Glaucoma following congenital cataract surgery--the role of early surgery and posterior capsulotomy.

BACKGROUND: To determine the rate of glaucoma following congenital cataract surgery at Moorfields Eye Hospital (MEH), and to investigate potential risk factors for glaucoma in our case series. METHODS: A retrospective case notes review was undertaken of all congenital cataract lensectomies performed at MEH between 1994 and 2000. The following parameters were ascertained: age at surgery, unilateral or bilateral cataract, whether a posterior capsulotomy (PC) was performed at the time of surgery, whether an intraocular lens (IOL) was inserted, duration of follow-up, and if aphakic glaucoma (AG)

Failure of apraclonidine to prevent delayed IOP elevation after Nd:YAG laser posterior capsulotomy.

An elderly pseudophakic man with advanced open-angle glaucoma underwent Nd:YAG laser posterior capsulotomy. Pigment deposits were also removed from the anterior lens surface with laser applications. In spite of pre- and post-laser treatment with topical apraclonidine (Iopidine) and normal IOP 1 hour postoperatively, the patient presented the next day with visual acuity of 20/300, corneal edema, pain, and IOP of 60 mm Hg. Prompt treatment reduced the pressure and normal visual acuity was achieved. While apraclonidine prevents early post-laser elevation of IOP in most cases, some pressure elevat

Posterior capsulotomy in sulcus-fixated versus bag-fixated intraocular lenses in diabetic patients.

Forty-one diabetic patients who had modified J-loop lenses with ten-degree angulation implanted in the ciliary sulcus and 58 diabetic patients who had the same style lenses implanted in the capsular bag had posterior capsulotomy performed on the table or later at predetermined intervals. The operation was performed with a cystotome needle or with the Nd:YAG laser on the first postoperative day or later. The study indicates that the development of new or the worsening of old diabetic iris and retinal changes, as well as cystoid macular edema, were much less if the intraocular lens was implanted

Ciliochoroidal effusions after neodymium:YAG posterior capsulotomy: association with pre-existing glaucoma and uveitis.

Two patients with a history of glaucoma and a propensity for uveitis developed ciliochoroidal effusions following Nd:YAG laser posterior capsulotomy. Both patients experienced a waxing and waning course with eventual resolution of symptoms after a steroid regimen. The ciliochoroidal effusions were presumed to be secondary to uveitis induced by the YAG laser surgery. The role of glaucoma, previous surgeries, and an open posterior capsule is uncertain but may have predisposed these patients to ciliochoroidal detachment. We advise caution in performing YAG posterior capsulotomy in patients with g

Axial myopia increases the risk of retinal complications after neodymium-YAG laser posterior capsulotomy.

We conducted a retrospective analysis of Q-switched neodymium-YAG laser capsulotomies performed in 122 eyes between April 1984 and June 1987. Retinal complications occurred in 3 (2.5%) of 121 eyes followed up for 1 year and in 2 (3.6%) of 55 eyes followed up for 2 years. Four eyes developed rhegmatogenous retinal detachments and 1 developed an acute symptomatic retinal tear. No patients developed clinical cystoid macular edema (visual acuity 20/30 or worse). These retinal complications were significantly correlated with axial myopia, preexisting vitreoretinal disease, male gender, younger age,
