Pediatric multisystem inflammatory overlap presentations represent a significant diagnostic dilemma. These transient, reactive conditions frequently mimic severe, chronic autoimmune, neoplastic, or demyelinating disorders, leading to potential diagnostic confusion. Following an antecedent respiratory infection, post-infectious immune activation can manifest as a complex cluster of concurrent multi-organ findings, including incidental neuroinflammatory imaging changes, hepatic steatosis, gastrointestinal inflammation, and transient autoantibody positivity. Distinguishing these benign, self-limi

Post-infectious syndromes including post-treatment Lyme disease syndrome (PTLDS), long COVID, myalgic encephalomyelitis/chronic fatigue syndrome (ME/CFS), and pediatric acute-onset neuropsychiatric syndrome (PANS)/pediatric autoimmune neuropsychiatric disorders associated with streptococcal infections (PANDAS) share overlapping clinical phenotypes characterized by fatigue, cognitive dysfunction, sleep disturbance, and neuropsychiatric symptoms. Increasing evidence suggests that immune dysregulation-including persistent inflammation, autoantibody production, and cellular immune dysfunction-may

Post infectious encephalomyelitis and multiple sclerosis are both inflammatory demyelinating disorders of the central nervous system. Whereas multiple sclerosis is a multi phasic disease with recurrent episodes disseminated in time and place, post infectious encephalomyelitis is usually considered to be a monophasic illness. This study used serial brain MRI to clarify whether the latter hypothesis holds for the long term. Post infectious encephalomyelitis was defined as the development of a central nervous system white matter disorder occurring in close temporal relationship with a viral, bact

Anti-N-methyl-d-aspartate (anti-NMDA) receptor encephalitis is a type of autoimmune encephalitis characterized by the rapid onset of neurologic dysfunction. Several studies have reported the important role of post-infectious effect in its pathogenesis. Herein, we report on a 68-year-old woman diagnosed with anti-NMDA receptor encephalitis with concomitant intracranial cryptococcal infection, who was followed up for two years and demonstrated clinical improvements after effective anti-fungal therapy. This case may help expand the clinical spectrum of post-infectious immune-mediated etiology of

Child sexual abuse (CSA) has been strongly associated with a range of psychological and physical problems in childhood and adulthood, such as anxiety, post-traumatic stress disorder (PTSD), and infectious diseases. Despite the strength of these associations, no studies to date have investigated psychobiological processes that might underlie the relationship between CSA and physical health problems occurring during childhood, such as infectious diseases. The goal of the current study is to evaluate PTSD as a potential mediator between CSA and the occurrence of infectious diseases among children

BACKGROUND: Insurance claims data have been used to inform an understanding of Lyme disease epidemiology and cost of care, however few such studies have incorporated post-treatment symptoms following diagnosis. Using longitudinal data from a private, employer-based health plan in an endemic US state, we compared outpatient care utilization pre- and post-Lyme disease diagnosis. We hypothesized that utilization would be higher in the post-diagnosis period, and that temporal trends would differ by age and gender.
METHODS: Members with Lyme disease were required to have both a corresponding ICD-9

OBJECTIVE: Functional dyspepsia is a common gastrointestinal disorder. The pathogenesis of functional dyspepsia remains unclear. Functional dyspepsia may begin after a bout of gastroenteritis (post-infectious functional dyspepsia) or de novo (nonspecific functional dyspepsia). The aim of this study was to investigate the prevalence and probable mechanisms of post-infectious functional dyspepsia.
MATERIAL AND METHODS: Functional dyspepsia patients with a history of unsanitary food intake and acute gastroenteritis 6-12 months ago were enrolled. (13)C-UBT confirmed absence of H. pylori infection

The evaluation of the hypothesis of an infectious etiology to some neurological diseases comprises four different situations. First, numerous neurological diseases have an obvious infectious etiology (encephilitis, meningoencephilitis). Second, some neurological disorders were primarily suspected to be have an infectious etiology, but the causative microorganism was discovered either longtime after the princeps description of the disease (neurologic Whipple disease, due to Tropheryma whippelii), or at the same time (tropical spastic para-paresis secondary to HTLV-I infection). Third, for other

INTRODUCTION: Pediatric Acute-Onset Neuropsychiatric Syndrome (PANS) is characterized by abrupt onset of obsessive-compulsive disorder or eating restriction along with the abrupt onset of other co-occurring symptoms (tics, behavioral and cognitive regression, etc.). PANS is thought to be a post-infectious immunopsychiatric disorder, although as with most post-infectious disorders, it is challenging to establish a causal relationship with proposed infectious triggers. Intravenous immunoglobulin (IVIG) can modulate inflammation and support the elimination of infection and has been used for treat

BACKGROUND: Neonatal disorders such as post-infectious hydrocephalus exhibit a higher incidence in Africa, where the intricate relationships between genetic ancestry, environmental exposures, and other risk factors likely contribute to the increased incidence.
METHODS: To start to characterize the common genetic architecture of Ugandan infants, we analyzed genome sequencing data from 1,030 Ugandan infants recruited from studies targeting neonatal sepsis and hydrocephalus. We employed genetic admixture analysis and integrated geospatial data to examine the relationships between genetic backgro

