Benign monomelic amyotrophy of lower limb: a rare entity with a characteristic muscular CT.

Six patients presented with amyotrophy confined to a single lower limb and characterized by insidious onset, slow progression and later stabilization. Wasting was out of proportion with disability and there were no sensory, pyramidal tract or bulbar signs. All cases were sporadic, and there was no history of poliomyelitis. CK, anti-ganglioside antibodies, motor and sensory conductions were normal. Quantitative EMG and muscle biopsy revealed neurogenic features also in clinically unaffected limbs. Muscular CT showed selective or predominant, asymmetrical involvement of posterior leg muscles and

Detecting lower motor neuron dysfunction of the pharynx and larynx with electromyography.

This study assessed the utility of clinical electromyography (EMG) for detecting lower motor neuron (LMN) or upper motor neuron (UMN) dysfunction affecting the intrinsic muscles of the larynx and pharynx. Twenty-nine subjects were examined; their clinical diagnoses included perioperative nerve injury, cerebral infarction, and lateral medullary infarction. Resting activity, motor unit action potential (MUAP) morphology, and MUAP recruitment were evaluated in every case. Medical records (excluding EMG data) were analyzed for clinical evidence of LMN or UMN dysfunction in the intrinsic muscles of
