The effect of the regular administration of ibuprofen on pain following abdominal surgery was investigated in a double-blind placebo-controlled study. The analgesic action of ibuprofen was measured by a reduction in the amount of morphine self-administered from an On-Demand Analgesic Computer. Pain scores, nausea scores and blood loss were similar in both groups, but patients receiving ibuprofen demanded significantly less morphine in the 24 h after surgery.

The aim pursued in postoperative analgesia is to mobilize patients, to prevent thrombo-embolism, to improve subjective well-being, and to reduce the stress reaction caused by pain. In sufficiently high doses, metamizol is equally as suitable for the treatment of postoperative traumatic pain as are opioids. Comparative studies provide evidence that metamizol is no less potent in its action than pethidine, and that in cases of pain in the head and neck region it is superior to buprenorphine in its usual dose. It can also be used to supplement opioid medication where this does not produce satisfa

BACKGROUND: High intensity of acute postsurgical pain is one of the strongest predictors of chronic postsurgical pain (CPSP). We investigated if different types of patients with distinct combinations of initial pain intensity and rate of pain resolution exhibit different risks for increased pain intensity six months after surgery.
METHODS: Data from 174 patients were examined using growth mixture analysis by means of structural equation modeling.
RESULTS: Three types of patients were distinguished on the basis of acute pain trajectories. The majority of patients (57%) showed an unproblematic

Perioperative analgesia is a major challenge for healthcare providers. Not well-controlled pain after surgery can lead to complications. The worldwide spread of acute pain service (APS) teams has proven to increase the effectiveness of postoperative pain management. This publication aims to describe the concept, structure, quality criteria, and implementation models of acute pain management services based on relevant publications. A systematic literature review was conducted in the relevant databases by the PICO technique, and 263 abstracts were evaluated. The service is available in many coun

PURPOSE: Recent public opinion has focused on the lay perception that postoperative pain control is inadequate. The Joint Commission on Accreditation of Healthcare Organizations has attempted to address this issue by mandating patient-reported numeric pain scores as part of postoperative pain assessment. No studies exist to validate these scores in orthopaedics. The purpose of this study was 3-fold: (1) to determine which preoperative variables were associated with elevated postoperative pain scores, (2) to identify whether pain scores correlated with more traditional signs and symptoms of pai

PURPOSE OF REVIEW: The aim of this review article is to present current recommendations as well as knowledge gaps and controversies pertaining to commonly utilized postoperative pain management after solid organ transplantation in the abdominal cavity.
RECENT FINDINGS: Postsurgical pain has been identified as one of the major challenges in recovery and treatment after solid organ transplants. Many perioperative interventions and management strategies are available for reducing and managing postoperative pain. Management should be tailored to the individual needs, taking an interdisciplinary a

Ulnar wrist pain, caused by a broad spectrum of bone and soft tissue injuries, is the most common clinical condition of the wrist. Multiple surgical techniques and their variants in the treatment of these injuries are constantly evolving. Postoperative evaluation of the wrist for many surgeons is limited to serial clinical and radiographic monitoring. However, imaging methods such as ultrasonography, computed tomography (CT), magnetic resonance imaging (MRI), and arthrographic techniques (arthrographic CT and arthrographic MRI) play a fundamental role in diagnosing and managing postsurgical co

INTRODUCTION: Pain management after surgery continues to be suboptimal; there are several reasons including lack of translation of results from basic science studies and scientific clinical evidence into clinical praxis.
OBJECTIVES: This review presents and discusses basic science findings and scientific evidence generated within the last 2 decades in the field of acute postoperative pain.
METHODS: In the first part of the review, we give an overview about studies that have investigated the pathophysiology of postoperative pain by using rodent models of incisional pain up to July 2016. The s

PURPOSE OF REVIEW: Postoperative pain remains poorly treated in many patients. One reason is the inadequate use of nonopioid analgesics. This review examines the most recent findings on nonopioid analgesics and how these translate into clinical practice.
RECENT FINDINGS: Commonly used nonopioid analgesics are paracetamol, NSAIDs and metamizol (dipyrone). For paracetamol, the efficacy is obviously inferior to NSAIDs and the risk of adverse events might have been underestimated in the past. For NSAIDs, there are increasing data supporting similar efficacy of nonselective and cyclooxygenase-2 se

BACKGROUND: Despite pharmacological treatment, 70-80% of patients report moderate to severe pain after surgery. Because nicotine has been reported to have analgesic properties in animal and human volunteer studies, the authors assessed the analgesic efficacy of a single 3 mg dose of nicotine nasal spray administered before emergence from general anesthesia.
METHODS: The authors conducted a randomized, double blind, placebo controlled trial of 20 healthy women (mean age 45 (SD 8) yr) who were to undergo uterine surgery through a low transverse incision. After the conclusion of surgery but befo

BACKGROUND: Deficits in quality of postoperative pain management are not caused by the complexity of the medical problem or shortage of suitable drugs but by difficulties in organization and hospital structures. Moreover, there is no consensus on how to measure the quality of pain management. Quality management programs consist of strategies to overcome such difficulties and to increase quality continuously. This study reports the implementation of a quality management program to improve postoperative pain management at a university hospital.
METHODS: An interdisciplinary task force consistin

The aim of the postoperative pain therapy is besides the amelioration of the patients well-being, also lessening of unwanted vegetative reactions and avoidance of the need for excessive care situations. There are numerous ways of therapy and medication at our disposal for this purpose. The first line of drugs are the analgesically most active opiates; they can be combined with antiphlogistics and antipyretics, in order to decrease the specific opiate-induced side effects. Besides the parenteral way the epidural application is being used. Local anesthetics are a valuable alternative to the syst

A study of the attitudes of general surgical patients to the management of their postoperative pain showed that although 86% initially expressed satisfaction with their postoperative pain relief, a quarter of these did in fact have moderate, severe or unbearable, unalleviated pain. These, together with those who expressed dissatisfaction with their pain relief, constituted one third of the total number, indicating that a problem of postoperative analgesic management existed in the hospital. As a result, techniques of continuous intravenous infusion of narcotics and more frequent use of regiona

Postoperative pain relief was studied in 280 patients undergoing various kinds of surgery, e.g., thoracic, upper and lower abdominal, perineal, obstetric, and orthopedic. Morphine, 2 or 4 mg, was given after surgery through an indwelling epidural catheter. Excellent analgesia was noted in 87% of patients; only 3.5% of patients were dissatisfied. A single injection gave complete pain relief for the entire postoperative period in 30% of cases; in the remaining patients the mean duration of analgesia was 10.7 hours (SD +/- 4.3). Plasma morphine concentrations recorded after 2-mg doses suggest a r

Postoperative pain relief is only apparently an easy task. A brief survey of literature investigating the discomfort experienced in the postoperative phase is all but encouraging. One can identify four basic problems in obtaining adequate results by delivery of analgesic drugs: a) the biological variability among individual patients, b) the unpredictable uptake of the drug administered intramuscularly, c) the time lag involved between request by the patient and the subsequent administration of a single dose and d) the lack of knowledge about the nature of the discomfort and its remedies.