Bronchiolitis may be encountered in numerous clinical circumstances. Previous history of smoking, infections, toxic exposure, immunodeficiency, chronic inflammatory disorders or transplantation must be known. CT findings consist in centrilobular micronodules with sharp or ill borders of various density and/or a mosaic attenuation with expiratory air trapping. Tree-in-bud pattern suggest an inflammatory or infectious bronchiolitis. The associated presence of bronchiectasis and bronchiolectasis must be considered. Imaging-pathologic correlations will be presented for inflammatory bronchiolitis (

Postviral syndromes are heterogeneous multisystem diseases without a uniform etiology that occur as a result of acute viral infections. During the COVID-19 pandemic, the number of patients increased dramatically due to infections with severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2). This is known as postacute sequelae of COVID-19 (PASC), with many cases also meeting the criteria for myalgic encephalomyelitis/chronic fatigue syndrome (ME/CFS), the most severe form of a postviral disease, characterized by severe fatigue, postexertional malaise (PEM), unrefreshing sleep, neurocogniti

Evidence is emerging that IBS, a hitherto enigmatic disorder thought to be predominantly related to psychological factors, has a microorganic basis in a subset of patients with the disease. Post-infectious IBS (PI-IBS), commonly of the diarrhoea-predominant subtype (defined as new development of IBS following acute infectious diarrhoea), is one such condition known to occur in up to 10-30% individuals after acute gastroenteritis. However, following acute infectious gastroenteritis, patients can also develop post-infectious malabsorption syndrome (PI-MAS), popularly known as tropical sprue. As

ChAdOx1 nCoV-19 vaccine (AstraZeneca) has been associated with rare adverse events following vaccination such as thrombosis with thrombocytopenia syndrome, inflammatory myositis, and autoimmune encephalitis. Para-infectious or post-infectious myelin oligodendrocyte glycoprotein-associated disorders (MOGAD) have been reported in association with coronavirus disease. However, post-vaccine MOGAD (PV-MOGAD) has not yet been reported. Here, we report three cases of PV-MOGAD who presented with a prolonged severe headache after the ChAdOx1 vaccination. Other features of MOGAD such as optic neuritis o

This study focused on identifying risk factors for adolescent post-infectious chronic fatigue syndrome (CFS), utilizing a prospective, nested case-control longitudinal design in which over 300 teenagers with Infectious Mononucleosis (IM) were identified through primary care sites and followed. Baseline variables that were gathered several months following IM, included autonomic symptoms, days in bed since IM, perceived stress, stressful life events, family stress, difficulty functioning and attending school, family stress and psychiatric disorders. A number of variables were predictors of post

The aim of the study presented here was to set up and standardize a semiquantitative polymerase chain reaction method for monitoring Epstein-Barr virus (EBV) DNA levels in blood and saliva samples from transplant recipients and to determine the value of these levels as an early marker for the development of post-transplant lymphoproliferative disorders. EBV DNA load was prospectively measured in 53 adult kidney transplant recipients. Results were correlated with clinical features and degree of immunosuppression. Healthy blood donors and patients with infectious mononucleosis were used as contr

The opsoclonus-myoclonus syndrome (OMS) was diagnosed in nine patients, two men and seven women, varying in age from 34 to 65 years. In two cases the findings indicated a post-infectious form, in three cases there was a malignancy, in one case there was a connection with the use of medication and in three cases the cause was unknown. OMS is a rare neurological disorder with symptoms of rapid, involuntary saccadic eye movements (opsoclonus) and sudden involuntary muscle contractions (myoclonus). It has been associated with infections, malignancies, intoxications and metabolic disorders. In 50%

BACKGROUND: Neurological manifestations of severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) are being widely documented. However, movement disorders in the setting of 2019 coronavirus infectious disease (COVID-19) have been a strikingly less discussed topic.
OBJECTIVES: To summarize available pieces of evidence documenting de novo movement disorders in COVID-19.
METHODS: We used the existing PRISMA consensus statement. Data were collected from PubMed, EMBASE, Web of Science, and Scopus databases up to the 29th January, 2021, using pre-specified searching strategies.
RESULTS: Twe

The direct impact and sequelae of infections in children and adults result in significant morbidity and mortality especially when they involve the central (CNS) or peripheral nervous system (PNS). The historical understanding of the pathophysiology has been mostly focused on the direct impact of the various pathogens through neural tissue invasion. However, with the better understanding of neuroimmunology, there is a rapidly growing realization of the contribution of the innate and adaptive host immune responses in the pathogenesis of many CNS and PNS diseases. The balance between the protecti

Opsoclonus Myoclonus (OMS) is a rare neurological disorder which appears to be the result of an autoimmune dysfunction. It is an extremely rare condition, affecting as few as 1 in 10,000,000 people every year. It affects 2 to 3% of children. Opsoclonus Myoclonus, which manifests itself as post infectious encephalopathy, occasionally along with HIV infection, post Streptococcal infection, West Nile virus encephalitis and Rickettsial infection, most often presents as a paraneoplastic syndrome, which is especially caused by occult neuroblastoma which is commonly seen in childhood and occurs in ad
