Two cases of isolated dissection of the posterior cerebral artery responsible for ischemic stroke are reported. Nine probable or certain additional cases were found in the literature. Most patients were female. The clinical presentation consisted of ischemic symptoms in 6 patients, subarachnoid hemorrhage in 4, and both symptoms in 1. Headache was present in almost all patients. The most prevalent angiographic picture was an eccentric stenosis followed by fusiform dilatation of the P1-P2 segments of the posterior cerebral artery. A double-lumen sign was observed in 3 cases. A favorable outcome…

PURPOSE: Variations of the posterior cerebral artery (PCA) are rare, excluding aplasia or hypoplasia of the P1 segment. To the best of our knowledge, there are few reports of complete duplication of the PCA.
METHODS: Herein, we report a case of complete duplication of the PCA diagnosed by 1.5 T magnetic resonance angiography.
RESULTS: A 55-year-old woman visited our hospital for cerebrovascular disease screening. Magnetic resonance angiography revealed two right PCAs with similar diameters. One PCA originated as the P1 segment of the PCA branching from the basilar artery, and the other was t…

This study analyzed the topography of acute ischemic stroke in the posterior cerebral artery (PCA) territory. We studied 84 patients with unilateral ischemic PCA stroke. Patients were classified according to lesion levels as cortico-subcortical (superficial), combined (cortical and mesodiencephalic) or isolated thalamic. To receive a lesion map, data from acute MR and CT imaging were normalized and labelled automatically by mapping to stereotaxic anatomical atlases. Cortical lesions accounted for 41.7%, combined for 36.9%, and isolated thalamic lesions for 21.4%. The maximum overlay of ischemi…

BACKGROUND: The posterior cerebral artery (PCA), as a paired terminal branch of the basilar artery, runs through four segments (P1 to P4) to the inferior and medial surfaces of the occipital and a part of the temporal lobes. There are many PCA variants in its course. The literature data indicated that a fenestration of the PCA was very rare and that its clinical significance is unknown. The purpose of this investigation was to present the frequency, location, and some morphological features of PCA fenestration in the prenatal and postnatal period.
METHOD: Using brain bases of 468 (200 fetal a…

BACKGROUND: Approximately 5-10% of all acute ischemic strokes (AIS) occur in the territory of the posterior cerebral artery (PCA). Little is known about intravenous thrombolysis (IVT) in this infarct subgroup in terms of outcome and intracerebral hemorrhage rates. The aim of our study was to evaluate differences between supratentorial PCA infarcts and anterior circulation infarcts regarding baseline characteristics, stroke severity, outcome, safety and clinical findings, which would implicate a change in the existing thrombolysis practice in patients with PCA stroke.
METHODS: All patients wit…

Previous reports of outcome after endovascular treatment of posterior cerebral artery (PCA) aneurysms have been limited. This study aimed to describe the clinical outcome and angiographic results obtained in the endovascular therapy of PCA aneurysm over a six year period. Endovascular treatment strategies were determined. A retrospective analysis was performed on 18 patients with PCA aneurysms evaluated at Beijing Tiantan Hospital between December 2000 and December 2006. Nine patients presented with subarachnoid hemorrhage (SAH): one of these patients had additional seizure and hemiparesis, on…

BACKGROUND: The role of neuroimaging in acute posterior cerebral artery (PCA) territory infarction is less well appreciated compared with the anterior cerebral circulation because PCA infarction occurs less frequently and more often is associated with limited neurologic deficit not qualifying for thrombolytic therapy. On occasion, however, hemiparesis and/or visual field defect accompanies PCA infarction that would warrant thrombolytic therapy.
REVIEW SUMMARY: As neuroimaging plays a central role in the diagnosis and treatment of acute stroke in the thrombolytic era, a series of case studies …

We present the case of a traumatic posterior cerebral artery pseudoaneurysm of a 29-year-old man due to a penetrating stab wound to the brain. The patient was brought to the emergency room in coma. The neurological examination revealed a Glasgow Coma Scale Score of 8 and left hemiplegy. The initial CT scan showed right temporal lobe haemorrhage with penetration in the lateral ventricle and subarachnoid haemorrhage. The cerebral angiography performed after three weeks, revealed a high PCA pseudoaneurysm. After surgery, a pseudoaneurysm was demonstrated in the histological study.

Infarction of the posterior cerebral artery may present only with signs of agitated delirium and an acute confusional state. In the absence of other prominent neurological deficits, this can be easily mistaken for toxic-metabolic encephalopathy, head trauma, post-ictal confusion, or a psychiatric disorder. Appropriate head imaging studies are important to detect an illness that might otherwise be missed and left untreated.

BACKGROUND: Brain infarction secondary to stenosis of the posterior cerebral artery is uncommon. We report a patient with midbrain infarction and symptoms of both Benedikt's and pupil-sparing oculomotor palsy syndromes secondary to posterior cerebral artery stenosis.
CASE DESCRIPTION: A 51-year-old woman developed diplopia, left ptosis, and right hemiataxia and hyperactive tendon reflexes. Pupils were equal and reactive. Radiological examination revealed stenosis of the posterior cerebral artery and left-sided midbrain infarct.
CONCLUSIONS: Stenosis of the posterior cerebral artery may cause…

BACKGROUND: Hemiplegia is a rare manifestation of posterior cerebral artery occlusion. The acute clinical picture may be difficult to differentiate from occlusion of the middle cerebral artery. A mechanism for the hemiplegia has not been conclusively determined.
CASE DESCRIPTION: We describe a patient with hemiplegia secondary to posterior cerebral artery occlusion by an embolized fragment of a prosthetic valve. Computed tomographic scan showed the foreign body just distal to the origin of the posterior cerebral artery with infarction of its vascular territory. These findings were later confi…

BACKGROUND: The most common neurovascular variant is the fetal posterior cerebral artery (FPCA), in which the P1 branch is absent or hypoplastic, and the majority of P2 supply is derived from the anterior circulation. While there are reports of hyperplastic anterior choroidal arteries (AChA) with supply to the temporo-occipital and calcarine regions, no reports of a duplicated FPCA exist.
METHODS: This case report describes a patient with a ruptured right FPCA aneurysm. Digital subtraction angiogram (DSA) revealed an artery with origin distal to the FPCA associated with the aneurysm. This was…

BACKGROUND AND PURPOSE: In the anterior circulation, the hyperdense middle cerebral artery (MCA) sign is a well-established marker for early ischemia. Similarly, the hyperdense basilar artery sign or the MCA "dot" sign may be a diagnostic clue for basilar artery or distal MCA branch thrombosis. The purpose of this study was to define the hyperdense posterior cerebral artery (PCA) sign and determine its incidence, diagnostic value, and reliability as a marker for ischemia in the territory of the PCA.
METHODS: Cranial computed tomographies (CCTs) of 48 patients with proven acute ischemia (<12 h…

Variants of the cerebral arterial circle, both common and rare, are frequently documented. The authors present a distinct case involving a 54-year-old male patient who underwent a computed tomography angiogram (CTA), which was analyzed using planar slices and 3-dimensional reconstruction. In this case, the left posterior cerebral artery (PCA) received the posterior communicating artery (PComA) 7 mm distal to its origin from the basilar artery (BA). In addition, an accessory vessel originated 4.2 mm distal from the PComA origin, branching from the left internal carotid artery (ICA). This vessel…
