Nursing interventions to prevent or relieve postoperative nausea and vomiting.

The purpose of this study was to identify documented interdependent and independent nursing interventions to prevent or relieve nausea and vomiting during the first 24 hours following uncomplicated cholecystectomy. A systematic sampling method was used to select 40 hospital records from 1986 to 1988. Results showed that nausea and vomiting was documented in only two PACU records and nine postoperative nursing unit records. Documentation was typically brief or absent altogether. There was no way to determine if this meant no occurrence of nausea and vomiting or just that interventions were not

Functional Magnetic Resonance Imaging of Brain Function and Emergence Agitation of Patients with Dexmedetomidine-Assisted General Anesthesia under Comfortable Nursing Intervention.

In order to explore the effects of dexmedetomidine (DEX) on functional magnetic resonance imaging (fMRI) and emergence agitation of patients who underwent general anesthesia surgery with sevoflurane under comfortable nursing intervention, 66 patients who received upper abdominal surgery were selected as research objects. According to nursing and anesthesia methods, the patients were randomly divided into control group (routine nursing and anesthesia), group A (routine nursing and DEX-assisted anesthesia), and group B (comfortable nursing and DEX-assisted anesthesia). The differences in the bra

Delayed onset of malignant hyperthermia in the intensive care unit: a rare but life-threatening disorder.

Malignant hyperthermia (MH) has been described as a rare inherited autosomal dominant metabolic syndrome triggered in patients after exposure to certain inhalation anesthetics and depolarizing muscle relaxants. Typically MH is seen within the operating room and postanesthesia care environment. This case study will discuss a patient with an atypical MH crisis in the intensive care unit. Pathophysiology, genetic basis of and testing for MH-susceptibility as well as the role of the critical care nurse in the management of this potentially fatal disorder will be discussed.

Oh, by the way, the patient is pregnant!

Approximately one to three per cent of pregnant women undergo surgery that is unrelated to their pregnancy. In Canada this represents about 5,000 patients each year that present unique challenges to the perioperative nurse and the entire surgical team. Approximately five to ten per cent of these patients are involved in trauma, which causes 46.3% of maternal deaths. A small percentage of elective procedures are carried out in the first trimester, before the patient herself is aware of the procedure. The majority of procedures are required for urgent and emergent conditions that require surgery

Detection and treatment of perioperative corneal abrasions.

Corneal abrasion is the most common ocular injury occurring in the perioperative period. Corneal abrasion may occur during general anesthesia, monitored anesthesia care, regional anesthesia, or in the immediate recovery period. This injury is not usually apparent until the patient is in the PACU, and the perianesthesia nurse may be the first clinician to detect this complication. Preventive measures and vigilant care can help reduce the incidence of corneal abrasion in susceptible patients. Early detection and prompt intervention may help reduce the incidence of ocular morbidity. The purpose o

Exposure of postoperative nurses to exhaled anesthetic gases.

UNLABELLED: The National Institute of Occupational Safety and Health (NIOSH) has established recommended exposure limits of 25 parts per million (ppm) as a time-weighted average for nitrous oxide and a ceiling of 2 ppm for volatile anesthetics. We quantified exposure of postanesthetic nurses to exhaled anesthetic gases. This study was conducted in the postanesthesia care unit (PACU) of a medium-sized hospital. PACU air exchanges averaged 8 vol/h; however, much of this air was recirculated. We evaluated 50 adults anesthetized with either isoflurane (n = 19) or desflurane (n = 31). Roughly half

Help! This postanesthesia care unit patient is hypothermic.

Hypothermia is a common intraoperative and immediate postoperative complication. Hypothermia causes morbidity from shivering, hypotension, cyanosis, and respiratory diseases. In severe cases, bradycardia, premature ventricular contractions, and even ventricular fibrillation may occur. The various causes of excessive heat loss, both intraoperatively and during the postanesthesia period, the methods of prevention, and the rewarming measures are important factors in administering patient care. Predisposing factors are large exposed body surface areas, open body cavities, prolonged exposure to low

Enhancing Voiding Trial Adherence Using QR Code-Based Video Education.

IMPORTANCE: Voiding trials are critical for identifying postoperative urinary retention following pelvic surgery. However, inconsistent nursing execution due to confusing protocols or complex electronic order sets can lead to poor adherence, patient harm, and unnecessary catheter use. OBJECTIVE: The objective of this study was to determine whether providing nursing staff with quick response (QR) code-linked instructional videos improves adherence to postoperative voiding trial protocols. STUDY DESIGN: A pre-post quality improvement study was conducted over 3 months at 2 tertiary care hospita

The Other Side of the Difficult Airway: A Disciplined, Evidence-based Approach to Emergence and Extubation.

Successful intubation of the patient with a difficult airway marks the beginning of the challenges facing the nurse anesthetist charged with delivery of that patient's anesthesia care. Too often, on successful intubation of this patient, the anesthetist and other members of the perioperative team may relax too much. Substantial planning and consultations have been employed to achieve the successful intubation of the patient with the difficult airway. Yet frequently, the final aspect of any general anesthetic-extubation-does not receive sufficient planning even though there continues to be a la

Evolution of postanesthesia care units: a legacy of politics, funding, and patient safety concerns.

Postanesthesia care units or PACUs (earlier called recovery rooms) flourished after World War II as social, political, and economic influences advanced enormous changes in health care expectations and delivery in the United States. Innovations in science and technology, facilitated by increased federal funding of scientific research in the years preceding and during the war, heralded a postwar need for greater complexity of hospital care safely delivered by trained personnel. PACUs began to open in new and modernized postwar hospitals as a means to safely provide more complex postoperative car

Using simulation training to improve perioperative patient safety.

Simulation learning provides medical and nursing personnel with the opportunity to develop and refine their skills without putting patients at risk. Faced with ensuring the competence of a large number of new staff members, the management team at one facility implemented a simulation training program. Surgical team members are able to participate in an ongoing program of simulated scenarios involving surgical drape fires and airway fires, cardiac arrest of patients in the supine position and prone position, respiratory depression in the postanesthesia care unit, and malignant hyperthermia. Intravesical antineoplastic therapy following transurethral resection of bladder tumors: nursing implications from the operating room to discharge.

An aging population and latent effects from exposure to carcinogens will likely augment the current trend of increased incidence of urinary bladder cancer. Intravesical antineoplastic therapy is a common treatment for urinary bladder cancer. Transurethral resection of bladder tumors often is followed immediately by the instillation of an antineoplastic agent in the operating room or postanesthesia care unit. Oncology nurses, who have a unique knowledge of safe handling and patient care, can improve staff safety and patient outcomes in several areas of healthcare organizations, as well as reduc

Primary hyperparathyroidism: a case study.

Primary hyperparathyroidism is a hypercalcemic condition generated as a result of overproduction of parathyroid hormone (PTH) by one or more of the parathyroid glands. The cause is usually an abnormal group of cells forming a benign adenoma and rarely carcinoma. The condition is usually discovered by routine serum chemistry analysis showing hypercalcemia, hypophosphatemia, and elevated PTH levels. Elevated 24-hour urine calcium provides further confirmation. During the last decade, three procedures have been developed to help diagnose the affected parathyroid gland(s) in preparation for surgic
